Abstract

Objective: To assess the safety and immunogenicity of Haemophilus influenzae type b/polyribosylribitol phosphate-Tetanus (Hib/PRP-T) liquid vaccine in healthy adults.Methods: An open label prospective intervention phase I study was conducted in Dr. Hasan Sadikin General Hospital from November to December 2010. Healthy adults aged 18−40 were eligible to participate. Participants received one dose of Hib/PRP-T liquid vaccine. Blood samples were taken before, 4 days and 1 month after vaccination. For a 28-day period following vaccination, solicited adverse events were collected in the subjects’ diary and assessed afterward. Results: Neither local reactions nor immediate systemic events were observed during a 30-minute period after immunization. There were no serious local or systemic reactions in this study. All of local and systemic reactions observed were slight, transient, self-limiting in time, without lasting for more than 72 hrs. after the administration of the vaccine, and resolved without any medical intervention. Hematologic and biochemical indices before and 4 days after vaccination showed in normal limits. All subjects (100%) reached protective levels of antibodies (seroprotectivity) against Hib. All subjects demonstrated antibodies performing high bactericidal activities 1 month after immunization. Conclusions: This study demonstrated that liquid Hib/PRP-T vaccine is highly immunogenic and have a beneficial safety when administered to healthy adults. Comparison of Superficial Surgical Site Infection in Simple Interrupted and Hybrid Mattress Suture after Posterior Approach for Spine Surgery

Abstract

Objective: To compare the incidences of superficial surgical site infection after posterior approach procedures in simple interrupted suture groups compared with hybrid mattress suture group. Methods: A number of 38 patients who underwent posterior approach procedure were randomized into two groups. First group was sutured using simple interrupted suture while the second was using hybrid mattress suture. Bivariate statistical analysis was performed using Chi square test and Mann Whitney test. The superficial surgical site infection incidence was evaluated and recorded for both groups.Results: A superficial surgical site infection was found in 26.3% of the simple interrupted group and no infection was found in the hybrid mattress group. Statistical analysis showed a p value of 0.046.Conclusions: Compared to simple interrupted suture, hybrid mattress suture has less superficial surgical site infection in posterior approach for spine surgery.Keywords: Hybrid mattress suture, simple interrupted suture, superficial surgical site infection, wound healing DOI: 10.15850/ijihs.v3n2.589

Abstract

Objective: Rapid and accurate TB diagnostics play an important role in detecting the disease. Currently, antigens secreted (ESAT-6, CFP-10 and MPT64) by M. tuberculosis and encoded by genes “Region of Difference” (RD)1, RD2 and RD3 give an opportunity for rapid TB diagnosis. Genomic region RD1-RD3 was deleted in M. bovis BCG strains and absent in mycobacteria other than M. tuberculosis. This property is advantageous because it enable to create a specific diagnostic tools for M. tuberculosis infection. The aim of this study is to determine the validity of TB antigens cocktail (ESAT-6, CFP-10 and MPT64) for pulmonary tuberculosis and TB meningitis diagnosis. Methods: This is a descriptive observational study design. The study was conducted at the Clinical Pathology Laboratory of Dr. Hasan Sadikin General Hospital during September 2012 until March 2013 for the pulmonary tuberculosis study and from January 2014 to May 2014 for the TB meningitis study. The TB antigen cocktail rapid immunochromatography (ICT) test was done on all of the samples. The sputum and CSF were cultured as gold standards. Results: There were 149 pulmonary and 41 TB meningitis subjects. The sensitivity of TB antigens cocktail rapid ICT for diagnosing pulmonary tuberculosis was 95.7% with a specificity of 87.2%. Of 41 TB meningitis subjects, based on Marais criteria, there were 6 (16%) subjects with a definite TB meningitis, 26 (63%) subjects with probable TB meningitis and 9 (21%) subjects with possible TB meningitis. The sensitivity and specificity of TB antigens cocktail rapid ICT for TB meningitis diagnosis were 83.3% and 68.5% respectively. Conclusions: In this study, rapid ICT TB antigens cocktail (ESAT-6, CFP-10 and MPT64) from sputum sample has good validity for diagnosing a pulmonary tuberculosis, and from CSF sample has moderate validity to diagnose TB meningitis. Abstract

Objective: Preeclampsia is still a problem in Indonesia due to the high maternal and neonatal morbidity and mortality. Placental soluble fms-like tyrosine kinase-1 (sFlt-1), an antagonist of vascular endothelial growth factor (VEGF), and placental growth factor (PIGF), is considered as one of the etiology factors that cause endothelial damage in preeclampsia due to increased sFlt-1 level modulating vascular endothelial integrity. This study aims to analyze sFlt-1 and sFlt-1 mRNA levels in severe preeclampsia and normal pregnancy, and the correlation between both in occurrence of severe preeclampsia.Methods: This is a cross-sectional analytic observational study involving 18 subjects with severe preeclampsia and 19 subjects with normal pregnancy as controls who met inclusion and exclusion criteria. Levels of sFlt-1 and sFlt-1 mRNA were measured with ELISA and RT PCR. Statistical analysis was performed with Chi square test, Fisher’s exact test, T-test, Mann-Whitney test, and Spearman’s rank correlation test.Results: This study showed no significant difference (p>0.05) in characteristics of maternal age, gestational age, and parity in both study groups. Mean level of sFlt-1 mRNA in severe preeclampsia was higher (6.3404 pg/mL) compared to its level in normal pregnancy (5.9701 pg/mL). There is a not significant (p>0.05) positive correlation between sFlt-1 mRNA with sFlt-1 level in normal pregnancy, and a not significant (p>0.05) negative correlation between both level in severe preeclampsia.Conclusions: sFlt-1 mRNA levels in severe preeclampsia are higher than its level in normal pregnancy. There is no correlation between sFlt-1 mRNA level and sFLt-1 protein level in severe preeclampsia. There is a not significant positive correlation between sFlt-1 mRNA with sFlt-1 level in normal pregnancy, and a not significant negative correlation between both in severe preeclampsia.Keywords: Normal pregnancy, severe preeclampsia, sFlt-1 mRNA DOI: 10.15850/ijihs.v3n2.586

