Abstract

ABSTRAK Perawat dalam tindakan keperawatan harus mampu  berkomunikasi,  komunikasi yang efektif menciptakan rasa aman dan nyaman bagi pasien. Penelitian menunjukkanbahwa komunikasi terapeutik perawat tidak dilaksanakan.,tidak dilaksanakan komunikasi terapeutik dan tidak puas pada tahap orientasi., komunikasi terapeutik perawat tidak dilaksanakan padatahap terminasi Peneelitian bertujuan untuk mengidentifikasi upaya yang dapat meningkatkan keterampilan komunikasi efektif perawat pasien. Pendekatan sistematika review menelusuri jurnal EBSCO E.Journal; Elsevier Science Direct; CINAHL Complete; google Scholar; ruang lingkup manajemen kepemimpinan keperawatan; definisi operasional adalah usaha yang dilakukan oleh perawat dalam memperbaiki pola strategi keterampilan komunikasi menjadi lebih baik kepada pasien; penelitian ini dilakukan di klinik /di rumah sakit, populasi penelitian adalah semua perawat klinik/ rumah sakit, sampel penelitian adalah setiap perawat yang belum  memiliki pola strategi, keterampilan komunikasi., bahan dan alat utama penelitian kuesioner, lembar observasi.  Komunikasi efektif perawat pasien dengan (1) pelatihan meningkatkan  kepercayaan diri perawat dalam memberikan pelayanan intensif kepada pasien. (2)  panduan keterampilan komunikasi peka budaya setiap pasien meningkatkanketerampilan komunikasi perawat dan kepuasan pasien.,(3) program komunikasi terapeutik lanjutan perawat anak,meningkatkan interaksi aman , sehat, menumbuhkan kepercayaan,  memperbaiki pemulihan(4) mini workshop meningkatkan kemampuan komunikasi. Komunikasi efektif perawat– pasienmelalui upaya pelatihan.,panduan keterampilan peka budaya.,program komunikasi terapeutik terencana.,mini workshop.   Kata kunci : Komunikasi efektif,  perawat  - pasien.   EFFORTS TO IMPROVE EFFECTIVE COMMUNICATION NURSE – PATIENT   ABSTRACT Nurses in nursing actions should be able to communicate, effective communication creates a sense of security and comfort for the patient. Research shows that therapeutic nurse communication is not implemented, no therapeutic communication is conducted and dissatisfied at the orientation stage, nurse therapeutic communication is not carried out at termination stage Purpose: To identify efforts that can improve the effective communication skill of the patient nurse. A systematic review approach to browse the journal EBSCO E.Journal, Elsevier Science Direct., CINAHL Complete., Google search., Scope of nursing leadership management, variable operational definition is effort undertaken by nurses in improving communication strategy skill patterns more both of the nurses of clinics / hospitals, the research samples are any nurses who do not have a strategy pattern, communication skills, materials and main tool of questionnaire research, observation sheet. Effective communication of patient nurses with (1) training improves nurse confidence in providing intensive services to patients. (2) guidance of culture sensitive communication skills each patient improves nurse communication skill and patient satisfaction, (3) advanced nursery communication therapeutic program, improves safe, healthy interaction, cultivates trust, improves recovery (4) mini workshop enhances communication skill. Abstract

