Abstract

Background: Patient immobility remains to be one of the primary causes of pressure ulcers. Therefore, mobilization is necessity for patients being treated in the intensive care unit. However, the occurrence of pressure ulcers is not only dependent on the mobilization, but also depents on the bed itself and the type of mattress. This study used the same mattress and bed and compared the effectiveness of progressive mobilization with regular mobilization. Objective: This study aims to examine the effectiveness of progressive mobilization level I and II on hemodynamic status and decubitus ulcer risk in critically ill patients.Methods: This was a quasy experimental study using repeated measure design. There were 40 respondents selected using purposive sampling, which 20 respondents assigned in each group. A Braden scale was used to measure the risk of decubitus ulcer. Paired t-test and repeated measures anova were performed for data analysis.Results: Paired t-test showed that there was a significant difference of systolic pressure, diastolic pressure, MAP, heart rate, and Braden score after given intervention with p-value <0.05. However, repeated anova test showed that diastolic pressure had p-value >0.05, thus could not be continued to post-hoc test. The progressive mobilization of level I and II in critical patients can stabilize systolic pressure (52.46%), stabilize MAP (58.43%), stabilize heart rate (68.99%), and reduce the risk of decubitus (55.03%) for 7 days of recurrent intervention.Conclusion: The progressive mobilization of level I and II can reduce the risk of decubitus and stabilize the patient's hemodynamic status in critical patients.

Abstract

Background: The mechanical ventilator is an indispensable breathing tool in the Intensive Care Unit (ICU). But the mechanical ventilator is associated with the risk of Ventilator Associated Penumonia (VAP). VAP occurs due to poor hygiene of the endotracheal tube (ETT). ETT hygiene should be maintained to inhibit bacterial development in the lungs using suction above cuff endotracheal tube (SACETT) to prevent VAP.Objective: To analyze the effectiveness of SACETT in preventing Ventilator Associated Pneumonia (VAP) in critical patients in the ICU.Methods: This was a quasy experimental study with posttest only with control group design with 15 samples in intervention group (SACETT and Chlorhexidine 0.2%) and 15 in control group (ETT, Open Suction, and Chlorhexidine 0.2%) with purposive technique sampling. The Simplified Clnical Pulmonary Infection Score (CPIS) was used to measure VAP.Results: This study illustrates that there was no VAP incidence in the intervention group, and as much as 13.3% VAP in the control group. SACETT was more effective in preventing VAP than in standard ETT on day 4 (p = 0.001).Conclusion: SACETT is more effective in preventing VAP than standard ETT in the fourth day in patients with neurological, cardiovascular, urinary, digestive, and immune system disorders.

Abstract

Background: One of the complications of ventilator use in patients in Intensive Care Unit (ICU) is Ventilator-Associated Pneumonia (VAP). Oral hygienes is one of the methods to prevent VAP.Objective: The objective of this study was to compare the value of clinical infection score (CPIS) in critically ill patients after given oral hygiene using chlorhexidine and Piper betle Linn mouthwash.Methods: This was an observational study with cross-sectional study design, which consisted of two intervention groups. Thirty respondents were selected using total sampling, with 15 respondents randomly assigned in each group. Independent t-test was used for data analysis.Results: Findings showed that  the mean of CPIS in the Piper betle Linn group was 3.80 and the mean of CPIS in the chlorhexidine group was 4.07.Conclusion: CPIS in the treatment group using Piper betle Linn mouthwash was lower than the mean of CPIS in the treatment group using clorhexidine.

