Risk Factor Analysis of Dengue Shock Syndrome Occuring to Children in RSD dr. Soebandi Jember Regency

Abstract

Dengue Shock Syndrome is a medical emergency situation caused by shocking DHF. Some factors that can influence the DSS. The purpose of this research know some risk factors that might cause DSS. Those are age, nutritional status, gender, and amount of platelet and hematocrit. This research was retrospective case control doing in dr. Soebandi Hospital Jember. The sample were divided into 2 groups. Those were DSS (case group) and non-DSS group (control group). Data analysis was done by Chi-Square test using SPSS 21 version. These were 136 patients which were divided into 94 patients of control group and 42 patients of case group. In this research, there were p-value for ages, nutritional status, gender, amount of platelet and hematocrit on each 0,450; 0,490; 0,198; 0,001; 0,007 respectively. In this search, it could be concluded that age, nutritional status, and gender were not significantly related to DSS case although amount of platelet and hematocrit were significantly related to DSS case.Keywords:dengue shock syndrome, age, nutritional status, gender, hematocrit, platelet.

Analysis of Risk Factors for Obesity in Urban Children in Some Elementary School in Jember Regency

Abstract

The prevalence of obesity in children has steadily increased, not only in the world but also in Indonesia. Urban area as a central of economic led the higher prevalence of childhood obesity than in rural areas. The objective of this study was to analyze the risk factors associated with obesity among urban elementary school childrens in Jember. A cross sectional study was conducted with a total sample of 176 pupils. Anthropometric status determined using body mass index for age and obesity stated if z-score exceed >+2SD based on WHO 2007. The questionnaire about risk factors was filled with interview. Analysis data performed with univariate, bivarate, and multivariate analysis. This study showed the prevalence of obesity was 17%. Overall, our result showed that in non modifiable risk factors, the prevalence of obesity was significantly higher in children who had obese parent when compared to children who hadnt. Children who had obese parent had the more risk of obesity by 6 times. In modifiable risk factors, children who had meal more than 3 times per day had the more risk of obesity by 2 times. Drinking milk frequently according to Dietary Guideline for American 2015 has a protective factors against obesity.Keywords: childhood obesity, urban, risk factor

Abstract

The increasing population of elderly causes getting more complex illness because impairment in  organ function, one of them is the blood vessels. Blood vessels will have decreased nitrite oxide that causes atherosclerosis, as the primary cause of the hypertension. The change of lifestyle and nutrient factor is related to the incident of hypertension. Meanwhile, Indonesians tend to prefer food sources of carbohydrate. The purpose of this research is to analyze relations between consumption of carbohydrate with blood pressure on elderly from active members of Karang Wreda Sriwijaya and Semeru Jaya Sumbersari Jember about 47 samples. This research is an analytic observational research with the cross sectional methods. In this research, samples were interviewed to describe the charracteristic ( the age, sex, the history of hypertension). Then samples were interviewed to know their carbohydrate consumption using 24-hours Food Recall method three times in different time with the maximum of two weeks and the blood pressure was checked with a mercurial sphygmomanometer. The results of this research based on the correlation of Spearman obtained a value of p=0,00 for the number of subject’s carbohydrates consumption with systolic blood pressure and p=0,02 for diastolic pressure. The conclusion is there is a correlation between carbohydrates consumption with systolic and diastolic blood pressure.Keywords: carbohydrates consumption, blood pressure, elderly

Abstract

Healthcare facilities such as hospitals, public healthclinics, and private practices are places that peoplevisit to obtain thorough medical care. The state billno.44 of 2009 stipulates that hospitals areinstitutions that provide complete healthcare servicefor individuals. The service included is providinginpatient facilities, home care, and medicalemergency service. According to the bill, among thefunctions of a hospital is to establish and maintainthe quality standard of healthcare service within thehospital, which is then used as a blueprint or areference in day to day patient care.The general public are becoming evermoreconscientious on choosing which hospital to go towhen they become ill. Which is why for everyhospital that offers medical service, the challengethen arises on how to implement various strategiesin order to develop and not merely survive in thegrowing healthcare industry. Among the strategiesthat needs attention is sustaining the qualitystandard of the hospital care service.The quality standard of the hospital care service canbe measured using the SERVQUAL model (ServiceQuality model), which gives rate to the indicators ofvarious qualities of service (Tjiptono, 2011). Thereare 5 main aspects that are measured and arrangedbelow according to the relativesignificance/importance (Tjiptono dan Chandra,2011):A. Reliability, which signifies the institutions abilityto provide error-free and accurate medical careupon patient arrival, within the agreed upon timeconstraint.B. Responsiveness, signifies the healthcarepersonnels availability and capability to provide fastresponse aid and cater to their needs and request. Italso involves the tactful knowledge of informing thepatient when the medical care will be given andperforming the promised care accordingly.C. Assurance, signifies how the healthcare personnelestablish trust with patients which in turn earns thepatients trust towards the institution. This can beachieved by providing healthcare personnel whomaintain courteous conduct at all time yet possessthe skill and ability to answer any arising questionsor handle any given situation.Furthermore, it involves the institutions ability tofoster a safe and secure environment for patients.D. Empathy, signifies the ability of healthcarepersonnels to establish effective communicationwhile delivering proper diagnosis. Caring is anessential component which can be implemented bygiving personal attention to each patient.E. Tangibles, correlates with the physicalenvironment of the establishment, which can comein the form of an attractive office design, availabilityof a full range medical equipment, and furthercomplemented by the pleasant appearance of thehealthcare personnels.Delivering optimum service is essential forinstitutions who thrive on gaining customersatisfaction and building towards patient loyalty inthe long run.

