Abstract

Abstract. Unsafe conditions and unsafe actions at PT Putra Perkasa Abadi are the main causes of accidents that cause loss and loss to humans and companies, damage to equipment, disrupt production and others. The number of work accidents has increased dramatically at the beginning of 2018, there are still employees who ignore the use of personal protective equipment, and there are still hazards found in the work environment. This problem aims to reveal the work location that has the highest potential hazard in the mining area, the potential hazards that exist in the work environment and reveal what actions should be taken to prevent work accidents, and reveal work accident statistics. The implementation of K3 in companies is still not optimal, this is evident in the increasing number of work accidents in early 2018. The danger cannot be completely eliminated, but can be minimized. this study uses the Job Safety Analysis and Job Hazard Analysis methods to identify and analyze hazards on a job. after identifying the danger, so that to control it will be given a recommendation. Not only do hazard identification activities, but this research will also make several Job Safety Analysis and Job Hazard Analysis. By analyzing these two requirements, it is expected that the Job Safety Analysis and Job Hazard Analysis will facilitate the establishment of an occupational health and safety management system. Keywords:  Production, Safety, Danger, Job Hazard Analysis, Job Safety Analysis

Abstract

From the research can be seen that the incidence of property damageaccidents increased. This is caused of unsafe conditions and unsafe work acts,inconsistent in supervising the K3 management system, lack of knowledge, employeeawareness and concern about K3. The absence of sanctions against employees whoviolate K3. The purpose of this study is to reveal whether PT. CBP has implementedK3 principles in accordance with the principles of K3 mine, reveals whethersupervisors and miners have implemented K3 principles, disclosing the extent ofoccupational risk, disclosing the causes of accidents, and disclosing the percentage ofunsafe and unsafe events.In carrying out this research, combined theory and direct interview withfield data, so that both of them got problem solving approach.Based on the results of the research conducted can be concluded thefollowing points. First, the factors that cause property damage accidents includehuman factors, equipment factors and environmental factors. Second, there are stillmany unsafe and unsafe practices. Third, the percentage of accidents for unsafe actsis 60.71% and for unsafe conditions is 39.29%. And the cause of the accidentproperty damage in PT. Cakra Bumi Pertiwi is a matter of implementation of K3policy in management, which subindicator is supporting activity, capabilityguarantee, and ability guarantee.

Abstract

kcal/kg that accumulate in stockpile PT. Nusa Alam Lestari. The authorconducted a preliminary survey to determine the amount of low quality coal inPT. Nusa Alam Lestari. In February 2017 the number of coal with a calorie valuebelow 5100 kcal /kg - 5500 kcal / kg on the stockpile of seam stack at 10422 tonsof stockpile.Here are several kinds of coal upgrading process, one of them is dryingprocess. Coal Drying aims to remove or decrease the water content contained incoal, so that with reduced moisture content is able to increase the calorific valueof coal.In the process of upgrading with the drying process (Coal Drying)obtained coal calorific value by using Furnance Carbolite to influencetemperature and reaction time that is at 400⁰C for 1 hour produce coal withcalories 5,697,94 kkal / kg. At 400⁰C for 2 hours produces coal with a calorificvalue of 5,987.8 kcal kg. At 400⁰C for 3 hours produces coal with a calorificvalue of 6,182.70 kcal/kg. At 500⁰C for 1 hour produce coal with a calorific valueof 6,192.44 kcal/kg. At a temperature of 500⁰C for 2 hours yields a coal calorificvalue of 6,233.86 kcal / kg. At a temperature of 500⁰ for 3 hours yields a calorificvalue of 6,219.24 kcal / kg. At 600⁰C for 1 hour yields a calorific value of6,423.15 kcal/ kg. At 600⁰C for 2 hours 6,470.15 kcal/kg. At 600⁰C for 3 hoursyields a caloric value of 6,375.14 kcal/kg.In this research, the optimum caloric value obtained at drying process is600⁰C for 2 hours with decreasing water content 3,95%. In the drying process tothe effect of temperature and calorie reaction time rose from 5,457.99 kcal/kg to6,470.15 kcal/kg.

Abstract

The things that should be success parameter of mining company is the amount of its production. Loading and hauling equipments performance will determine production planning. Unproductive working hours directly also be an affect from unappropiate work system. As the draft of this research, the author examine the data of its equipments cycle time so that it can be obtained the details working hours of equipments such as effective working hours, and working hours obstacles. The methodology of this research used quantitative analysis approachment which the biggest point should be done to reach the goals of this research is to examine the data and make a detail of working hours such as the number of annual equipments and then evaluated it, thus it would be obtained the production. The results showed the average of equipments was working about 3.55 hours/day with production getting (25,417.997 ton). Then the autors do an evaluation, if working hours is increased to 5 hours/day, then it also will increase more than (35,819.3467 ton) and additional revenue from (Rp. 12,708,998,500) be (Rp. 17,909,673,800).

Abstract

This final project is based on the partially fragmentation which the boulders blasting results more than 80cms that intervene the crusher’s performance. The purpose of this Final Project is to get the exact value of powder factor; in order to attain the value of fragmentation up to 80 with optimally hold on 10%.The methodology problem that used is finding the value of rock factor by using Monte Carlo simulation and calculate the effect of powder factor on fragmentation with linear regression.The result of this final project is attained the value of rock factor 5.01 by using Monte Carlo simulation and the relationship between powder factor with the fragmentation is 72% with equation Y = -102,89X + 50,086. Based on its calculation, to attain the maximum result of fragmentation holding up to 10%, therefore the exact value of powder factor is 0,389.

