Abstract

Patients with type 2 DM are more at risk of developing hypertension. This study aims to explain the factors that afect the occurrence of hypertension in patients with type 2 diabetes in the work area of Pati Puskesmas during 2014. The study was conducted by observation using a case control study design in Puskesmas patients in Pati District. The case group was 57 patients with type 2 diabetes with hypertension while the control group was 57 patients with type 2 diabetes without hypertension. Data were obtained from observations of medical recordsand qualitative interviews. Chi-square test on bivariate analysis and multiple logistic regression on multivariate analysis. Factors afecting the occurrence of hypertension in patients with type 2 diabetes are less physical activity (OR = 6.4, 95% CI: 2.18 - 18.77, p = 0.001), long sufering DM ≥5 years (OR = 5, 4, 95% CI: 1,97 - 14,704; p = 0,001), and DM medication adherence (OR = 3,6; 95% CI: 1,32 - 9,83; p = 0,012). Factors that have no efect are: age ≥45 years, male gender, DM diet compliance, history of hypertension, smoking habits, salty eating habits, cofee drinking habits, and length of time to sleep. Type 2 diabetes mellitus patients> 5 years may reduce the risk of hypertension by performing physical activity management and medication adherence. Keywords: risk factor, hypertension, type 2 DM

Abstract

Background: Limitations of data regarding risk factors that affect the incidence of leptospirosis in endemic areas make leptospirosis control in Leptospirosis endemic areas not effective. Therefore, a study is needed to determine the factors that influence the occurrence of Leptospirosis in an area. Methods: An observational analytic was conducted with case-control study design. The population in this study were all patients found at the Hospital in Pati Regency who showed clinical symptoms of Leptospirosis (fever >38 oC, severe headache, calf muscle pain, red/yellowish red eyes) and had a laboratory examination with Leptotek Lateral Flow and recorded on medical records. The sample in this study were 44 cases and 44 controls with consecutive sampling technique. Data were analyzed by chi-square and logistic regression. Results: Variables that influence the occurrence of leptospirosis are the characteristic of the non-ratproof house with OR = 3.14 (95% CI = 1.16-8.47), the existence of rats in and around the house with OR = 3.66 (95% CI = 1.67-12.28), the behavior of walking barefoot during outdoor activities with OR = 1.57 (95% CI = 1.31-10.28).Conclusion: Several factors that have been proven to have an effect on the occurrence of leptospirosis are the characteristic of the non-ratproof house, the existence of rats in and around the house and the behavior of walking barefoot during outdoor activities.

Abstract

ENGLISHThe prevalence of hypertension in type 2 diabetic patients is 1,5-3 times higher than in nondiabetic The objectives of this research is to explain the risk factors affecting hypertension in type 2 diabetic patients. The research used an observational studies with case-control study design in Primary Healthcare Centers patients in Pati Regency of 2014. Case group were 57 patients with hypertension in type 2 diabetes, while control group were the type 2 diabetes patients without hypertension. Data were obtained from medical records and qualitative interviews. Chi-square test in bivariate and multiple logistic regression in multivariate analysis. This study has been obtained ethical clearance from The Ethical Committee of Health Research Medical Faculty of Diponegoro University or dr. Kariadi Hospital. Results : factors that influence hypertension in type 2 diabetic patients were physical activity (OR=6.4; 95% CI: 2.18-18.77; p=0.001), diabetes duration ≥ 5 years (OR=5.4; 95% CI: 1.97 – 14.704; p=0.001), and medication adherence (OR=3.6; 95% CI: 1.32-9.83; p=0.012). Other risk factors that not significantly influenced were age ≥45 years, male, diet compliance, history of hypertension, smoking, salt consumption, coffee consumption, and sleep duration. Kariadi Semarang. Hasil Penelitian : faktor yang berpengaruh terhadap terjadinya hipertensi pada penderita DM tipe 2 adalah aktivitas fisik kurang (OR=6,4; 95% CI: 2,18 - 18,77; p=0,001), lama menderita DM ≥5 tahun (OR=5,4; 95% CI: 1,97 - 14,704; p=0,001), dan kepatuhan minum obat DM (OR=3,6; 95% CI: 1,32 - 9,83; p=0,012). Faktor yang tidak berpengaruh adalah : usia ≥45 tahun, jenis kelamin laki-laki, kepatuhan diet DM, riwayat hipertensi, kebiasaan merokok, kebiasaan makan asin, kebiasaan minum kopi, dan lama waktu tidur.