ABSTRAK Restrain/ pengikatan fisik (dalam psikiatri) secara umum mengacu pada suatu bentuk tindakan menggunakan tali untuk mengekang atau membatasi gerakan ekstremitas individu yang berperilaku diluar kendali. Indikasi restrain meliputi perilaku amuk yang membahayakan diri sendiri, orang lain dan lingkungan. Restrain adalah bagian dari implementasi patient safety, karena bertujuan untuk memberikan keamanan fisik, psikologis dan kenyamanan pasien. Restrain yang dilakukan pada pasien di rumah sakit jiwa juga dapat menimbulkan dampak negatif  berupa cedera / luka pada ekstremitas yang dilakukan restrain. Tujuan dari systematic review ini untuk mengetahui tindakan restrain yang aman dan efektif di rumah sakit jiwa, systematic review dilakukan dengan mencari artikel melalui Ebscho, Science direct, Portal Garuda dan Google Scholar. Jurnal yang telah terkumpul selanjutnya dilakukan critical appraisal. Restrain efektif untuk mengatasi pasien agresif, tetapi dapat menimbulkan efek samping berupa luka / cedera, untuk mencegah terjadinya luka / cedera,  restrain dilakukan dengan menggunakan alat yang bermanset, area restrain diberikan lotion, durasi restrain paling lama 4 jam, selama di lakukan restrain perawat mengobservasi kondisi dan memenuhi kebutuhan pasien, restrain dilakukan oleh staf yang terlatih.   Kata kunci : Restrain, Cedera, Efektif   EFFECTIVE RESTRAIN FOR PREVENTING INJURIES   ABSTRACT Physical restraint (in psychiatry) generally refers to a form of action using a strap to curb or limit the movement of an individuals extremity that behaves out of control. Restrained indications include self-harm behavior, others and the environment. Restrain is part of the implementation of patient safety, as it aims to provide physical, psychological and patient comfort. Restrain performed in patients in psychiatric hospitals can also have a negative impact of injury / injury to the extremity of the restrain. The purpose of this systematic review to find safe and effective restrain actions in psychiatric hospitals, systematic review is done by searching articles through Ebscho, Science direct, Garuda Portal and Google Scholar. The collected journals are then performed critical appraisal. Restrain effective to overcome aggressive patient, but can cause side effect in the form of wound / injury, to prevent injury, restrain is done by using tool bermanset, restrain area given lotion, restrain duration of 4 hours long, during restrained nurse observe conditions and meet patient needs, restrained by trained staff.   Keywords:  Restrain, Injury, Effective

Abstract

Background: Low independent nurse actions leads to poor health care quality. Therefore, understanding the factors affecting independent nursing action is necessity. Objectives: This study aims to analysis the independent nurse actions and its related and predicting factors.Methods: This study used a cross-sectional design with observational technique for data collection. There were 165 nurses recruited using simple random sampling in 14 inpatient wards. Spearman Rank Correlation and multiple regressions were used for data analysis.Results: Results of this study showed that there were significant relationships of age, gender, education, employment time, family burden, working experience, knowledge, workload, and nurse ability with independent nursing action with p-value <0.05. Nurse knowledge is the most dominant factor predicting independent nursing action with R-value 0.450.Conclusions: Independent nurse actions in daily living activities, caring & support, and rehabilitation were influenced by multiple factors such as nurse knowledge, ability, education and gender. Nurse knowledge is the most predicting factors affecting independent nursing action. Therefore, hospital and nurse managers need to well manage these predicting variables in order to improve independent nurse action that leads to the better quality of health service in the Dr. M. Haulussy Ambon Hospital.

Abstract

Background: The SBAR (situation, background, assessment, recommendation) method assists nurses in communicating information in nursing shift handover. Inaccurate shift handover can have a serious impact on patients due to poor communication. Optimal resource development can be done by coaching as the best guidance method from manager for directional discussion and guidance activity to learn to solve problem or do better job and build nursing leadership culture in clinical service.Objective: To analyze the effectiveness of coaching method using SBAR communication tool on nursing shift handovers.Methods: This was quasy experimental study with pretest posttest control group design. Fifty-four nurses were selected using consecutive sampling, which 27 assigned in the experiment and control group. An observation checklist was developed by the researchers based on the theory of Lardner to evaluate the effectiveness of the implementation of coaching using SBAR on nursing shift handover. Independent t-test, Mann-Whitney test and Wilcoxon test were used for data analyses. Results: There was an increase in coaching ability of head nurses in the implementation of SBAR in nursing handover after 2-weeks and 4-weeks of coaching. And there was also a significant improvement of the use of SBAR on nursing shift handover in the experiment group (p <0.05) compared to the control group. Conclusion: coaching using SBAR (situation, background, assessment, recommendation) communication tool was effective on nursing shift handovers. There was a significant increase of the capability of head nurse and nursing shift handover after coaching intervention.