Abstract

Penyimpanan rekam medis merupakan kegiatan untuk melindungi rekam medis dari kerusakan fisik dan isi dari rekam medis. Proses penyimpanan rekam medis mempunyai resiko yang dapat mengancam keselamatan dan kesehatan kerja petugas rekam medis. Salah satu upaya dalam keselamatan dan kesehatan kerja dengan memberikan perlindungan bagi petugas penyimpanan rekam medis dengan cara menggunakan alat pelindung diri. Berdasarkan studi pendahuluan di RSUD K.R.M.T Wongsonegoro Semarang penggunaan alat pelindung diri yang belum terlaksana dengan baik dan ruang penyimpanan yang berdebu serta resiko terjadi gesekan/tergores kertas atau map rekam medis yang tajam. Tujuan penelitian ini adalah untuk mengetahui keselamatan dan kesehatan kerja pada penyimpanan rekam medis. Jenis penelitian adalah penelitian deskriptif kualitatif yaitu penelitian dengan mendeskripsikan obyek dengan metode pengumpulan data menggunakan wawancara dan observasi. Penelitian ini menggunakan analisis data deskriptif yaitu mendeskripsikan dan menggambarkan hasil penelitian dalam bentuk narasi. Hasil penelitian menjelaskan bahwa keselamatan dan kesehatan kerja sudah berjalan namun belum secara maksimal. Hal ini dikarenakan belum terdapat SPO yang khusus mengatur keselamatan dan kesehatan kerja di rekam medis. Alat pelindung diri yang digunakan oleh petugas penyimpanan rekam medis meliputi alat pelindung pernapasan/ masker dan alat pelindung tangan/ sarung tangan. Pengetahuan petugas penyimpanan rekam medis bahwa alat pelindung diri merupakan hal yang penting digunakan sebagai perlindungan pada saat melakukan pekerjaan. Selain itu, sikap dan perilaku petugas penyimpanan rekam medis dalam penggunaan masker mencapai 91% dan sudah cukup baik, namun pada penggunaan sarung tangan hanya mencapai 41% sehngga perlu peningkatan.AbstractStorage of medical records is an activity to protect the medical records from physical damage and the content of the medical records. The process of storing the medical records has risks that can threaten the occupational safety and health of medical records staff. One of the efforts in occupational safety and health is by providing protection for the medical records staff by using personal protective equipment. Based on the preliminary study in RSUD K.R.M.T Wongsonegoro Semarang, the use of personal protective equipment that has not been done well and dusty storage space and the risk of friction/ scraped paper or sharp medical record map. The purpose of this study is to determine the occupational safety and health on the medical records storage. The type of the research is descriptive qualitative research which is a research by describing an object with the data collection method using interview and observation. This study uses descriptive data analysis that describes the results of the research in the form of narrative.The result of the study explains that the occupational safety and health had been done but not maximally. This is because there is no SPO that specifically regulate the occupational safety and health in medical records. Personal protective equipment used by medical records staff includes respiratory protective equipment/ masks and gloves. The knowledge of medical record storing staff is that personal protective equipment is an important thing to be used as protection during the work. In addition, attitudes and behavior of medical record storing officers in the use of masks reached 91% and it is good enough. However, the use of gloves, it only reaches 41%, therefore, it needs improvement.

Abstract

Hipertensi disebut silent disease karena biasanya pasien tidak sadar bahwa dirinya mengalami hipertensi. Hipertensi dapat menyerang siapa saja dari berbagai kelompok umur maupun kelompok sosial ekonomi. Pengobatan Hipertensi tidak harus menggunakan tidakan farmakologis, namun hipertensi dapat diobat menggunakan terapi non farmakologis berupa terapi yang berbentuk meditasi berupa terapi yoga. Penelitin ini bertujuan untuk mengetahui pengaruh pemberian terapi yoga terhadap penurunan kecemasan dan tekanan darah pada penderita Hipertensi. Metode ini adalah desain Quasi-eksperimental dilakukan di Kelurahan Pedalangan RW 01. Sampel adalah 60 penderita hipertensi primer, yang secara acak dikelompokkan menjadi dua kelompok yaitu kelompok intervensi (n = 30) dan kelompok kontrol  (n = 30). Hasil Analisa penelitian ini menunjukkan pada tekanan darah sistole dengan nilai p value 0,001 (p <0.05) dan tekanan darah diastole dengan nilai p value 0,000 (p <0.05) pada kelompok intervensi dan kontrol setelah dilakukan yoga. Ada pengaruh yoga terhadap penurunanmtekanan darah pada penderita hipertensi. Peneliti menyarankan kepada semua tenaga kesehatan untuk menerapkan terapi yoga kepada pasien hipertensi karena yoga mampu menurunkan  tekanan darah. Kata kunci: Yoga, tekanan darah, hipertensi Abstract The effect of yoga therapy to decrease blood pressure in patients with hypertension. Hypertension is called the silent disease, it’s usually caused an patient not be aware that they was being hypertension. Hypertension can affect anyone of various age groups and socio-economic groups. Hypertension can treatment should not used pharmacological, measure, however hypertension can treatment by nonpharmacologycal teraphy a form of transcedental meditation or yoga. The purpose of this research is conducted to determine the effect of yoga therapy to decrease blood pressure in patients with hypertension. This  method design Quasi-experimental done in Pedalangan village RW 01. The sample was 60  subject primary hypertension, who were randomized include 2 groups: there was intervention group (n = 30) and control group (n = 30). Analysis result Man Whitney experiment systolic blood pressure  p value 0.001 (P <0.05) and diastolic blood pressure with p value 0.000 (p <0.05) in the intervention and control groups after yoga. There is the influence of yoga to lowering blood pressure in  hypertensive patients. Researchers suggest to all health workers to apply yoga therapy to patients with hypertension because yoga can lower blood pressure.Keywords: Yoga, blood pressure, hypertension
