Abstract

Background: In Indonesia, in 1995, 50.9% pregnant women suffer from Iron Deficiency Anemia (IDA).Objective: To compare the effect of weekly iron/folate vs standard daily iron/folate supplementation in pregnant women on compliance and hemoglobin level. Methods: The study was a quasi-experimental, non-equivalent control group design. Pregnant women with 20-28 week gestation were enrolled in this study in Bantul District. They were divided into two groups. One group (n=70), in Pajangan and Sewon Sub District, was given 120 mg iron/0,50 mg folate once a week, and the second group (n=65), in Bambang Lipuro and Pundong Sub District, received 60 mg iron/0,25 mg folate per day. Compliance with supplementation was assessed by the women self-reporting, pill count, and stool test. To measure hemoglobin level, the cyanmethemoglobin method was conducted. Results: Compliance was significantly higher in the weekly group (84.69% compared with 71.94%, p=0,004). A multiple logistic regression analysis indicated odds ratio 6.605, it means that in the weekly group compliance was 6,6 times higher than in the daily group. Hemoglobin level increased in both groups. There was no significant difference between groups for changes in hemoglobin. Initial haemoglobin values for the weekly (m=11.61 g/dl) and daily (m=11.27 g/dl) groups as well as final hemoglobin values (11.99 g/dl and 11.83 g/dl, respectively) did not differ significantly between the two groups. Conclusion: Supplementation on a weekly basis is as effective as daily supplementation in improving iron status in pregnant women. In the weekly group, compliance can be ensured.

Abstract

Background: The prevalence of exclusive breastfeeding until the age of 4 months is low. One of the many factors associated with it is healthcare supports on breastfeeding. Objective: The aim of this study was to assess the effect of healthcare supports for breastfeeding on the prevalence of exclusive breastfeeding until the age 4 months. Methods: This was a cohort study including 99 infants born at 10 birth clinics in Pekanbaru City during August-December 2003. Exposure was whether mother received supports for breastfeeding. Infants were followed monthly for 4 months to assess the duration they received exclusive breastfeeding. Results: Healthcare supports for breastfeeding were significantly associated with the duration of exclusive breastfeeding, i.e. RR (95%CI): 1.66 (1.08-2.54). Variables associated with longer duration of exclusive breastfeeding were availability of rooming in and early lactation. Early formula feeding did not support exclusive breastfeeding. Other variables associated with exclusive breastfeeding were husbands’ and other family members’ supports, maternal opinion on exclusive breastfeeding and maternal occupation. Conclusion: Healthcare supports on breastfeeding were important for promoting exclusive breastfeeding.

Abstract

Background: The main problem facing Indonesian people today is nutrition deficiency which brings effect to low quality of human resources. Infants aging 6 – 12 months are at high risk of having growth and development disorder. Complementary breastfeeding pattern is one of factors causing gross motoric movement growth and development disorder. Objective: This study was meant to know the relationship be-tween complementary breastfeeding pattern and gross motoric movement growth and development of infants aging 6 – 12 months at Bermani Ulu Sub district, Rejang Lebong District.Method: This was observational type of research which used cohort design. The samples consisted of 87 infants of 6 – 12 months selected through simple random sampling. Complementary breastfeeding pattern data were collected using interview and food call methods, growth data using anthropometry measurement, gross motoric movement data using motoric guideline of infants aging 3 – 18 months. Analysis of data used chi-square, anova, logistic linier and logistic regression. Results: The result of the study showed there was no significant relationship (p>0.05) between complementary breastfeeding pattern and gross motoric movement growth and development. There was significant relationship (p<0.05) between energy intake and protein intake and growth, with energy intake and protein intake. There was significant relationship (p<0.05) between energy intake and protein intake and development, with energy intake and protein intake. Result of multivariate analysis showed there were 2 variables as growth predictor i.e. energy intake (B=0.096) and protein intake (B=0.351) with (24.7) R square and there were 2 variables as gross motoric movement development predictor i.e. energy intake and protein intake (p<0.05).Conclusions: 1) Complementary breastfeeding were not related to gross motoric movement growth and development, 2) energy and protein intake of complementary breastfeeding were related to gross motoric movement growth and development, 3) energy and protein intake were dominant factor which were related to gross motoric movement growth and development.

Abstract

Background: Lung diseases are one of the main health problems in Indonesia. Among the lung diseases, Chronic Obstructive Pulmonary Disease (COPD) is the 7th leading causes of death in Indonesia. The prevalence COPD tends to increase due to increasing smoking habit, industrialization, air pollution, and more elderly people. Diet regulation for COPD patients were mainly for energy consumption, and accurate proportion of carbohydrate and fat. Excessive carbohydrate intake will increase blood CO2.Objective: To evaluate the influence of low carbohydrate and high fat diet on the decrease of CO2 concentration and improving respiration in acute exacerbation of COPD.Methods: This is an experiment study, with pre and posttest design. The subjects were COPD patients who hospitalized due to acute exacerbation. The subjects must be conscious, no left heart failure, no coronary heart disease, no acute respiratory failure that need ventilator and  hospitalized for minimum 7 days and could take oral diet. Results: Women, younger, and no smoking COPD patients had decreased blood CO2 offer low carbohydrate high fatty diet. All patients with this diet experience had improvement in their respiratory symptoms. Conclusion: Low carbohydrate high fat diet decreases blood CO2 in new acute exacerbation of COPD patients, no smoking women and younger patients. However, this diet improved respiratory symptoms for all patients who receive it.

Abstract

Background: Food poisoning occurrence in Indonesia is increasingly growing. Studies also show that nosocomial infection of digestive tract happens in some hospitals because of bacteria transmission. Lack of knowledge, attitude and skills of food handlers about food safety will lead to high risk of disease occurrence cause by food. To prevent such on occurrence, training on food safety for food handlers at hospitals should be held. Objective: To know knowledge, attitude, and practice of food handlers trained on food safety at nutrition installation of Harapan Kita Heart Cardiovascular Hospital..Methods: The study was a quasi-experiment which used pre test-posttest times series design. Subject of the study were 40 food handlers out of 73 total population. Each subject was trained on food safety and observed 3 times (prior to training, 1 month and 2 months after training). Data were analyzed using SPSS for windows version 10.0.Results: After training on food safety given for 5 days there was significant increase of knowledge, attitude and skill of food handlers with p=0.00 (p<0.05) at the period before up to 1 month after training and at the period before up to 2 months after training, but at the period of 1 month up to 2 months there was significant decrease with p=0.00 (p<0.05). Respondent characteristics consisting of age education and length of work did not have significant relationship (p>0.05) with knowledge, attitude and skills of food handlers. Conclusion: There was increase of knowledge, attitude and skill of food handlers before and after 1 month of training.
