Abstract

The turnover rate of employee in Surabaya Surgical Hospital increased over the last three years since 2014 to 2016 that was 13,67% in 2014, 13,69% in 2015 and 16,91% in 2016. The high employee turnover rate of Surabaya Surgical Hospital was probably due to high turnover intention of employees in some work-units at Surabaya Surgical Hospital on 2016 with the individual average value by 50.80%. The high value of turnover intention could give an impact in employee performance. Holzer, Stoll, and Wissoker (2004) stated that employees would reflect the high turnover intention by not fulfilling the tasks and responsibilities at work. Turnover intention did not only impact the final performance, but also affected employees commitment to work. This research aimed to analyze the effect of turnover intention on employee performance. The sample of this research was Surabaya Surgical Hospital employees with minimum six months working period. The method of this research was analytical observational study with cross sectional design. The result showed that turnover intention affected the employee performance with the p value or significance was 0.03 and smaller than α = 5% (0.05). It can be concluded that turnover intention has an effect on employee performance.Keywords: turnover intention, work commitment, work performance

Abstract

Lack of patient visits to FKTP causes capitation not being optimally utilized. The purpose of this research is to analyze the causes of the unfullfilled capitation payment indicators based on fulfillment of service commitment at FKTP. The type of research used is observational with cross sectional design. Approach in the research is quantitative and qualitative. The research used total sampling, where population is secondary data covering result of indicator achievement from 62 Puskesmas, 31 DPP, and 97 Klinik Pratama in Surabaya. This research concluded that indicator of AK and RPPB at FKTP still not fulfill target of safe zone. AK can be caused by the lack of participants’ knowledge of FKTP services, limited resources, and BPJS Kesehatan online application that sometimes have interruptions. FKTP needs to provide promotions and education to participants’, add resources and provide training to staff, advocate with health offices, and cooperation with other parties to help provide facilities. Whereas the target indicator of RPPB has not been achieved due to lack of socialization of Prolanic. Abstract

Patient safety incidents type of clinic administration are incidents that occur in the process of patient identification, handover, agreement, informed consent, queuing, referral, admission, discharge of patients from inpatient, transfer of care, division of tasks, response of emergency. Based on patient safety incidents reports of RSU Haji, there were 12 patient safety incidents type of clinic administration (standard 0% or zero accident) during January 2014-June 2016 in inpatient unit. This research aimed to analyze the correlation between nurse’s individual characteristics with patient safety incidents type of clinic administration. This research was an observational descriptive study with cross sectional design. Questionnaires were distributed to 48 nurses that divided into 6 inpatient units. The results of this research showed that the lower the nurses knowledge about patient safety and patient safety incidents type of clinic administration, the higher the nurse’s work fatigue, the lower the nurse’s motivation toward to the application of patient safety program, the higher the tendency of patient safety incidents type of clinic administration occured in inpatient unit of RSU Haji Surabaya. The conclusion of this research are knowledge, work fatigue, motivation of nurse have a correlation with patient safety incidents type of clinic administration in inpatient unit of RSU Haji Surabaya.  Keywords: patient safety incidents, clinic administration, nurse, hospital

Abstract

Turnover can be detrimented to the organization both in terms of cost, resources, and motivation of employees. The purpose is to determine the relationship variables on job satisfaction and commitment with the intention to leave. The method is an observational study with cross-sectional study. Samples from this study are employees Pura Raharja Hospital Surabaya who meet the criteria that respondents who are still within reach of researchers and respondents are willing to fill out a questionnaire study. Samples were calculated using simple random sampling and obtained by 50 respondents and the value of α = 0.05. The results of linear regression showed that the intention to leave the job is job satisfaction, career development job satisfaction and organizational commitment. The value of significance job satisfaction that is equal to 0,004 jobs, job satisfaction significant value that is equal to 0,04, career development and organizational commitment of significant value that is equal to 0.027. The variables that do not affect the intention to leave are a co-worker job satisfaction, job satisfaction wages, job satisfaction supervision and work commitments. The conclusion of this study is Job satisfaction affects the intention to leave and also commitments affect the intention to leave. Keywords: commitment, intention to leave, job satisfaction, turnover

Abstract

Based on the standart that Public Health Center “X” completeness of SOAP, KIE, and ICD X must be 100%. The aim of research was to analyze how the completeness of writing SOAP, KIE, and ICD X inGeneral Poly and Health of Mother and Child Family Planning Public Health Center “X”. This study was descriptive research with 500 medical records that consist of 260 for January and 240 for February as sample and they taken by random sampling. The result showed that completeness of SOAP, KIE, and ICD X on January in General Poly were 48% and decrease on February became 45,8%.While Health Mother and Child Family Planning Poly showed that completeness on January were 97,8% and increase on February became 98,6%. The incompleteness of medical records can be influenced by several factors is compliance the health workers who responsible in filling the medical records and they have multi job in Public Health Center “X” also the patient was increase. To minimize the incompleteness of SOAP, KIE, and ICD X, medical staff needs to expose by socialization of medical record to remember their responsibilty of their job description.Keywords : Completeness, medical record, Public Health Center

