Abstract

Background: Dolly was the biggest red light district in Southeast Asia with more than 1000 (a thousand) women working as commercial sex workers. Because the district imposed a very negative effect  to the surrounding inhabitants especially to children, therefore red light district area of  Dolly and Jarak which are located in  residential area had to be closed. The quality of life of the residents especially the commercial sex workers was very much affected after the district was closed.  The study aimed to understand the representation of the social, economic, psychological, and reproductive health condition after the Dolly district in Surabaya was closed.Subject and Method: This was a qualitative descriptive study with phenomenology approach. This study was  conducted in former red light districts of Dolly and Jarak in Surabaya from January 18-February 28, 2017. The informant of this study were commercial sex workers, former commercial sex workers, head of hamlet (RW), former pander of Dolly,  Head of Civil Society Organization, healthcare workers, and Social Office of Surabaya City. The sampling technique used was snowball sampling, with in-depth interviews, observation, documentation study. The data were analyzed by interactive analysis model including data collection, data reduction, display and verification. Results: The commercial sex workers characteristic prior and post closing of the district were around 28-43 years old, with elementary – senior high school educated. Most of the commercial sex workers were from outside the city with 1-5 customers/day. They charged Rp. 100-300 thousand (Dolly’s commercial sex workers) per customer and Rp. 100-200 thousand/customer (Jarak’s commercial sex workers). The representation of quality of life post-closing was poor, altered social condition,  lack of interaction with others, the degradation of economic condition, poor psychological condition due to the economical problem, and reproductive health was less protected  because they did not get  regular examination like before the complex was closed.Conclusion: The representation  of quality of life in terms of the condition of social, economic, psychology, and reproductive health of the commercial sex workers post Dolly and Jarak closing were poor. Abstract

Background: Aging process results in biological, physical, and mental changes in the elderly. Degenerative diseases are common in the elderly. They can be treated by pharmacologic or non pharmacologic treatment. Acupuncture therapy potentially can be used as a complementary or alternative therapy to treat degenerative diseases in the elderly. This study aimed to investigate factors that determine the decision to choose acupuncture therapy for the degenerative diseases in the elderly at Ja’far Medika Hospital, Karanganyar, Central Java.SubjectsandMethod: This was a qualitative study with phenomenology approach. This study was conducted at Ja’far Medika Hospital, Karanganyar, Central Java, fromMarch to April 2015. Ten informants were selected for this study by purposive sampling. The data were collected by in-depth interview, observation, and document review.They were validated by source triangulation, and analyzed by Miles and Huberman method.Results: The sample consisted of 5 men and 5 women, aged 53 to 72 years old, with school attainment varying from primary school to university graduate. Job category included farmer, civil servant, and pensioner. The degenerative diseases that were treated by acupuncture therapy including post-stroke, hernia nucleous pulposus, and eyelid disorder.  Internal factors that determined the decision to choose acupuncture were motivation to recover, (religious) belief, and boredom to medical service. External factors that determined the decision to choose acupuncture included the quality of service, family support, trust in the therapist, and hospital image. According to the informants, the acupuncture therapy could help recover their health from the degenerative diseases. Most of them used acupuncture as a complementary treatment.Conclusion: Some internal and external factors determine the decision to choose acupuncture therapy for the degenerative health problem in the elderly. Abstract

Background: Human Immunodeficiency Virus (HIV) infection is a global public health issue. Global AIDS Response Progress Reporting (GARP) reported that in 2015, about 36.7 million people worldwide suffered from HIV-AIDS in all age groups. The highest number of HIV/AIDS cases occured in East Africa and South Africa. This study aimed to examine the effects of predisposing, enabling, and reinforcing factors on the uptake of voluntary counselling and testing (VCT) among female  sex workers in Grobogan, Central Java.Subjects and Method: This was an analytical observational study with cross-sectional design. It was conducted in Grobogan, Central Java, in July 2017. A sample of 142 female sex workers  were selected for this study by exhaustive sampling. The dependent variable was uptake of VCT. The independent variables were attitude, perceived benefit, external motivation from others, and social support. The data were collected by a questionnaire and analyzed by multiple logistic regression.Results: Positive attitude towards HIV status (OR= 6.09; 95% CI= 0.968 to 38.38; p= 0.054), positive perceived benefit (OR= 10.58; 95% CI= 1.48 to 76.93; p= 0.019), external motivation (OR= 8.30; 95% CI= 1.21 to 56.82; p= 0.031), and social support (OR= 9.45; 95% CI= 1.46 to 60.83; p= 0.018), positively affected uptake of  VCT.Conclusion: Positive attitude towards HIV status, positive perceived benefit, external motivation, and social support, positively affect uptake of  VCT.Keywords: HIV, Voluntary Counselling Testing, female sex workersCorrespondence: Muhammad Vidi Perdana. Abstract

