Effects of Acupoint Stimulation with Digital Massager of Oxytocin on the Breast Milk Production of Working Mothers

Abstract

Background: Many breastfeeding mothers find that their milk production decreases or even stops when they return to work due to reduced breastfeeding hormones. Massage can stimulate the oxytocin hormone, and to replace the manual massage, a digital massager of oxytocin (DMO) is developed.Purpose: This study aimed to identify the effects of the DMO on the milk production of the breastfeeding working mothers. Methods: This study employed a quasi-experiment with pretest-posttest control group design. The samples were 93 breastfeeding working mothers in the community with a baby below 6 months. The samples were assigned into the intervention group (n=46) and the control group (n=47). An intervention of using the DMO was given to the mothers in the intervention group by applying the electrodes on the first intercostae of BL-17 and Bl-18 acupoints for 20 minutes in 3 consecutive days. Meanwhile, a standard treatment was given to the control group. Breast milk expression was carried out before and after the use of DMO. The data were analyzed using the Mann-Whitney and the Wilcoxon tests. Results: The results showed that there were differences in the volume of the expressed breast milk before and after the use of the DMO (p=0.00). The mothers in the intervention group increased their milk production on an average of 23 ml when other variables affecting the breastfeeding were controlled.Conclusion: The DMO stimulated and increased the milk production of working mothers. It can also be an alternative to the complementary therapy, especially for the nursing care management of breastfeeding mothers.

Abstract

Background: Hemodialysis process is useful for patients with end-stage renal disease. However, this is a situation of abject dependence on a machine, a procedure and a group of qualified medical professionals for the rest of their lives that may influence their functional status.Objective: This study aims to describe the functional status of patients with chronic kidney disease undergoing hemodialysis.Method: This was a descriptive study with cross-sectional design, which conducted in the PKU Muhammadiyah Hospital Yogyakarta on March 2017. There were 78 patients selected using purposive sampling. Functional status was measured using SF-36 questionnaire. Descriptive statistic was performed for data analysisResult:. The total average of functional status level was 42.92, with Physical Component Scale (PCS) of 42.31 and Mental Component Scale (MCS) of 45.78.Conclusion: There were functional deficiencies  in both physical and mental status in patients with CKD undergoing hemodialysis. Thus, it is suggested that pharmacological management of these patients need stringent monitoring on part of the psychiatrist to deal with mental health problems, and the interdisciplinary team  need to do great efforts to improve functional status and quality of life of patients with CKD.

Abstract

Background: Spiritual wellbeing (SWB) is an important quality-of-life dimension for cancer patients. Therefore, health professionals are demanded to improve SWB in these patients. A deeper understanding regarding the factors associated with SWB is needed.Objective: This study aims to examine the relationships of demographic characteristics of patients and spiritual wellbeing in patients with cancer.Methods: This was a cross-sectional correlational study with 60 respondents recruited using consecutive sampling. A spiritual wellbeing scale (SWBS) was used, and data were analyzed using Kendall's Tau and Spearman's rank.Results: Findings in this study showed that only age was statistically significant with spiritual wellbeing of cancer patients with p-value 0.003 (<0.05). There were no significant relationships of gender, education, occupation, long suffering, and type of cancers with spiritual wellbeing with p-value >0.05.Conclusion: There was a significant relationship between age and spiritual wellbeing in patients with cancer. This study provides the insight of knowledge regarding the factors affecting spiritual wellbeing in patients with cancer.

Abstract

Background: Job satisfaction influences the quality of health services in hospitals. However, low job satisfaction of nurses exists in the literature and influenced by various factors, including supervision. Thus, a reflective supervision model is considered effective in improving nurse job satisfaction.Objective: This study aims to analyze the effect of the reflective supervision model on nurse job satisfaction.Methods: This was a quasi-experimental study with control group design conducted between May-July 2017 in the Tugurejo Hospital and Dr. Moewardi hospital. There were sixty-eight nurses selected using purposive sampling, with 34 nurses assigned in the experiment and control group. Data were collected using questionnaires and then analyzed by paired t-test.Results: Paired t-test obtained p-value of 0.000, which indicated that the reflective supervision model has a significant influence to nurse work satisfaction.Conclusion: The reflective supervision model has a significant effect in increasing nurse job satisfaction. Therefore, this model could be applied by the head nurses to improve nurse outcome indicator, especially for nurse job satisfaction.