Abstract

Plannig of the health program in district level has been more dominated by top-down process. The targets specified from top level are ususally based on national projections and incompatible with the real situation in this area. The goal of this research is to analyse annual health planning in Immunization program Dinas Kesehatan Kota Surabaya. The research is a descriptive study with qualitative data. Data collected with in-depth interview to person in charge of program and document observation in City Health Department of Surabaya. The result of this research are: (1) Technical planning such as amount of target person of immunization, amount of logistic and vaccine needs, financial planning has been done, but the administrative implementation has not been fully done showed by some documents which actually do not exist. (2) Non-technical planning that is based on the problems of last year have not been implemented. (3) POA (Plan of Action) has never been made in immunization program, so it is necessary to form POA raw format and guidelines for the preparation of planning. (4) Steps in planning has not conform the rules justified. (5) Annual Training is required for the planning in immunization program for each person in charge. Keywords: district health department, immunization program, planning

Abstract

Surabaya had more than 1.600 pharmacists in 2016. However, based on the data there were 144 pharmacists that did not have practice licenses in hospitals and primary health care in Surabaya. It meant that the implementation of The Regulation of Health Minister Number 889 Year 2011 was ineffective. The purpose of this research was describing the pratice lincesing of pharmacist in hospitals and primary health care in Surabaya and the factors that influenced the problem. This research was an observational descriptive study with cross sectional method. Primary data were collected through observation, interview and discussion with the staff in charged in Surabaya Public Health Office. While the secondary data could be achieved from official documents, reports, and archives. Comparing the number of pharmacists in the hospitals or primary health care with the minimum manpower standard of pharmacist based on The Regulation of Health Minister Number 56 Year 2014, the result showed that 33 hospitals had not fulfilled the standard minimum number of pharmacists which mean they had not made their practice licenses. Therefore, the given recommendation was strengthening Public Health Office’s role to direct warning and punishment toward hospitals and pharmacists.Keywords: pharmacist, practice license, hospital and primary health care

Abstract

Inaccurate drugs planning may cause excessive budget, stagnant, and stockout. This research aims to analyze drugs planning in medical logistics of Rumah Sakit Islam Jemursari Surabaya. This research is a descriptive study using cross sectional design. Primary data were obtained through observations and interviews. Secondary data were obtained through drugs planning and drugs consumption data from November to December 2016. Afterward, data were processed and found that 40% drugs item on November 2016 have greater amount of consumption than their planning and 65% drugs item on December 2016 have greater amount of consumption than their planning. The method of drugs planning that used in medical logistics of Rumah Sakit Islam Jemursari Surabaya was consumption method, but it was not calculate the average of drugs consumption. After the calculation based on Keputusan Menteri Kesehatan Nomor 1121 Tahun 2008, there is only 20% drugs item on November 2016 which have greater amount of consumption than their planning and 20% drugs item on December 2016 which have greater amount of consumption than their planning. This research showed that the calculation of drugs consumption for medical logistics in Rumah Sakit Islam Jemursari Surabaya need repairement.Keywords: consumption method, drugs planning, hospital

Abstract

Preliminary survey result showed the number of employees knowledge about the event held by Undaan Eye Hospital in Surabaya reached 52,8%. It means more than 20% employees are not aware of activity outside the work which that number can affect to decreasing employee commitment to the organization. Job satisfaction factors divided into 5 there are pay satisfaction, promotion, coworker, supervisor and the job characteristic. The aim of this study is to analyze the influence job satisfaction to employee commitment. Observational analytic was used in this research with the cross sectional design. The data was obtained through questionnaire with simple random sampling involving 62 employees as the sample of this research. The data obtained were analyzed using logistic regression test univariate (α=0,05) to examine the influence of Job satisfaction to employee commitment. The test result indicates that job satisfaction significantly influenced employee commitment at significance value of 0,02. Job satisfaction factors that there are influences toward employee commitment are employee’s pay satisfaction (sig=0,001), promotion (sig=0,005), supervisor (sig=0,004) and job characteristic (sig=0,001). On the other hand, the employee’s coworker satisfaction does not influence the employee commitment.Keywords: employee commitment, job satisfaction, job satisfaction factors.

Abstract

The era of National Health Security (NHS) enacted tiered referral system start in primary health care. High number of referrals at Public Health Center (PHC) X indicates that the implementation of referral system did not go well. The objectives of this study is to confirm implementation of refferal system in PHC in the era NHS based on the regulations and policies. This was descriptive observational research and cross sectional design with implementation research method. This research used triangulation to get deep information toward referral documents include referral data, registration book, medical records, referral letter, interview with refferal officer, and observation without intervention. This research was conducted at PHC X Surabaya on the Februari-March 2017. The results showed implementation of refferal system has appropriate with the regulations and the guidelines. The implementation has appropriate with 7 section on The Regulation of Health Minister No.1 of 2012. The implementation has appropriate with Guidelines of National Health Refferal include requirement to refer patient, refferal clinical procedure, and refferal administrative procedure, however there is not appropriate implementation: informed consent, contact to refferal destination healthcare, and print out of refferal paper. Recommendation for PHC is to do informed consent and print refferal letters 2 copies.Keywords: national health security, public health center, refferal system