Background: Studies have shown that sexual assault can lead to the profound impact on the victims. It may last short or long, and can even generate a "ripple effect" of negative symptoms in the affected workplace or living environment. This study aimed to explore the psychological and biological impacts of sexual assault and approach to cope with the trauma in female adolescent victims in Surakarta.Subjects and Method: This was a qualitative study conducted in Surakarta, Central Java. The informants were social workers working at Yayasan KAKAK, a non-government organization dealing with sexual harassment issues. The data were collected by in-depth interview. The informants provided information about psychological and biological impacts of sexual harassment as well as the approach to cope with the trauma in female adolescent victims in Surakarta.Results: The psychological impacts of sexual harassment included anger, fear, self-consciousness or embarrassment, difficulty sleeping, loss of appetite. According to the information provided by the informants, the victim’s life turned into a misery, and school became a place to be avoided. The sexual attacks led to feelings of demoralization and humiliation, causing loss of self-confidence and self-esteem. The victims were reported as having trouble studying or paying attention, less able to perform well, participating less, no longer going to study group, thinking about dropping a class or even leaving the school. Victims experienced symptoms such as headaches, stomach aches, nightmares and anxiety attacks. Yayasan KAKAK assisted victims by supporting and guiding about steps to be taken to protect victims and to cope with the aftermath.Conclusion: Sexual harassment has serious emotional and biological effects on adolescent victims. According to this research finding, one of the greatest mitigating factors to trauma is the acknowledgment that it is happening. Abstract

Since 2011, the number of case of HIV and AIDS suffered by Indonesian women tends to increase. However, the prevention of HIV and AIDS in Indonesia focuses only on high-risk group. Besides that, measures are not differentiated base on different needs of targetedgroup of programs. The article discusses gender mainstreaming in the prevention of HIV and AIDS program in Surakarta as well as the fulfilment of gender need of housewife who is prone to HIV and AIDS infection. The article is the result of a qualitative study with interactive analysis method. The research targeted housewives as research subjects due to the increasing number of housewives who suffer from HIV and AIDS infection. Moreover, they even placed as the second highest group infected by the disease in Surakarta. The data was collected from an in-depth interview, documentation and observation. The finding showed that the quality of gender equality in the HIV and AIDS mitigation policy in Surakarta is positioned in a gender neutral level, providing the equal rights and obligations for all citizens, regardless of their sex. This matter gives impact to the prevention of HIV and AIDS program that only focuses on the fulfilment of housewives practical gender needs.

AbstrakDi tingkat nasional dan daerah telah terdapat kebijakan publik, program dan kegiatan untuk menanggulangi HIV/AIDS, namun jumlah kasus HIV/AIDS meningkat setiap tahun. Tujuan penelitian ini untuk mengetahui efektivitas collaborative governance antarpemangku kepentingan dalam pelayanan komprehensif berkesinambungan untuk menanggulangi HIV/AIDS di Kota Surakarta. Penelitian ini merupakan studi kasus jenis exploratory. Informan ditentukan dengan menggunakan purposive sampling. Pengumpulan data dilakukan pada 2 Juli – 2 September 2013 dengan wawancara mendalam, observasi, focus group discussion, dan dokumentasi. Teknik analisis data dengan model analisis interaktif. Hasil penelitian menunjukkan bahwa pemberi dan penerima layanan berperan dalam penanggulangan HIV/AIDS sesuai tugas dan fungsinya masing-masing. Program pencegahan dan penjangkauan, layanan kesehatan, reduksi bahaya, dan pemberdayaan belum efektif karena komitmen terhadap tujuan dan sikap saling percaya antarpemangku kepentingan belum optimal, petugas lapangan kurang profesional, terdapat konflik laten antarpemangku kepentingan, kurang optimalnya koordinasi antaranggota Komisi Penanggulangan AIDS Daerah Kota Surakarta dan rendahnya anggaran untuk penanggulangan HIV/AIDS karena HIV/AIDS belum menjadi isu prioritas dalam pembangunan daerah. Disimpulkan bahwa kolaborasi governance antarpemangku kepentingan belum efektif. Untuk meningkatkan kualitas dan efektivitas kolaborasi governance antarpemangku kepentingan, harus ada norma, struktur, dan proses yang jelas dalam menanggulangi HIV/AIDS.AbstractThere have been public policies, programs and activities to cope with HIV/AIDS in Indonesia at national and local level, but number of HIV/AIDScases is increasing every year. This study aimed to determine effectivity ofcollaborative governance between stakeholders in a sustainable comprehensive service to cope with HIV/AIDS in Surakarta City. This study was an exploratory study. Informants were selected using purposive sampling technique. Data collection was conducted on 2 July – 2 September 2013 using in-depth interview, observation, focus group discussion, and documentation. Technique of data analysis was an interactive analysis model. Results showed that service provider and receiver had taken roles in HIV/AIDS coping based on their own duty and function. Prevention and outreach, healthcare service, harm reduction and empowerment programs had not been yet effective because of less optimal commitment to purpose and mutual trust between stakeholders, less professional fieldworkers, latent conflict occurred between stakeholders, less optimal coordination between AIDS Coping Commission of Surakarta City members, and low budget for HIV/AIDS coping as HIV/AIDS is not yet a priority issue in regional development. In brief, collaborative governance between stakeholders is not yet efffective. To improve the quality and the collaborative governance effectivity between stakeholders, there should be any clear norm, structure and process in coping with HIV/AIDS.

