Abstract

ABSTRACTSmoking inhalation consists of about 4685 chemical product with nicotine, carbonmonoxide, and tar are three main product. There are 400 product can be influence our body, 100 make toxic effect and 43 can be carsinogenic. The effect smoking inhalation to inhibit callus formation is not clearly understandWith The post-test only control group design thirty two Rattus norvegicus were divided into 4 groups. The control groups were sacrificed at 8th and 21st day , treatment groups were sacrificed 8th and 21st day.All were performed open fractured and imobilisation with cast. After sacrificed bone density was measuredwith DBM Sonic and osteoblas count was performed.The result of these study show that there was significant difference between the treatment group andnon treatment group (p<0.05) and there was no significant difference between treament group (p>0.05) for bone density, and there was significant difference between treatment group (p<0.05) for osteoblast.Keywords : smoking inhalation, callus formation, experimental study

Abstract

A B S T R A KLatar Belakangz Benang yang ideal seharusnya mempunyai beberapa sifat l<aral<teristil< yaitu: dapatdigunakan pada semua jenis operasi (all-purpose), mudah dilihat, mudah diikat, clapat membentuk simpul yang kuat, mempunyai tensile strength yang tinggi, tidak atau sedikit menimbulkan reaksi jaringan, tidak mengundang infeksi, mudah olisterilkan, mudah didapatkan dan relatif murah. Tempi tidak ada satupun bahan material benang yang dapat memenuhi semua sifat karakteristik tersebut. Sangatlah penting untuk oliketahui dan dimengeiti sifat dasar dari bahan material benang yang akan digunakan agar dapat memilih benang yang tepat, sehingga dapat memperoleh hasil bedah mikro yang Iebih memuaskan. Benang yang biasa digunakan di bldang bedah mikro adalah benang nonabsorbable balk benang nylon maupun polypropylene karena kedua jenis bahan material benang nonabsorbable ini sudah dibuktikan efektif secara klinis. Dalam studi ini, dilakukan perbandingan antara benang polypropylene B-0 dengan nylon 8-0 paola mikroanastomosis end-to-end arteri femoralis kelinci. Desain Penelitianz Penelitian animal experimental.Metode: Tiga puluh arteri femoralis dari 75 ekor kelinci jantan New Zealand dengan berat badan sekitar2000 - 2500 gram digunakan dalam penelitian ini, Kelincl tersebut secara acak dibagl dalam tiga kelompok. Tiap kelompok terdiri dari 5 ekor kelinei, Arteri femoralis l<anan ditranseksi secara tajam dan dianastomosis secara end-to-end dengan menggunakan benang monofilamen polypropylene 8-0, Arteri femoralis kiri ditranseksi secara tajam olan dianastomosis seeara end-to-end dengan menggunakan benang monofilamen nylon 8-0. Semua waktu operasi yang dibutuhkan dicatat, Kemudian dilakukan pemeriksaan secara histologisterhadap pembentukan oklusi trombus 24 jam pasea-anastomosis (kelompok 7), pemeriksaan secarahistologis terhadap reendotelisasi 7 hari pasca-anastomosis (kelompok 2), dan pemeriksaan secarahistomorfometri terhadap reaksi jaringan 74 hari pasca-anastomosls lkelompok 3), Analisis Statistlk: SPSS Version 77 with T-Test dan Chi-square.Hasilz Tidak ada perbedaan yang bermakna terhadap waktu operasi yang dibutuhkan, pembentukan oklusitrombus dan reendotelisasi antara kelompok yang menggunakan benang polypropylene 8-0 dengankelompok yang rnenggunakan benang nylon 8-0. Benang polypropylene 8-0 menimbulkan reaksi jaringan yang lebih seclikit dibanding benang nylon 8-0 dengan perbedaan yang bermakna.Keywords: nylon, polypropylene, nonabsorbable, bahan material benang, monofilamen

