Abstract

The continuity of the participation in Prolanis is one of the primary health care services indicators of commitment, so finding out the level of adherence and the factors influencing it is very important. The aim of the study is to investigate the adherence level of Prolanis participanst and its determinants in type 2 diabetes patients at five BPJS primary health care in Bekasi. This is a cross sectional study, using questionnare to 220 participants with type 2 diabetes by simple random sampling method proportionally. Adherence level of the participants is 3.59 out of 6 times. Length of illness, perceived benefits, perceived barriers and the implementation of the program guidelines are correlated with the adherence level and the dominant factor is perceived barriers. Factors in individual and provider can be used as evaluation tools for the primary health care in facilitating the participants’ needs so that it would increase the level of adherence.

Abstract

Health development in the period 2015-2019 is Indonesia Sehat program with the goal to improve the health and nutritional status of the community through health and community empowerment efforts which are supported with financial protection and health care equity. Social Health Security Agency (BPJS) as JKN organizers estimate in 2015 is a deficit of more than 6 trillion rupiahs. In 2016, the deficit is estimated to be 11 trillion rupiahs. This study used a cross-sectional study design. The population in this study were outpatients at the sub-district Kramat Jati and Pasar Rebo primary health care with the total number of 800 people and studied sample of 100 people. Patient satisfaction in this study has mean score of 43,06. Education variable; job and services are not significantly affects patient satisfaction JKN Non PBI at East Jakarta district primary heallth care. In conclusion, dimension of punctuality; convenience in obtaining services and ease in obtaining services most influence on patient satisfaction JKN Non PBI in East Jakarta district primary health care with the p value = 0,000 < 0,05. In addition, politeness and friendliness within the service still have big influence on patient satisfaction bicause it is important factors on patient satisfaction. Therefore, the author gives some recommendations to appropriate and thorough improvement added with upgrading to the punctuality of service, conducting a survey of patient satisfaction by BPJS and primary health care, disseminating the research results regarding the effect of service quality on patient satisfaction level, further and in-depth research needs to be done related to the effect on service quality dimension to JKN patient satisfaction level in all primary health care included in East Jakarta district, especially at primary health care which has never been conducted with survey and research on JKN patient satisfaction level.

Abstract

One of the prevention efforts of medical errors that occur in health services is by identifying the potential failure of the service before the failure occurs. FMEA is one way to identify the risks of failure. Therefore, the authors wanted to find out whether FMEA was effective for reducing medical error based on previous studies. This article was a literature review using references in an online database such as EBSCOhost. The author found 280 articles while searching by using the keyword “FMEA”. After filtered by publication period from 2012 to 2017, full text and language, finally got 7 articles. Finally, the author used the three most relevant literature. FMEA was proven to decrease the potential failure rate after follow-up to failure was done in service process, so medical error can be prevented. In the application of FMEA, bias can occur during the determination of potential failure and determination of scoring on RPN. Therefore, it is expected that the team involved in making FMEA experts in the process of service to be designed. FMEA could prevent medical errors by determining potential failures before failure occurs.

Abstract

The process to establish a planning and budgeting policy in the Directorate for Occupational and Sports Health is very dynamic and it fluctuates according to the goals and priorities of the policy. This research aims to ascertain how far the political aspects influences the establishment process. This research was a qualitative research accomllished through in-depth interviews with related parties. The framework used the planning and budgeting systems model and process/cycle. Results of this research indicated that politics played a major role in the policy establishment process in conflicts, cooperation, and negotiations when setting the size and allocation of the budget and the targets aimed. The main points in the process are the priorities of the program, the fiscal capacity of the nation, and the results of the previous year’s evaluation. We recommend that the human resources of the directorate are equipped abilities to advocate the importance of the occupational and sports health program

Abstract

By 2015 the deaths due to PTM are 68% and projected in 2030 to increase to 74%. Indonesia in 2013 based on Riskesdas data shows that 69.6% of diabetes mellitus and 63.2% of hypertension are still undiagnosed. The governments proactive efforts are through the implementation of Posbindu PTM which shows a very different number of visits in the target area of the Pasir Mulya Public Health Center. The purpose of this study is to know more in the description of management, communication, partnership and innovation in the implementation of Posbindu PTM and the factors that determine the results of the evaluation of the implementation of Posbindu PTM. This study is a qualitative study with in-depth interviews, focus group discussions (FGD), document review and observation at two Posbindu PTM with the highest and lowest visits to people with similar characteristics. The results of this study indicate that human resource management is a contributing factor in the implementation of Posbindu PTM,  communication and innovation need to be supported by partnership. The implementation of Posbindu PTM is supported by the optimization of management, communication, partnership and innovation factors that are interrelated in improving the successful implementation.