Abstract

Background: Patients with chronic kidney disease undergoing hemodialysis suffer changes in lifestyle, which cause physical and psychosocial problems, particularly anxiety. Progressive Muscular Relaxation is considered as an intervention to reduce anxiety.Objective: This study aims to examine the effect of progressive muscular relaxation in in reducing anxiety in patients with chronic kidney disease undergoing hemodialysis.Methods: This study used a quasi-experimental design involved 78 respondents, with 38 randomly assigned in the intervention and control group. The progressive muscular relaxation was performed on 14 muscle groups for 4-week period. Hamilton Anxiety Rating Scale was used to measure anxiety. Univariate, bivariate and multivariate analyses were performed for data analyses.Results: The results showed a statistically significant difference in anxiety values between the intervention and control group with p-value 0.000 (<0.05).Conclusion: There was a significant effect of progressive muscular relaxation in reducing anxiety in patients with chronic kidney disease undergoing hemodialysis. The results of this study is expected to be one reference in making the  progressive muscular relaxation as a nursing intervention in reducing anxiety in patients undergoing hemodialysis.

Abstract

Es puter merupakan bentuk es krim khas Indonesia yang berbahan dasar santan kelapa. Es puter banyak di gemari oleh masyarakat dari berbagai kalangan usia maupun kalangan status sosial ekonomi, akan tetapi es puter yang banyak dijual pada umumnya mengandung bahan yang tidak aman terutama bila dikonsumsi oleh anak-anak karena mengandung pewarna dan pemanis buatan. Kegiatan Ipteks bagi masyarakat ini bertujuan untuk meningkatkan kualitas dan kuantitas es puter yang di produksi oleh mitra agar dapat menghasilkan produk yang sehat dan bergizi sehingga omset mitra akan meningkat. Kegiatan ini telah dilakukan pada 2 mitra UKM yaitu usaha es puter yang berada di di Kota Semarang. Metode yang digunakan adalah analisis situasi pada kedua mitra, perencanaan kegiatan, implementasi dan pendampingan serta evaluasi. Adapun kegiatan yang telah dilakukan adalah penyusunan modul pembuatan es puter sehat, pelatihan tentang es puter sehat yang meliputi: Pentingnya menyediakan makanan sehat dan bergizi bagi anak, bahaya bahan kimia seperti pemanis sintetis, pewarna, dan pengawet bagi kessehatan, pemilihan bahan makanan yang sehat untuk membuat es puter, dan pembuatan ice puter dengan menggunakan bahan alami yang sehat dan bergizi. Hasil dari kegiatan ini adalah meningkatnya pengetahuan dan keterampilan dalam memproduksi es puter sehat pada kedua mitra, tersedianya es puter sehat dalam kemasan, meningkatnya jumlah produksi es puter, dan tersedianya varian es puter rasa buah yang lezat dan bergizi. Kegiatan ini telah meningkatkan kualitas dan kuantitas produk es puter pada mitra sehingga produk es puter tidak hanya lezat, namun juga bergizi dan aman dikonsumsi sebagai camilan bagi anak-anak.

Abstract

Background: Qualified health services are a demand for health care providers in the global era. Qualified health services can be achieved through collaborative practices. Integrated education which was initiated as a collaboration media according to WHO is Interprofessional Education (IPE). IPE is a process where a group of students or health workers with different backgrounds learn together for a certain period of time during education, with interaction as the main goal, for collaboration in providing preventive, promotive, rehabilitative and other health services (WHO, 2010). The concept of IPE learning can be applied in 3 activities, namely learning in hospitals, academics, and communities. IPE learning in hospitals that runs well is expected to produce professionals in the health sector who are able to collaborate with other health professions so that they can participate in health development in Indonesia. Objective: a Literature review to find out whether IPE can build nurses' ability to collaborate with other health teams. Method: This literature review was conducted by searching publication articles on EBSCO, Google search, PubMed, Sciencedirect.com with selected keywords namely interprofessional education (IPE), collaboration, interprofessional collaboration (IPC). The searching was done by limiting publications from 2010-2015. Results: From the literature search results that IPE can effectively build the ability of nurses to collaborate with other health workers. It is expected that the IPE implementation can be carried out continuously with more optimal preparation, considering that health institutions are the main providers of prospective professional health workers.