Abstract

ABSTRACTIntroduction : The past epidemiologic studies did not have definite explanation about the effect of consuming Mefenamic Acid on Longitudinal Bone Development.Objective: To explain the effect of Mefenamic Acid on Longitudinal Bone Development Methode 2 The design of this study was The Separate pretest. Posttest Only Control Group Design. The research was done using 40 one month old male wistar strain white rats. These rats were randomly divided into four groups. They consists of one pretest group. one control group and two treatment groups. The pretest group directly measure body weight and length of femur (K0). The control posttest group were given orally 2 ml of distilled water/200 g BW/day (Ki). The treatment groups weregiven mefenamic acid orally in the dose of 27 mg/200 g BW/day (K2). and 54 mg/200 g BW/day (K3) respectively. all of which were dissolved in 2 ml of distilled water. After 4 weeks. the length of femur. length of growth plate and number of longitudinal cell of growth plate are noticed. the measurement was undertaken using manual and light microscope. The result was subsequently showed descriptively and analyzed using Manova. It showed in all of experimental animals with had a significant difference to control (p<0.05). Pair wise comparisons showed that significant difference (p<0.05) occurred in all of groups except between mefenamic acid 27 mg and mefenamic acid 54 mg that not significant difference. Linear regression analysis resulted in linear equation that showed all of variables had a significant difference ( p<0.05) . This study show that mefenamic acid treatment cause decrease of femur length. decrease of growth plate length and decrease number of growth plate cell. The following studies with different methods and measurement devices needed as comparison as well as to know the doses that can effect the Longitudinal Development of Growth plate andincrease doses of mefenamic acid will make decrease biggerKeywords; mefenamic acid. chondrocyte. growth plate. experimental study.

Abstract

AbstractBackground: Antibiotioimpregnated beads have been approved for standard treatment of orthopaedic infection. in developing country, Septopal® is one of many types of commercial local antibiotic beads available on the market, unfortunately this implant does not seem to be cost effective, therefore, the orthopaedic surgeon make this kind of beads by himself by handroll.Methods: The antibiotic rapid release is maximum amount of antibiotic that release from beads in 24 hours, and it induenced by porosity and permeability. it will observe by measuring clear zone of inocullant agar of Staphylococcus aureus 252953 in 24 hours. This in vitro study described the comparison of rapid releasebetween handmade gentamycin beads or fosfomycin sodium beads and commercially beads product bymeasuring clear zone of inoculated Staphylococcus aureus 252953 agar over 24 hr.Results: Serial data of inhibitonr zone from both beads will be evaluated and compared. The outcomes wereanalyzed by SPSS for Windows 11.5 program and statistically tested by using Kruskall-Wallis ANOVA.There was a significant difference of means of antibiotic rapid release between the three groups, especially between handmade fosfomycin and Septopal®, as well as between handmade fosfomycin and handmade gentamycin. On the other hand, means difference between handmade gentarnycin and Septopal® were not signihcant.Conclusion: Rapid release of handmade fosfomycin is higher than Septopal® as well as handmade gentamycin.Keywords: Beads porosity — Antibiotic rapid release — Clear zone of inoculant agar over 24 hr

Abstract

Stabilitas fiksasi interna menentukan keberhasilan tatalaksana operatif pada fraktur intertrokanter femur. Dimana faktor-faktor yang mempengaruhi kekuatan dari stabilitas fiksasi interna tersebutditentukan oleh :Kualitas tulang, Geometri fragmen fraktur, Reduksi fragmen tulang, Ienis implan dan Penempatan imp|an,Pada fraktur intertrokanter femur jenis implan yang dipakai saat ini pada umumnya menggunakan 135 Angled Blade Plate .Di Divisi Orthopaedi dan Traumatologi FKUI/RSUPN Cipto Mangunkusumo telah dikembangkan jenis pelat daun waru sistem locking Indonesian Patent Nomor ID 0000 6435 yang telah banyak memberikan manfaat untuk fraktur periartikuler namun sampai saat ini belum ada data bimekanik.Oleh karena itu penelitian ini bermaksuol memperoleh data biomekanik pelat daun waru sistim locking dibandingkan dengan 135 Angled Blade Plate terhadap gaya aksial.Metode : Studi eksperimental Post test only control group design. Hasil yang didapatkan dari pemberian gaya aksial sebesar 225.89N dan 338.79N pada tulang sapi dengan garis patah inteitrokanter yang telah dipasang ABP 135 dan Pelat Daun Waru (PDW) . Pada analisa data denga |ndependentTtest dengan P value = 0,010( > 0,05).Kesimpulan: Pelat Daun Waru memberikan fiksasi yang Iebih stabil daripada 135 Angled Blade Plateterhadap gaya aksial pada fraktur intertrochanter femur bovine OTA 31-A2.1.Kata kunci : Pelai Daun Waru, 135 Angled Blade Plate, Fraktur lntertrochanter Femur