Abstract

Objective: The objective of this study is to examine the relation between nutrition and brain memory. Qualified human resource is the most valuable national asset. Adequate nutrition and regular exercise provide higher achievement to create healthy and smart generations.Methods: The method applied in this study was experimental laboratory. This study was conducted on Juni 2011–Juli 2012 at the Biomedical and Biochemistry laboratory, Faculty of Medicine, Universitas Padjadjaran. The subjects were 8 weeks approximately 200–250 grams, male Wistar rats. The examination involved 56 male Wistar rats divided into 8 groups with different treatments. The groups which received exercise treatment were also given Glycine max and Phyllanthus niruri intakes. Results: NR2B gene expression changes can be described as the following results: (1) without practicing exercise (3,8) and after exercises (4,6); (2) Glycine max intake minus exercise (2,86) and Glycine max intake and exercises (3,17); (3) Phyllanthus niruri intake minus exercises (4,7) and Phyllanthus niruri intake and exercises (4,9); and (4) Glycine max and Phyllanthus niruri composition intake minus exercises (3,14) and Glycine max and Phyllanthus niruri composition intake and exercises (4,83).Conclusions: This study determines that exercises and Phyllanthus niruri intake enhance NR2B gene expressions. Glycine max inhibits the NR2B gene expressions. Glycine max and Phyllanthus niruri composition intake and either with or without practicing exercises enhance NR2B gene expressions. Therefore, practicing exercises and Phyllanthus niruri intake might cause brain cell apoptosis while Glycine max intake inhibits brain cell apoptosis. Abstract

Objective: To evaluate the efficacy of superoxide dismutase (SOD) in lung function (FEV1 reversibility) and respiratory symptoms (drug scores, symptoms scores) in asthmatic and house dust mite allergic children receiving house dust mites immunotherapy.Methods: Forty subjects aged 6–17 years old with asthma and tested positive for house dust mite allergy on skin prick test, and received immunotherapy were enrolled in this study. All subjects completed clinical based assessments and diary-based assessments for drug and symptom scores. Following a four-week baseline assessment, all subjects were randomized to receive SOD or placebo. Respiratory symptoms (drug and symptoms score) and FEV1 were evaluated at the end of the 1st, 2nd, 3rd, and 4th weeks after randomization. Drug score, symptoms score and FEV1 reversibility test results were analyzed using a Paired t test and repeated measure of ANOVA. Results: There was a significant difference in drug scores, symptoms score and FEV1 reversibility test outcomes between SOD and placebo. SOD group showed a significant decrease in all outcome measures compared to those in placebo group. Conclusions: The use of SOD as antioxidants is effective in accelerating symptom relief for children with asthma and house dust mite allergy receiving house dust mite immunotherapy. Relationship of Prostate-Urethral Angle and Intravesical Prostatic Prtusion with International Prostatic Symptoms Score and Uroflowmetry in Benign Prostatic Hyperplasia Patients

Abstract

Objective: To investigate the correlation between Intravesical Prostatic Protrusion (IPP) and Prostate Uretral Angle (PUA) in Benign Prostatic Hyperplasia (BPH) patient with Lower Urinary Tract Symptoms (LUTS) on International Prostatic Symptoms Score (IPSS) and uroflowmetry.Methods: A cross sectional study was done to 71 BPH patients with LUTS who attended Urologic Clinic in Dr. Hasan Sadikin General Hospital and matched the inclusion and exclusion criterias. The Intravesical Prostatic Protrusion and PUA were measured using transrectal ultrasound and the degree of LUTS was measured using IPSS and uroflowmetry. Patients were classified based on PUA (<350 dan ≥350) and degree of IPP (grade 1< 5mm;II 5-10mm;III >10mm). Data were analyzed using Spearman correlation test. Results: Prostatic-Urethral Angle has a strong correlation with IPSS total and voiding (0.670 and 0.715) but the correlation was weak with IPPS storage (0.381). Furthermore, PUA has a strong negative correlation with Qmax dan Qave values. The bigger the PUA, the smaller Qmax dan Qave values. IPP has a strong correlation with IPSS total and voiding (0.645 and 0.662), but has moderate correlation with IPSS storage (0.442). Furthermore, IPP has no correlation with the values of Qmax, Qave, Voided volume and PVR.Conclusions: Prostatic-Urethral Angle and Intravesical Prostatic Protrusion have a strong correlation with the occurence of LUTS in BPH patients. Penetrating Wound in the Skull by a Sharp Metal Object

Abstract

Objective: Penetrating skull injury (PSI) is a neurosurgical emergencies. Most PSI cases reported were caused by gunshot wounds. Although the diagnosis and management of those cases has been described, the actual cases, especially in our center, were quite rare.Methods: A craniectomy debridement with a supine position was performed and removal of the object was achieved. Intraoperatively, we succed control the bleeding as expected.Results: The patient was operated and hospitalized in our center; physical examination revealed neurologically intact before and after the removal of the object. The patient was sent home without any neurological deficits or other complications.Conclusions: We reported a case of penetrating skull injury in a 5-year-old girl caused by a scissors. Penetrating skull injuries were interesting due to its mechanism, management and complications.
