Role of 8-isoprostane, Matrix Gla Protein (MGP) and Bone Morphogenetic Protein-2 (BMP-2) in Vascular Calcification in Chronic Kidney Disease

Abstract

BACKGROUND: Vascular calcification may be an important risk factor of cardiovascular disease in Chronic Kidney Disease (CKD). The pathobiology of vascular calcification in CKD is complex and involves some factors including inflammation, oxidative stress and balancing of calcification regulators. The aim of the study was to investigate the interaction between 8-isoprostane with calcification regulators such as matrix gla protein (MGP) and bone morphogenetic protein (BMP)-2 with vascular calciication in CKD.METHODS: A cross-sectional study was performed on 63 subjects undergoing haemodialysis maintenance for more than 3 months. Abdominal aortic calcification (AAC) was measured using a lateral abdominal X-ray for calcification in abdominal aorta, which is related to severity of calcific deposits at lumbar vertebral segment (L)1-L4. Serum levels of 8-isoprostane, MGP and BMP-2 were measured by enzyme-linked immunosorbent assay method.RESULTS: Results showed that 8-isoprostane levels were correlated with BMP-2 (r=0.266, p=0.018) and MGP levels (r=0.410, p≤0.001). MGP/BMP-2 levels ratio was correlated with AAC score (r=0.279, p=0.013). Subjects were then stratified into 3 groups based on AAC score: 0, 1-6 and ≥7. The highest mean of MGP levels was in AAC score 1-6 group, and the highest mean of 8-isoprostane levels was in AAC score ≥7. In the group of AAC score 0, 8-isoprostane levels were correlated with MGP levels (r=0.499, p=0.001) and MGP/BMP-2 levels ratio (r=0.291, p=0.034). In AAC score 1-6 group, 8-isoprostane levels were correlated with BMP-2 (r=0.661, p=0.005) and MGP levels (r=0.569, p=0.017). In AAC score ≥7 group, MGP levels were positively correlated with AAC score (r=0.608, p=0.041). With multivariate logistic regression analyses, we identified that increased MGP/BMP-2 levels ratio (OR=12.436; 95% CI=1.595-96.971) was an independent predictor in aortic calcification event.CONCLUSION: We concluded that regulators of calcification including calcification inhibitor and promoter related to oxidative stress, were associated with vascular calcification in CKD. MGP levels were increased in the early of calcification and MGP/BMP-2 levels ratio was a strong predictor for vascular calcification in CKD.KEYWORDS: vascular calcification, CKD, oxidative stress, 8-isoprostane, MGP, BMP-2

N-MID Osteocalcin (N-MID Oc) dan β-Crosslaps (β-CTx) sebagai Penanda Biokimia Bone Turn Over pada Wanita Menopause

Abstract

  Osteocalcin merupakan senyawa penanda untuk pembentukan tulang sedangkan β-CrossLaps (β-CTx) merupakan fragmen hasil pemecahan kolagen tipe 1. Tujuan penelitian ini yaitu untuk menentukan N-MID osteocalcin (N-MID Oc) dan β-CTx sebagai penanda biokimia Bone Turn Over untuk deteksi dini osteoporosis pada wanita menopause. Nilai N-MID Oc dalam serum ditentukan dengan pereaksi Elecsys N-MID Osteocalcin kit dan nilai β-CTx dalam serum ditentukan dengan pereaksi Elecsys β-CROSSLAPS kit yang bekerja berdasarkan prinsip Electrochemiluminescence Immunoassay (ECLIA). Data yang diperoleh diuji dengan menggunakan Uji-T. Sampel yang dikumpulkan berasal dari wanita normal, osteopenia, dan penderita osteoporosis pada wanita menopause. Hasil penelitian menunjukkan terdapat perbedaan nilai N-MID Oc pada wanita menopause normal dengan wanita menopause osteopenia, wanita menopause normal dengan wanita menopause penderita osteoporosis dan wanita menopause osteopenia dengan wanita menopause penderita osteoporosis (α=0,05), sedangkan pada nilai β-CTx terdapat perbedaan antara wanita menopause normal dengan wanita menopause osteopenia, wanita menopause osteopenia dengan wanita menopause osteoporosis, tetapi antara wanita menopause osteopenia dengan wanita menopause osteoporosis tidak menunjukkan perbedaan (α=0,05). Penelitian ini menunjukkan bahwa N-MID Oc dan β-CTx dapat dijadikan parameter untuk deteksi dini osteoporosis.Kata kunci: N-MID Osteocalcin (N-MID Oc), Osteoporosis, β-CrossLaps (β-CTx) N-MID Osteocalcin (N-MID Oc) and β-Crosslaps (β-CTx) as Biochemical Markers of Bone Turnover in Menopausal Women Osteocalcin is a marker compound for bone formation while the β-crosslaps a fragment split from collagen type 1. The aims of this study were to determined of the value of N-MID osteocalcin (N-MID Oc) and β-CTx as a biochemical marker of Bone Turn Over as early detection of osteoporosis in postmenopausal women. The value of N-MID Oc in serum was determined by reagent Elecsys N-MID Osteocalcin kit and the value of β-CTx in serum was determined by reagent Elecsys β-Crosslaps kit which works with the principle of Electrochemiluminescence Immunoassay (ECLIA). The data obtained were examined using T-test. The sample collected from normal, osteopenia, and osteoporotic in menopausal women. There were a significant difference N-MID Oc between normal, osteopenia, and osteoporotic in menopausal women (α=0,05). There were significant differences of β-CTx between normal and osteopenia, normal and osteoporosis but between osteopenia and osteoporotic menopausal women not showed any difference (α=0,05). Early Screening of Hemoglobinopathy in Indonesia Using Erythrocyte Indices

Abstract

BACKGROUND: The mutation spectrums of hemoglobinopathy are different among populations that yield a different result of erythrocyte indices. Calculation of erythrocyte indices with some formula has been reported to differentiate between hemoglobinopathy and non-hemoglobinopathy, but its cut-off should be recalculated specific for each population to gain a better sensitivity and specificity. We aimed to evaluate red blood cell count (RBC), Mentzer index, red cell distribution width (RDW), RDW index (RDWI), Shine and Lal index (S&L) and Green and King index (G&K) to screen hemoglobinopathy in Indonesia.METHODS: A retrospective cross-sectional study was performed on 202 subjects. The diagnosis of hemoglobinopathy was determined based on the results of complete blood count (CBC) data, high-performance liquid chromatography (HPLC) and Hemoglobin H (HbH) inclusion body. The ferritin concentration was checked to determine the status of iron. The erythrocytes indices were analyzed and calculated to predict hemoglobinopathy. RESULTS: A total 202 subjects who met the criteria were involved in this study. Fifty percent showed pure hemoglobinopathy and 4% showed a combination of thalassemia and hemoglobinopathy. The hemoglobin concentration and RBC were significantly higher, and the mean corpuscular volume (MCV) and RDW were significantly lower in hemoglobinopathy compared to iron deficiency. The difference was not significant if the hemoglobinopathy was combined with iron deficiency. By this study's cut-off, the G&K and RDWI showed the highest accuracy, sensitivity, and specificity.CONCLUSION: The new cut-off of erythrocyte index and its calculation to screen hemoglobinopathy in Indonesia showed a higher sensitivity and specificity, especially for G&K and RDWI with cut-off 73 and 228, respectively. The presence of iron deficiency in hemoglobinopathy could decrease the sensitivity.KEYWORDS: hemoglobinopathy, RBC, Mentzer index, RDW, RDWI, S&L, G&K