Abstract

Based on the data obtained from the company, the data accidents that occurred between the years 2012-2014 there were 222 cases of accidents, as well as activities that are harmful to workers at risk of injury, lost work days, and the cost of repair tools. The causes of such unsafe acts and unsafe conditions of the workers. The purpose of this study were: 1) Revealing the dangers that occur in coal mining activities. 2) Uncover the risk of hazards that occur in the coal mining activities. 3) Disclose the value of the likelihood and severity of hazards that occur in the coal mining activities start from land clearing activities until the loading of coal in the stockpile.This type of research is descriptive with quantitative approach, in which data is collected or obtained from direct observation in the field and interviews with leaders and staff as well as employees of the company who are competent in connection with the object of research. The data is taken from the data mining activities, the risk of hazards that occur in the coal mining activities.Results of the study revealed that the risk of hazards that occur in the coal mining activities at PT. Kuansing core affluent who are at a high level going on the road hauling.

Abstract

ABSTRACT The concentration units at Open Pit TB 1.42 Pemali is using Pan American Jig as its main equipment. There are two steps applied to the Pan American Jig; primary jig and clean up jig. At the clean up jig operational step, the final gain of cassiterite should get a serious attention. To obtain the cassiterite concentrate content as conditioned by Tin Ore Processing Center (PBBT) Pemali, that is 40% - 65% with 97% gain, there should be an analysis to the jig operation variables that are being used and analysis to the resulting product, so that a good suitability would be acquired in order to increase the cassiterite gain based on the operational standard in every clean up jig step. According to the sampling and the calculation of the final gain of cassiterite at clean up jig, the value of the right clean up jig is 96.82%, whereas the value of the left clean up jig is lower, 91.43%. The final concrete content were resulted as follow: the right clean up jig, compartment A 38.31% SnO2, compartment B 20.76% SnO2., compartment C 13.80% SnO2.Whereas,the left clean up jig, compartment A 49. 15% SnO2,compartment B 25.09% SnO2, compartment C 23. 43% SnO2. In order to increase the cassiterite gaining through clean up jig based on the requirement, that is 97%, which done by: first, change the setting of the compartment A, B and C hit length in a chronological order, 18mm, 10mm, and 6mm with push speed resulted at the three compartments 5.943cm/s, 3.302 cm/s, and 1.981 cm/s. Second, the management of horizontal crossflow with the characteristics and the jig feed rate. Third, the uniformity of bed jig shape and size, the placement of bed jig size should be adapted with the sequence of compartment and the jig steps. Fourth, arrange the entering feed pulp by installing feed combs grader flow at feed box.Key words: jig, pan american jig, cassiterite

Abstract

PT. Guguk Tinggi Coal, is a coal mining company located in salak village, Talawi, Sawahlunto city, West Sumatra province. In executing the mining accidents that often occur is influenced by the perception of employee knowledge about K3 norms and the high level of accident risk that occurs. The purpose of this study to reveals the perception of employes PT. Guguk Tinggi Coal About the safety norm, Reveals the risk persentage of work accident occurring at PT. Guguk Tinggi Coal.Data collected or obtained directly from the respondents with direct observation in the field and direct interviews with employees that competent in companies and have relation with the object of research. The data taken are the conditions in the environment (mining area to the stock of the workplace files) as well as potential hazards that may occur, the employee's knowledge of safety norms and work programs of K3 management, workers' responses to the programs undertaken by K3 management.The results obtained are the employee's knowledge about K3 and how to evaluate the work activities are still low with the results obtained by the questionnaire test with a value of 3.35 for the implementation and 3.21 for the evaluation of this category is still quite good, there are still factors of work accident from unsafe conditions and unsafe actions, PT.Guguk Tinggi Coal should form a special management for K3 and provide training and guidance on the importance of occupational safety and health, management is also required to always evaluate the activities of the workers in intensive mining activities.

Abstract

Coal oil mixture (COM) is one of the methods of making liquid coal fuel in addition to liquefaction and coal water fuel. Coal oil mixture (COM) is a mixed fuel with a balanced composition between coal, petroleum, and additives (Maeda, 1991: 66).In this study, the coal used is low rank coal mixed with additives and diesel. The ratio of coal: diesel: the additives used are 25%: 75%: 0% (COM A1), 25%: 74%: 1%, (COM A2) and 25%: 73%: 2% (COM A3).The results showed that the viscosity characteristics tested on COM A1, A2, A3 were closer to MDO than HFO as standard ISO 8217 2010 MDO (Marine Diesel Oil) viscosity with maximum limit of 11 cSt at 400c temperature. The calorific value of COM A1, A2, A3 is lower than MDO due to the utilization of low calorie coal in the mixture.

Abstract

At PT. Sumbar Calcium Pratama found several violations against existingSOP. There are still many workers who work in hazardous work positions, the useof equipment and machines in an unsafe way and they not use the protection toolsthat have been provided by the company. The purpose of this research is to revealthe factors that cause work accidents, reveal the most dominant factors as thecause of occupational accidents, reveal the potential hazards that can causeaccidents, can calculate the percentage of the frequency and severity of workaccidents, can cause human error or unsafe employee behavior, and can give anidea of the use of hazard card.In this research, combined between theory with field data, so get problemsolving approach. And the results of data processing will be analyzed forsubsequent to be generated a recommendation.Based on the results of research conducted can be summed up some of thefollowing. First, accident-causing factors are generally caused by unsafe andunsafe measures, the most dominant factor is unsafe, other potential hazards thatallow the occurrence of accidents in the implementation of mining activities inPT. Sumbar Calcium Pratama, such as the convetional mining, in 2016, 87.3%frequency rate and severity rate of 43.69%, and the results of the questionnairenote that the attitude and application of employees are in fairly good category.Keywords: K3, unsafe action, unsafe condition