Abstract

Objective : To determine the role of hyperbaric oxygen to bone density of the Male Rattus Norvegicus thathave been administered with corticosteroid.Design : Experimental study with Separate-Sample Pretest-Posttest Control Group DesignSetting : Naval Health Department of Surabaya and Animal laboratory of Biochemistry Department of Medical Faculty Airlangga UniversityIntervention : Conducted between lanuary and March 2007. There are three groups of male Rattus Norvegicus consisting of 10 in each group. First group is a control group. Second group is administered with corticosteroid (methyl prednisolon) for 3 weeks. The third group has been given with high dose corticosteroid and hyperbaric oxygen 2.4 ATA for 90 minutes daily for the period of 3 weeks.Result : The measurement of bone density of male Rattus Norvegicus is conducted by the means of BoneDensitometer and the average result of the first group is 1312.200 m/s, while the second group is only1245.200 m/s and the third group is 1305.900 m/s. Analysis is done by using the independent T test between the first group and the second group, and the result is the significantly reduced bone density (p< 0.05). While the result of comparation between the first group and the third group showed insignificant result (p>0.05). The measurement of Alkaline Phosphatase in each group is as follows, the first group 572.700 U/L, the second group 416.100 U/L and the third group 652.300 U/L. The analysis of this measurement is performed with Independent T- test. The result between the first group and the second group is significantly different (p<0.05) while the result between the first group and the third group is insignificantly different (p>0.05)Conclusion : Hyperbaric oxygen inhibits the decrease of bone density in Rattus Norvegicus that is administeredwith corticosteroid.Key words ; Hyperbaric oxygen, corticosteroid, bone density, alkaline phosphatase, Rattus Noniegicus

Abstract

Background: Incidence of subtrochanteric femoral fracture have been increased, approximately 10-34%. These fracture commonly causing morbidity and mortality, especially in elderly patienm. There were many implant able to stabilized these fractures. In Cipto Mangunkusumo General Hospital, developing Locking Clover Plate (LCP) to senie orthopaedic trauma cases. These research purpose to know strength fixation of Condylar Blade Plate (CBP) and LCP. Both of its act as fixed angle plate, and there are no biomechanical datas, especially foraxial, bending and rotation forces that involving proximal femoral motion. Purpose :T0 know whether, LCP able to against axial and bending forces than CBP to applied in mobilization post operatively.Methods: These research is a biomechanical study, using femoral bovin bones as specimens. There are 2groups, first group using CBP, second group using LCP with each of group with 7 samples. Both of groupsloaded with axial and bending forces with Marshal Proving Ring 5000 kgf. After stabilized make a gap 2 cm beneath lesser trochanter, than displacement of its measured using Peacock Dial lndicator. Results: Mean of axial loading with 1 mm displacement of CBP 64,086 N, SD 1 9,67 N, median 63,45 N, range (50,42-77,89) and LCP mean 49,196 N, SD 1 10,96 N, median 45,17 N, range (35,30-69,72). t-test independent significance p=0,019 (<0,05). Axial loading with 2 mm displacement, CBP with mean 93,643 N, SD 19,11 N, median 89,44 N, range (81,92-106,12).LCP mean 79,67 N, SD 1 20,55 N, median 82,63 N, range (46,74-111,76). t-test independence not significance p=O,126 (>0,05).Mean bending loading with 1 mmdisplacement CBP 168,088 N, SD 1 64,67 N, median 159,07 N, range (77,89-270,85). LCP mean 517,93 N,SD 1 133,31 N, median 513,38 N, range (384,17-781,55). t-test independent significance p=0,000 (<0,05).Bending loading with 2 mm displacement of CBP mean 364,47 N, SD 1 164,29 N, median 306,82 N, range (128,13-626,63). LCP mean 636,83 N, SD 1 222,22 N, median 698,21 N, range (225,85-918,00). t-test independent significance p=0,023 (<0,05). Conclusions:LCP have provide stability than CBP in bending force, thus in clinically allow for patient to abduction-adduction motion. There is no statically significance between LCP and CBP in axial force to make displacement 2 mm, both of its, start for partial weight bearing until radiologically callus formation appear. Abstract

