Abstract

Myocardial Infarction is the death of myocardial cells as a result of continuous insufficient oxygen caused by decreasing or the non- existance of blood flow to myocardium as the result of the narrowing or the blockage of coronary artery. This triggers a heart attack that forces the treated in the hospital. Te development of the diagnosis and treatment of the severe heart disease involves sophisticated equipment with high cost and expensive drugs. In the drugs treatment, more than one kind of drugs is given with the result, that can make bigger risk of side effects. The research intends to analyzed the cost that covers the type of component cost and the mount of each component and to know the drug’s side effect. The research was done at Panti Wilasa Citarum Hospital Semarang as a concurrent research by following the doctors and making a dialogue with the patient of acute myocardial infarction. There were four patients acute myocardial infarction during the period of February – April 2009. The result of the research shows that the average doctors fee is 11,54%, the average cost of lodging is 18,21%, the average cost of treatment is 1,15%, the average cost of drug is 14,15%, the average cost of laboratory is 18,39%, the average cost of non drug is 36,56%. Out of four acute myocardial infarction patients, one patient accepted excessive drug while one patient accepted insufficient drug. All of the patients felt side effect of drug, that was the decrease of the blood tension, bleeding and anemia. Key words : cost analysis, drug use evaluation, acute myocardial infarction.

Abstract

Hepatic Cirrhosis represents the end stage of any chronic liver disease. In recent years, important advances have been made in the management of cirrhosis. Patients with cirrhosis are at high risk for other complications of cirrhosis. Thus, preventive measures should be undertaken with the aim of reducing morbidity and improving survival. Based on the background, it is necessary to evaluate drugs usage in patients with hepatic cirrhosis which include : appropriate drugs, appropriate dosage, drug interactions and the effectiveness of the therapy. The study was performed retrospectively through reviewing medical notes of 50 patients with hepatic cirrhosis who hospitalized at the RSUD Tugurejo Semarang period July 2009 – June 2010 The results showed that Patients who received improper drug selection (70%), appreciate dosage (76%), over dosage (18%), subtherapeutic dosage (6%), and potentially drug interaction (18%). 45 patients (90%) could go home better, 1 patient (2%) forced to go home, 1 patient (2%) getting worse and were sent to another hospital, 3 patients (6%) were dead.