Abstract

The holistic nursing service is based on the concept that humans are sick not only physically that cured by drug delivery, but also pay attention to other aspects of mental where the patient needs motivation and spirit to cope with illness, social where the patient wants to meet and gather with family/friends and spiritual where the patient wants to pray and pray for healing. Spiritual care is an important part of the overall care provided to improve the quality of life of patients. The role of nurses is now more involved on treatment measures. Proper methods in clinical supervision are necessary for the implementation of spiritual care to be as important as physical care, one of them using drill method. This study aims to analyze the influence of drill methods in the team leader's clinical supervision on the implementation of nurses spiritual care. The research method used is quasy experiment with pretest-posttest research design with control group design. The population of nurse research in inpatient room of PKU Muhammadiyah Yogyakarta and PKU Muhammadiyah Gamping Hospital. A total of 32 nurses were taken as samples through consecutive sampling technique. To see the implementation of spiritual care used observation sheet refers to the label Nursing Interventions Classification, spiritual care is observed before and after the application of drill methods in clinical supervision. Data were analyzed by paired t-test. The results showed the average of nurses' spiritual care before the drill method applied to the intervention group was 6,56 and 6,13 in the control group, after drill method applied in the intervention group was 17,44 and 6,50 in the control group. The results of statistical tests showed that there was a significant difference in nurse's spiritual care before and after the application of drill methods in the intervention group. It is recommended for the hospital to improve the implementation of clinical supervision to the nurses related to non-physical skills using drill method.

Abstract

Interprofessional collaboration will be realized if two or more people from different professions interact to produce a shared understanding that would not have been possible if those professions worked independently. How each profession views the meaning of collaboration must be understood by both parties so that the same perception can be obtained. The application of collaboration between professions is currently limited to traditional or multiprofessional collaboration models. A high level of knowledge is needed by the nursing profession to be able to provide nursing services independently and collaborate with other health workers. This level of education and knowledge gap will hinder the effective communication process among health professionals. Continuous nursing education can be used as a strategy to eliminate the knowledge and education gaps between professions so that it will improve the effective communication process among health professionals. Collaboration is a complex process that requires special attention accompanied by high knowledge in exchanging ideas in discussing patients. Nurses who always try to improve their knowledge and skills to achieve their desired career will have an impact on their ability to exchange ideas (communicate) with other professions or collaborate with other professions. Continuous nursing education and career planning are two strategies that can increase nurses’ knowledge and skills to improve nurses' confidence in exchanging ideas with other professions.

Abstract

The role and function of interdisciplinary care providers in health services is still fragmented so that interdisciplinary health services provided to patients become overlapping. Interdisciplinary fragmentation can be overcomed through Interprofessional Collaborative Practice (ICP). Good collaboration between the team of health professionals is needed, in order to exchange information clearly and comprehensively. The core competency in Interprofessional Collaborative Practice (ICP) is effective interprofessional communication. The purpose of this systematic review is to find out whether ICP can improve the ability of health workers to carry out interprofessional communication. The method used by Systematic review was by searching literature databases from Ebscho, Sciendirect, PubMed and Google scholar by using advanced search keywords. The keywords chosen were Interprofessional Collaborative Practice (ICP), Interprofessional Communication, health workers. The searching was limited in 2006 - 2016, full text pdf and English. The inclusion criteria from the article was the health workers as the respondents, intervention with Interprofessional Collaborative Practice (ICP), the outcome measured was interprofessional communication skills of health workers. There were 6 articles that correspond to the inclusion criteria. Those 6 articles were reviewed. Results: each article gives an outcome to the improvement of interprofessional communication and 2 articles produce 3 themes. ICP can effectively improve the ability of health workers to carry out interprofessional communication. It is expected that the head of the hospital can make a special policy related to the implementation of ICP and design a model for ICP involving all health professions so that it can be applied in health services as an effort to develop effective interprofessional communication and improve the quality of health services so that the professional role of each discipline runs well.