Raloxifen is a SERM (Selective Estrogen Receptor Modulator) which have been used for treatment ofosteoporosis cause by menopause. The effect raloxifene on Hampering Decreasing Bone Density in Long—termCorticosteroid Therapy in Male white Rat remain unclear.Thirty two Rattus norvegicus are devided into 4 groups are control group, non treatment group,treatment group one and treatment group two. The study was conduct for 5 weeks and by the end of 5 weeksthe bone density and measured with DBM Sonic and osteoblas count was performed.The Result of these study show that there is significant difference between the treatment group andnon treatment group (p<0,05) and there is no significant difference between tretament group one andtreatment group two. (p>0,05)This experimental study shows that raloxifen could inhibit the decrease of bone density in long termcorticosteroid treatment and have the equal effect compare to alendronate to inhibit the decrease of bonedensity in long term corticosteroid treatment

Abstract

Penilaian pendidikan oerthopaedi dan traumatologi telah berubah perlahan-lahan. Perubahan tersebut harus difokuskan pada penilaian program orthopaedi dan traumatologi pada berbagai metode . kondisi ini telah distimulasi oleh beberap faktor yang meliputi adanya malpraktekyang dilakukan oleh ahli, kebutuhan yang lebih besar akan kenyataan pada penilaian dan terakhir adalah implementasi kurikulum orthopaed dan trumatologi. Tetapi penrting pada aspek klinis penilaian berbasis kompetensi yang masih kurang, karena beberapa area masih tidak dibicarakan. Piramid Miller cukup berguna pada penilaian klinis orthopaedi dan traumatologin guna menentukan kerja penilaian. Tingkat terbawah adalah pengetahuan  (knows), kemudian diikuti oleh kompetensi (know how), performance (show how) dan kerja praktek (does). Semua sistem penilaian harus direncanakan dengan penjelasan tujuan komponen penilaian kurikulum orthopaedi dan traumatologi dengan bentuk lain untuk tes. Oleh karena itu, objektifitas bentuk program kerja dasar penilaian dan ujian akhir masih dipertanyakan tetapi harus reliabel dan valid.

Abstract

Background : External Fixation is a minimally invasive method to maintain reduction with adequate stability. Since 1995, we have used unilateral,  one plane double – threaded bar external fixator ( UOPDBEF) which was developed by the Department of Orthopaedic and Traumatology in the University of Indonesia, but there has been no biomechanical studies on this method. In this study, we evaluated the effects of bar-to-bone distances of 25 mm (group I), 50 mm (group II) and 80 mm (group III) on the stability of this external fixation by measuring the amount of anteroposterior bending forces required to posteriorly-displace fragments. Objectives : To determine the amount of AP bending forces required to procedure posterior displacement of fracture fragment at bar-tobone distances of 25,50 and 80mm.To determine the amount of AP bending forces required to procedure posterior displacement of fracture fragment at bar-tobone distances of 25, 35 45, 55, 65 and 75 mm in sample four.To compare the degrees of stability obtained by fixations with bar-to-bone distances of 25, 50 and 80mm against the imparted anteropoeterior bending forces.   Methods : The anteroposterior bending loads were applied to the UOPDBEF. The loads were applied directly onto a fracture bone. The stiffness values were obtained by measuring displacement of tibial bone bovine fracture gap. Result The average AP bending forces that produced posterior displacement in group I, II and III are 137.84 N/mm ± 8.99, respectively.The average AP bending forces resulting in posteror displacement for each bar-to-bone distance of 25, 35, 45, 55, 65 and 75mm in sample four are 152.68 N/mm, 76.44 N/mm, 42.14 N/mm, and 36.26 N/mm, respectively.There are significant differences in the degrees of stability between group I and II, group I and III, and between group II and III. Conclusion : Bar-to-bone distance significantly influences the UOPDBEF degree of stability against the imparted AP bending force. With greater bar-to-bone distance, less AP bending force is required to procedure posterior displacement of fracture fragment. Thus, the closer bar-to-bone distance is,the more stable the external fixation is.