Abstract

The rapid change from presidential regulation no.19 year 2016 on health insurance into presidential regulation no.28 year 2016 on health insurance get a big attention. This research was purposed to analyze about health insurance policy which changed very quickly. It changed from presidential regulation no.19 year 2016 into presidential regulation no.28 year 2016 on health insurance. The researcher used qualitative methods. The analysis from the input processing and output showed that the change of presidential regulation is a responsive form from president when he looked public rejection response for the increase of fee. The president extended it through the department of health affairs. In terms of inputs, the resources of this policy change are still very limited, while at the stage of the process there is still a lack of cross-sector coordination with related institutions and difficulties in getting the materials needed in the discussion. Presidential Regulation No.28 of 2016 on Health Insurance that became the output in this change is considered to be able to accommodate the demand of the people but the changes are not in accordance with the actuary calculations. This change of presidential regulation not yet affected to appropriate the fee adequacy on BPJS Implementation. The department of health affairs as a leader of health sector was recommended to increase the cross-sectoral coordination which can manifest the better product of health policy and to complete the policy instrument that yet to be determined. It also used to be concern from the department of health affairs, DJSN and BPJS which explained the increase of fee must be offset by a quality improvement rather than the implementation of national health insurance.

Abstract

Abstract. The Health Social Security Agency (BPJS kesehatan) is one of the government’s efforts to provide quality healthcare for the people. To guarantee its quality and maintain efficiency, a tiered referral system is used. However, there were many problems in its implementation. One of the ways to investigate is to evaluate the non-specialist outpatient referrals (RRNS). Therefore, in March-July 2018, this research was performed in order to analyze whether the system was implemented at Beji and Depok Jaya Health Center. The method used was the qualitative research method, with primary data from in-depth interviews and secondary data from documents. It was discovered that both Centers had optimal RRNS ratios (0%). This was because the doctors at both Centers has provided referrals as indicated and there was feedback on the RRNS total. The civil servant (PNS) doctors at both centers were experienced and skillful, but the number of doctors was insufficient compared to their workload. The non-PNS doctors also were not sufficiently trained. Other problems include insufficient equipment and medication provision. Abstrak. Badan penyelenggara jaminan sosial (BPJS) kesehatan adalah salah satu upaya pemerintah untuk memberikan pelayanan kesehatan yang bermutu bagi rakyatnya. Untuk menjamin kualitas pelayanan yang diberikan sekaligus menjaga efisiensi, maka diterapkan sistem rujukan berjenjang. Tetapi ini mengalami berbagai permasalahan dalam penerapannya. Untuk melihat apa permasalahannya dapat dilihat dari rasio rujukan rawat jalan non-spesialistik (RRNS). Oleh karena itu, penelitian ini dilaksanakan untuk menganalisis apakah sistem tersebut berjalan di Puskesmas Beji dan Puskesmas Depok Jaya pada bulan Maret-Juli 2018. Penelitian ini menggunakan metode kualitatif, dengan data primer berupa wawancara mendalam dan data sekunder berupa telaah dokumen. Hasil penelitian menunjukkan bahwa di Puskesmas Beji dan Puskesmas Depok Jaya memiliki rasio RRNS yang optimal (0%). Hal ini disebabkan karena dokter di kedua puskesmas memberikan rujukan sesuai indikasi dan terdapat feedback mengenai capaian RRNS. Selain itu, dokter PNS di Puskesmas Beji dan Puskesmas Depok Jaya merupakan dokter yang cukup berpengalaman dan terampil, walaupun jumlah dokter di kedua Puskesmas tidak sesuai dengan beban kerjanya. Dokter non PNS juga belum menerima pelatihan yang memadai. Selain itu, peralatan yang tersedia masih kurang lengkap. Obat di kedua puskesmas cukup lengkap namun terkadang terjadi kekosongan obat.