Abstract

Healthcare-Associated Infections (HAIs) are infections that patients get while undergoing treatment and medical procedures in the hospital or other healthcare facilities after more than 48 hours. Improving the quality of hand hygiene practices can reduce pathogen transmission that causes HAIs. Infection and Prevention Control Nurses (IPCNs) as the pioneers of infection prevention and control should play an active role in cultivating hand hygiene in the hospital environment. Unfortunately, until the present time, there is evidence that IPCNs have not played their roles optimally in making hand hygiene a culture in the hospital. This study aimed to explore the experiences of IPCNs in cultivating hand hygiene at the inpatient wards in a regional hospital in Semarang, Indonesia. This study used a qualitative research design with a phenomenological approach. The samples were recruited using purposive sampling technique, consisting of three IPCNs as the main participants, and one Infection Prevention and Control Officer (IPCO) and three Infection Prevention and Control Link Nurses (IPCLNs) as the triangulation participants. Data were collected using semi-structured in-depth interviews and analyzed using the content analysis. The results revealed two themes. First, the appropriate education implementation and its documentation are important to provide an adequate understanding of medical and non-medical staffs, patients and families so that they are willing to perform hand hygiene properly. Second, the implementation of hand hygiene requires complete infrastructures and facilities, proper monitoring and evaluation system, as well as appropriate role models and motivation so that hand hygiene can become a positive culture. This study concludes that IPCNs have positive and negative experiences while trying to cultivate hand hygiene. IPCNs understand and agree that their ability to provide proper education and sensitivity to the factors affecting the implementation of hand hygiene can improve the people’s understanding and compliance with hand hygiene. In the end, hand hygiene will eventually become one of the positive cultures in the hospitals. Keywords: IPCNs, hand hygiene

Abstract

The high demand for role and function as a nurse can potentially lead to burnout.The impact of burnout causes patients to have secondary infections that are not caused by the disease, decrease patient satisfaction and quality of care and mortality. Efforts that can be done to reduce the nurse's burnout is through the application of an effective leadership style. Caring leadership as a leadership style can facilitate the needs of nurses so that it will improve performance. This study aims to determine the effect of caring leadership of the head of space on the burnout of nurses. The method used is quasi experiment pre and post design with control group. A total of 46 nurses were taken as samples through purposive sampling fron 426 nurses. Data analysis using pairred t-test. The results showed that there was difference of average burnout value before and after applied caring leadership with p-value=0.000, which means there is influence of head room caring leadership against nurse's burnout. Implementation of caring leadership should be done consistently for all nursing activities in the hospital, given the caring leadership can reduce the burnout of nurses that will ultimately have an impact on the quality of nursing services.

Abstract

Pendahuluan: Perawatan diri secara mandiri pada ibu nifas sangat diperlukan, berdasarkan Teori Orem manusia pada dasarnya mempunyai kemampuan untuk merawat dirinya sendiri. Cara untuk meningkatkan kemandirian ibu adalah dengan adanya pengaruh seorang pemimpin (tenaga kesehatan). Kepemimpinan transformasional adalaha gaya kepemimpinan yang dapat memotivasi bawahannya untuk dapat beebuat lebih baik, dalam hal ini dapat diterapkan dalam meningkatkan kemandirian ibu nifas. Tujuan: Memberikan gambaran terhadap cara meningkatkan kemandirian perawatan diri ibu postpartum melalui kepemimpinan transformasional. Metode: Metode yang digunakan dalam proses pencarian dengan menggunakan website PubMeds, Jurnal Keperawatan Bina Husada, Jurnal Keperawatan Diponegoro. Kata kunci pencarian yaitu kemandirian, ibu nifas, perawatan diri. Hasil dan Pembahasan: Idealized/Charismatic Influence: pengaruh dari seorang pemimpin sangat dibutuhkan agar menimbulkan pengaruh yang ideal, pengalaman melahirkan mendukung kemampuan ibu nifas dalam melakukan perawatan masa nifas. Inspirational Motivation: motivasi dan inspirasi yang dapat memberikan dorongan, bina keluarga mandiri berpengaruh terhadap tingkat kemandirian ibu. Individualized Consideration: seorang perawat harus mampu menghargai latarbelakang dan lingkungan pasien agar tercipta hubungan yang harmonis dan meningkatkan rasa percaya diri pasien. Intellectual Stimulation: gaya kepemimpinan transformasional yang mendorong bawahannya untuk menyelesaikan permasalahan dengan cermat, makin tinggi tingkat pengetahuan makin mudah menerima informasi. Kesimpulan: Kemandirian ibu nifas dapat ditingkatkan melalui kepemimpinan transformasional.