Abstract

Patients with chronic kidney disease (CKD) undergoing haemodialysis require fulfilment of physical, psychological and spiritual needs. In the effort to meet the spiritual needs, it is necessary to do an appropriate spiritual needs assessment of the patients. Assessing a Patient’s Spiritual Needs (APSN) and Spiritual Needs Questionnaire Scale (SpNQ) are such available instruments for use in spiritual assessment. This study aimed to determine the spiritual needs of patients with chronic kidney disease undergoing haemodialysis using the APSN and SpNQ instruments. This study used a quantitative observation with a cross-sectional design. The samples were 104 patients undergoing haemodialysis for more than three months at Kardinah Hospital and RSUI Harapan Anda Kota Tegal. The results showed that the need of prayers was the highest spiritual needs in APSN in the dimension ‘divine’ as reported by 80 respondents (76.92%). Meanwhile, in SpNQ, the dimension of religiosity was rated the highest with 98 respondents (94.23%). Patients with CKD had spiritual needs in all dimensions both in APSN and SpNQ. The result of chi-square test showed a p-value of 0.170, indicating that there was no difference in the assessment of spiritual needs of patients using the APSN and SpNQ instruments. The APSN and SpNQ instruments could be further considered as alternative instruments to assess the spiritual needs of patients with CKD undergoing haemodialysis.

Coping Of Pre-Release Women’s Prisoners

Abstract

Coping of women’s prisoners could changed in various conditions, one of them when they were in pre release condition. Various kinds of coaching has been done, however, did not close possibility of maladaptive coping still found on women’s prisoners pre release. The aim of this study was to identity the coping of women’s prisoners pre release in Prison.This study used quantitative descriptive. The number of samples in this study were 26 women’s prisoners pre-release. Sampling by purposive sampling technique. The data were compiled with a Ways of Coping Scale questionnaire according to Lazarus and Folkman (1984) consisting of 60 statements divided into two types, namely 20 emotion-focused coping statements and 40 problem-focused coping statements. This study was conducted in one of the Prison in Indonesia. The analysis of this study used univariate and frequency distribution. The results showed that women’s prisoners who used emotion-focused coping as much as 57.69% in passable category and 42.31% have poor category. While respondents who used problem-focused coping as much as 7.69% in good category, 84.62% passable and 7.69% poor category.Based on the results of this study, it could be concluded that women’s prisoners pre release used both types of coping and majority of respondents were categorized as good enough and less good. Women’s prisoners who had passable coping category risky lead for poor coping and poor coping could lead to maladaptive coping, so that they could harm himself and others.Therefore, it was necessary to do deeper screening of coping women’s prisoners pre release.The results of this study could be used as a reference in determining the appropriate intervention for preparation of women’s pre release and return to the community.Keywords: coping, prisoners, Women, pre release

Abstract

Interprofessional communication is very important in today's health services, where it can expand the population of health services especially in providing care in health services. Effective interprofessional communication is an important skill that can improve the function of high-quality teams in the care of patients by involving several scientific disciplines such as doctors, nurses and other health teams (interprofessional collaboration). The purpose of systematic review is to find out whether interprofessional communication can develop interprofessional collaborative efforts in hospitals. The method used was a systematic review. The articles were searched through Google Scholar, Proquest, Science Direct, are done using advanced in accordance with the keywords Professional, Communication, Collaboration. There were 3 articles in which each article produced that interprofessional communication can develop interprofessional collaboration in hospitals, with effective interprofessional communication, nurses, doctors and other health teams can respect each other and have the same vision and mission in providing services to clients to improve service quality and satisfaction for the clients themselves.
