Abstract

Objective:  To examine the effect of the combination of cold pack, water spray, and fan cooling on body temperature reduction and level of success to reach normal temperature in critically ill patients with hyperthermia.Methods: This was a randomized control trial (RCT) with pretest postest control group design and repeated measurement, conducted on December 2016 – January 2017. There were 32 respondents selected using total sampling, with 16 respondents randomly assigned in the experiment and control group. A digital thermometer was used to measure hyperthermia. Paired t-test, Repeated Anova with post hoc, and Mann Whitney were used for data analysis.Results: Findings showed that the mean of body temperature in the experiment group in pretest was 38.762oC and decreased to 37.3oC after given intervention for 60 minutes. The mean difference of body temperature was 1.4625, with p-value 0.000 (<0.05). In control group, the mean of body temperature in pretest was 38.669oC and decreased to 38.188oC given intervention for 60 minutes. The mean difference of body temperature was 0.4812, with p-value 0.000 (<0.05). Conclusion: There was a significant effect of the combination of cold pack, water spray, and fan cooling on body temperature reduction and level of success to reach normal temperature in critically ill patients with hyperthermia. This combination is more effective than water compress alone.

Abstract

Background: Patients with acute myocardial infarction have a change in sleep pattern. Acupressure is identified as a therapy with the principle of healing to deal with sleep changes.Objective: To analyze the effect of acupressure on sleep quality and pulse rate in patients with acute myocardial infarction.Methods: This study employed a quasi-experiment study with non randomized pretest-posttest with control group design. There were 50 respondents selected in this study, with 25 assigned in the intervention and control group using consecutive sampling. Quality Sleep Questionnaire and bedside monitor were used to measure sleep quality and pulse rate. Accupressure in combination of Neiguan point (PC 6), Shenmen (HT7), Sanyinjiao (SP6), Yintang (EX3) and Tay (EX5) was given as the nursing intervention in this study. Data were analyzed using paired t-test and independent t-test.Results: There was statistically a significant increase of quality of sleep and significant decrease of pulse rate in the intervention group with p-value 0.000 (<0.05).Conclusion: Accupressure has a significant effect on the increase of sleep quality and the decrease of pulse rate in patients with acute myocardial infarction. Thus, it is suggested that accupressure could be one of the nursing intervention for AMI patients to increase sleep quality and decrease of pulse rate.

Abstract

Background: One of the complications of ventilator use in patients in Intensive Care Unit (ICU) is Ventilator-Associated Pneumonia (VAP). Oral hygienes is one of the methods to prevent VAP.Objective: The objective of this study was to compare the value of clinical infection score (CPIS) in critically ill patients after given oral hygiene using chlorhexidine and Piper betle Linn mouthwash.Methods: This was an observational study with cross-sectional study design, which consisted of two intervention groups. Thirty respondents were selected using total sampling, with 15 respondents randomly assigned in each group. Independent t-test was used for data analysis.Results: Findings showed that  the mean of CPIS in the Piper betle Linn group was 3.80 and the mean of CPIS in the chlorhexidine group was 4.07.Conclusion: CPIS in the treatment group using Piper betle Linn mouthwash was lower than the mean of CPIS in the treatment group using clorhexidine.

Abstract

Background: Diabetic ulcer is an open wound on the skin layer to the dermis due to hyperglycemia and neuropathy. This condition often causes infection and becomes an inhibitor in the wound healing process. Red fruit oil soap can be used as an alternative cleansing to reduce the number of bacterial colonies and accelerate the wound healing process.Aims: This present study aimed to observe the effect of red fruit oil soap in the cleansing process to reduce the number of bacterial colonies and the wound healing process in grade II Diabetic ulcer patientsMethods: This is a quasi-experimental study with the pre-test post-test non-equivalent control group design. Wound cleansing in the intervention group used 0.9% NaCl solution and red fruit oil soap with a pH of 5.74, while the wound cleansing in the control group used 0.9% NaCl solution. Bacterial colonies were assessed and the type of bacteria was observed by the swab method. The wound healing was assessed with Bates-Jensen Wound Assessment Tool (BWAT) instrument for 14 days with observations every 3 days. Analysis test used Mann Whitney and Repeated Measure ANOVA.Results: There was no difference in the mean of the number of bacterial colonies between the wounds that were cleansed with 0.9% NaCl solution and red fruit oil soap and the wounds which were cleansed by using 0.9% NaCl solution (p> 0.05). The mean of the number of colonies in the intervention group until the 14th day reduced by 3.14x106 and in the control group was reduced h 1.40x106. There was a significant decrease in the wound healing scores in each group, in each assessment for 14 days.Conclusion: This study found that the wound cleansing using red fruit oil soap and 0.9% NaCl solution could reduce the number of bacterial colonies on the wound surface and accelerate the wound healing process among patients with grade II diabetic ulcer.
