Abstract

ABSTRACTIntroduction:  The use of gabapentin  for the treatment of neuropathic  is found to provide a good effect of the therapeutic  while the side effects still can be tolerated.  This study aims to describe the difference in pain score reduction and incidence of drug side effects in patients who suffering from neuropathic  pain receiving Gabapentin  therapy of 300 mg in once a day, compared  with the use of Gabapentin  of 300 mg in twice a day. The re- search  was conducted  at Neurology  Department  of Hasanuddin  University  Hospital  in Makassar  from 19th   of April to 8th  of June 2016. Methods:  It was a prospective  cohort study using 50 samples selected with the consecutive  sampling technique. The samples were divided into two groups. The first group (n=25) received  Gabapentin  of 300 mg in once a day; while the second group (n=25) received Gabapentin of 300 mg in twice a day. Each group was examined  in terms of the pain score reduction  using the Visual Analog Scale (VAS) and the incidence of side effects using a questionnaire after the first and the second  week of the medical  treatment.  Results:  The results  revealed  that the use of Gabapentin  of 300 mg in once a day and Gabapentin  of 300 mg in twice a day for two weeks in reducing pain scores (examined based on VAS) resulted in the value of 4.12 and4.92 with P=0,000 (P<0,05). This showed a significant difference between the two groups. Conclusion: The use of Gabapentin of 300 mg in twice a day for two weeks, as a therapy of neuropathic  pain  at Neurology  Departement of Hasanuddin  University  Hospital  in Makassar,  resulted in a better decrease in pain scores. Meanwhile,  side effects with the highest  percentage  found  in the group using Gabapentin  of 300 mg in once a day, in comparison with the group using Gabapentin 300 mg in twice a day were drowsiness and dizziness. Keywords: Neuropathic  pain, Gabapentin,  Dosage Variation, Pain scores

Abstract

ABSTRACT Introduction: One of the potential genetic links to Age-related macular degeneration (AMD) is vascular endothelial growth factor (VEGF). The aim of this study was to investigate the association of polymorphism promoter VEGF gene and VEGF serum level with the type of AMD. Methods: Genomic DNA was extracted from peripheral blood of 66 samples, consist of wet and dry AMD, and age-matched controls. DNA was subjected to polymerase chain reaction amplification at the position -460 coupled with the restriction fragment length polymorphism. VEGF serum level was measured by Enzyme-linked Immunosorbent Assay. Results: The polymorphism promoter VEGF gene demonstrated significant association with the type of AMD (p=0.004, OR=3.1 95% CI 1.1-8.6) and the risk for increasing VEGF serum level (p=0.011, OR=3.2 95% CI 1.18.9). Despite there was no significant association between dry and wet AMD with VEGF serum level (p=0.081), but this study revealed significant increased of VEGF serum level in AMD patients, compared to those in control’s serum (p<0.001). Finally, there was coassociation between polymorphism and VEGF level with the type of AMD (p<0.001). Conclusion: This study reveals the significant association between promoter VEGF gene polymorphism and VEGF serum level with the type of AMD of Indonesian patients.  Keywords: AMD, VEGF gene polymorphism; VEGF serum level.

Abstract

ABSTRACTMicroalbuminuria (MA) and Left Ventricular Hypertrophy (LVH) are as independent predictor to cardiovascular morbidity and mortality in Hypertensive (HT) patients. Hipertension causes target organ damage such as LVH and chronic kidney disease. Microalbuminuria one of the earliest indications of kidney injury in patients with HT. Left Ventricular Hypertrophy is adaptation process to maintain the stability of the contraction function, but will be a pathological process. The pathogenesis relationship between MA and LVH in essential HT is unclear. The presence of MA and LVH in HT show the high risk for cardiovascular events. This study aimed to knows the factors that contribute the incidence of MA and LVH in HT patients. Using observational study with cross sectional design in HT subjects, at Wahidin Sudirohusodo Hospital, aged over 18 years old. Measuring of Body mass index were performed on all study subjects, MA refers to an abnormally increased excretion rate of albumin in the urine and Echocardiography examination used to evaluation of LVH. This study showed Obesity influence the incidence of LVH in HT subject.   Key words: Microalbuminuria, Left Ventricular Hypertrophy, Hypertension.

Abstract

ABSTRACT  Introduction: HIV / AIDS is a viral infection that causes decrease of immunity system characterized by decrease CD4+ T lymphocyte counts. Low CD4 T-cell counts are considered due to destruction of lymphocytes T by human immunodeficiency virus. The spiritual psychotherapy is a form of psychiatric treatment that provides peace, and happiness of the soul. Peace of mind is expected to generate individual immune response. Objectives: Objective  to determine the effect of spiritual psychotherapy to CD4 T-cell count. Methods: This is a quasi experimental study pretest-posttest control group design of 40 sample with HIV / AIDS who were selected by purposive sampling. This research was conducted in the HIV / AIDS Working Group and hospital inpatient BLU. Dr. Wahidin Sudirohusodo. Makassar. The samples were divided into two groups, the treatment group of 10  inpatient who received intensive spiritual psychotherapy, 10 sample from Lapas Bolangi and 20 control group who did not receive supportive psychotherapy. Statistical analysis using the Wilcoxon test. Results: The results of the analysis of CD4 + serum between the treatment and control groups was significantly different (p<0.05). After treatment, the CD4 T-cell count in the inpatient group increased serum from 15 to 160, but  in the other groups (group bolangi from 446.50 to 365.50 and the control group from 424.5 to 201.0) was decreased, especially in the control group (Tables 2 and 3). Table 4 shows that of the 10  inpatient samples group, all had increased levels of CD4 + serum, whereas in the group Bolangi only 3 samples (30%) among the 10 sample of the control group , of 20 sample only 1 person (5%) had increase of CD4 + serum , and 19 sample (95%) had decrease.   Keywords: Spiritual Psychotherapy – relaxation - CD4 count

Abstract

ABSTRACT Introduction: Estimated blood loss is an estimation of how much blood is loss during surgery. Surgical procedure requires a preparation of blood stock, but the demand for blood often larger than the actual blood used. This predicament happens because there is no blood requirement protocol being used. This study aims to determine the estimated blood loss during craniotomy procedure and it’s conformity to blood units ordered for craniotomy procedure. Methods: This study is a retrospective study using data from Wahidin Sudirohusodo General Hospitals’ medi- cal records in the period of January 2010-December 2012. We found 89 craniotomy patients that meet the inclusion criteria comprise of 66 men and 23 women. Results: This study showed that the average estimated blood loss in craniotomy was 3.20 units of whole blood (1120 ml) and the average of the demand of blood ordered was 4.0 units of whole blood (1400 ml). There was no mismatch between the amount of blood ordered for surgery and estimation of blood loss (p=0.73). Conclusion: Estimated blood loss in craniotomy procedure in Wahidin Sudirohusodo hospital, Makassar is 3.20 units of whole blood (1120 ml) was in accordance with amount of blood ordered for surgery. Keywords: Estimated blood loss, craniotomy