The Difference of Plasma Malondialdehyde (MDA) Level in Farmers Using Chemical Pesticides and Farmers Using Organic Pesticides

Abstract

Many farmers still use chemical pesticides. Chemical pesticides inside the body will form free radicals that will bind to unsaturated fatty acids (cell membrane structures) that cause increased levels of malondialdehyde (MDA). Organic pesticides can be used as an alternative to control pests. Organic pesticides leave no residues to plants and the environment. This study aims to determine the difference of plasma malondialdehid (MDA) levels in farmers who use chemical pesticides in Dawuhan Village, Tenggarang Subdistrict, Bondowoso Regency and farmers who use organic pesticides in Lombok Kulon Village, Wonosari Sub-district, Bondowoso District. This was an observational analytic research with cross sectional research design conducted on 30 farmers who use chemical pesticides and 30 farmers who use organic pesticides. Blood sampling was taken from Mediana cubiti vein. Measurements of plasma malondialdehyde (MDA) were performed using the TBARS method. Based on the result of Unpaired T-test, it was found that p < 0.001 which showed significant difference in plasma malondialdehyde (MDA) level in farmers using chemical pesticides and farmers using organic pesticides. Abstract

Chronic Kidney Disease (CKD) is a chronic disease that results in the progressive decline in renal function and irreversible. Patients with end stage CKD requiring hemodialysis therapy. CKD patients who undergo hemodialysis therapy largely experiencing symptoms of anxiety and changes in quality of life. The aim of this study was to determine the correlation between the anxiety level and the quality of life of patients with CKD during hemodialysis therapy at dr. Soebandi Regional Hospital Jember. Research using Cross Sectional Design with the total number of respondents as many as 30 people and was carried out in November 2015. The respondent's anxiety level was measured using a HARS questionnaire and assessment of quality of life using the WHOQOL-BREF questionnaire. The results revealed respondents with mild anxiety were 16,67% (10% with good quality of life and 6,67% with bad quality of life), respondents with moderate anxiety were 40% (10% with good quality of life and 30% with bad quality of life), and respondents with severe anxiety were 43,33% with bad quality of life. Spearman correlation test showed the significance (p) 0,00 with correlation coefficient (r) – 0,517. Thus, it can be concluded that there was a correlation between the level of anxiety and quality of life of CKD patients during hemodialysis therapy at dr.Soebandi Regional Hospital Jember.   Keywords: Chronic Kidney Disease (CKD), hemodialysis, anxiety, quality of life.

Abstract

The increasing number of hospitals causes competition among hospitals to get patients so that each hospital must provide quality services to reach patient’s satisfaction. Patient’s satisfaction determines patients behavior in the future, include their interest in utility. The purpose of this study was to find out the relationship between patient satisfaction index on assurance of service quality and utility. This study used cross-sectional design with 50 respondents. Patient’s satisfaction index and utility were measured using ServQual questionnaire. The results of statistic test with Spearman's Rho correlation test showed that the significance (p) = 0,030 with correlation coefficients (r) = 0.308. So, it can be concluded that there was a weak relationship between patient’s satisfaction index about assurance of service quality and utility at Interna Medicine Polyclinic Kalisat Hospital Jember.   Keywords: Patient’s satisfaction, service quality, assurance, utility.

Abstract

RSD Kalisat is one of the hospital that is very concern with the quality of health services. The reliability of service quality is often perceived as the most important dimension for the customer. Quality of service is very influential on patient satisfaction. If the health care provided is good, then the patient will be satisfied and the possibility to reuse services in health care-related agencies will increase. The purpose of this study was to determine the correlation between patient satisfaction index in the reliability of service quality with utility at RSD Kalisat. This study used cross sectional design with total sample of 84 respondents. The instrument used was RSD Kalisat profile and questionnaire to determine patient satisfaction index in the reliability of service quality and statement for utility. The results of statistical test by Spearman's Rho showed a significance value (p=0.021) and the correlation coefficient (r =0.252). Thus, it can be concluded that there was a significant correlation between patient satisfaction index in the reliability of service quality with utility at RSD Kalisat Jember with a weak correlation.   Keywords: index satisfaction, reliability, service quality, utility