Abstract

The capability of the Internal Government Supervisory Apparatus (APIP) is the ability to carry out supervisory tasks consisting of three interrelated elements: capacity, authority, and competence. The objective of this research is to give policy recommendations for enhanced APIP capability in the Inspectorate General of the Ministry of Health. This research is a descriptive study with qualitative analysis method with in-depth interview and literature study. Results of this research indicate that there are some obstacles: the socialization done only to some employees of Itjen; no Special Team on the process of improving APIP capability; the time and task division is unclear; has no special budget yet; there has not been a derivative rule from the Internal Audit Charter (IAC); no reward and punishment system; no documentation of supervision working papers; the policy has not been internalized. This research concludes that the implementation of the policy has not been reached optimally based on PERKA BPKP Number PER-1633/K/JF/2011. Communication is the most influential factor in the implementation of APIP enhancement policy. The recommendation from this research are consistently socialize to employees within the Inspectorate General, make Standard Operating Procedures (SOP), allocate budget activities in 2018, create memorandum of understanding with other agencies, and self-assessment and program evaluation absolutely must do continuously.

Abstract

Schoolchildren are assets for the development of the nation. Therefore, healthy meals at schools are very important, because for them, it covers about 25-36% of the daily energy requirements. Unhealthy eating habits or consumption of nutrition deficient meals may cause stunting, cardiovascular diseases, cancer, diabetes, and osteoporosis. In the short term it may also cause dental caries, anemia, overweight children and obesity. Around 40-44% of meals at schools do not satisfy the food safety requirements. To increase the quality of the meals at the schools, the community should be empowered to perform independent monitoring of school meals. This is inscribed in the national action policy for safe, good-quality, and nutritious meals for schoolchildren. The aim of this research was to assess the effectiveness of the implementation of the national action policy for the snacks of schoolchildren and the factors that influences it. This was a qualitative research. The data was gathered from in-depth interviews and documentary studies and then analyzed using the policy implementation analysis of Mazmanian and Sabatier. From this research we discovered that the policy did not perform effectively in Batam City. There were no  Procedure Standard Norms NSPK on the roles, tasks, and responsibilities of the implementers, there was no implementation structure, and no support from the Government and the Parliament of Batam City for the monitoring.

Abstract

Abstract. In order to control Antibacterial Resistance at the hospital in Indonesia, the Government of Indonesia established an Antibiotics Resistance Control Committee and issued a Regulation from the Ministry of Health, regulation no 8/2015 on an antibiotics resistance control program at the hospital. This is an investigation on the Antibacterial Resistance Control Program (ARCP) at the Hospital, which compares its implementation at a government hospital and a private hospital, Persahabatan Central General Hospital (CGH) and Bogor Indonesian Red Cross Hospital (BIRCH). This was a descriptive study with a qualitative analysis obtained through indepth interviews and documentary research. We discovered that the ARCP was implemented better at Persahabatan CGH compared to BIRCH. Problems include ineffective communication, limited resources, lack of an antibiotic guideline (at BIRCH), lack of coordination between organizations, lack of supervision from the government, and the lack of motivation to implement the policy. These issues can be solved by intensifying the socialization of the policy, conducting more workshops and trainings for private and government hospitals that has not received any, providing an incentive for them to implement the program, such as including ARC into hospital accreditation indicators, and clarifying the benefits of the program to the stakeholders. Abstrak. Salah satu upaya pengendalian resistensi antibiotika di Indonesia adalah pembentukan pembentukan Komite Pengendalian Resisten Antibiotika (KPRA) dan diterbitkannya Permenkes no. 8 Tahun 2015 tentang program pengendalian resistensi antibiotika (PPRA) di rumah sakit (RS). Penelitian ini bertujuan untuk menggali lebih dalam terhadap implementasi kebijakan PPRA di RS dibandingkan antara RS pemerintah dengan RS swasta yaitu RS Persahabatan dan RS PMI. Metode penelitian merupakan studi deskriptif dengan analisis kualitatif melalui wawancara mendalam dan telaah dokumen. Hasil penelitian menyimpulkan bahwa pelaksanaan PPRA di RS Pemerintah lebih baik dari RS Swasta. Saran yang diajukan adalah mempercepat peningkatan kesadaran akan pentingnya PPRA, terutama bagi RS.