Abstract

Penelitian ini bertujuan untuk mengetahui karakteristik demografi, sosial, dan ekonomi ODHA; pengetahuan, sikap, dan perilaku stakeholders terhadap HIV/AIDS dan ODHA; dukungan dan pelayanan komprehensif berkesinambungan serta pengaruh stakeholders dalam program penanggulangan HIV/ AIDS di Kota Surakarta. Metode penelitian ini adalah eksploratif. Teknik pengambilan sampel dengan purposive sampling. Pengumpulan data dilakukan dengan observasi, indepth interview, Focus Group Discussion, dan dokumentasi. Teknik analisa data dengan model analisis interaktif. Hasil penelitian menunjukkan bahwa di Kota Surakarta pada umumnya ODHA berusia produktif, laki-laki, heteroseksual,bekerja sebagai karyawan, mempunyai masalah medis dan kultur sosial seperti mempunyai infeksi oportunistik, efek samping obat ARV, depresi, mengalami diskriminasi, stigma negatif, dikucilkan keluarga dan anggota masyarakat. Stakeholders pelayanan komprehensif berkesinambungan dalam penanggulangan HIV/AIDS di Kota Surakarta mempunyai pengetahuan tentang HIV/AIDS yang baik, sikap peduli dan bertindak responsif terhadap ODHA. Dukungan dan pelayanan komprehensif berkesinambungan di mulai dari saat menjalani test; pengadaan obat-obatan Antiretroviral (ARV); perlakuan yang etis dan tidak diskriminatif; pencegahan dan pengobatan infeksi oportunistik; terapi non medis dan Kelompok Dukungan Sebaya. ABSTRACT Continuous Comprehensive Care Program for HIV / AIDS in Surakarta; This research aimed to find out the demographic, social and economic characteristics of people with HIV/AIDS (ODHA); stakeholders’ knowledge, attitude, and behavior toward HIV/AIDS and people with HIV/AIDS; comprehensive and sustainable support and service, as well as the contribution of stakeholders to the HIV/AIDS management program in Surakarta City. The research method employed was an exploratory one. The sampling technique used was purposive sampling one. The data collection was conducted using observation, in-depth interview, Focus Group Discussion, and documentation. Technique of analyzing data used was an interactive model of analysis. The result of research showed that in Surakarta, generally people with HIV/AIDS were productive, female, heterosexual and working as employees, having medical problem and social culture such as developing opportunistic infection, ARV drug side effect, depression, discriminated, negative stigma, isolated by the family and society member. The stakeholders of sustainable and comprehensive service in dealing with HIV/AIDS in Surakarta City had good knowledge about HIV/AIDS, caring and responsiveness to the people with HIV/AIDS. A comprehensive and sustainable support and service started during undertaking test; antriretroviral (ARV) drugs procurement, ethical and non-discriminatory treatment; opportunistic infection prevention and treatment; non-medical therapy and Peer-Group Support. Abstract

Permasalahan  kesehatan  ini dapat terjadi pada siapapun  tanpa memandang status seseorang dalam  masyarakat seperti narapidana laki-laki atau perempuan. Berdasarkan penelitian prevalensi HIV dan Sifilis pada narapidana pria 1,1% dan 5,1% sedangkan pada narapidana perempuan lebih tinggi yaitu mencapai 6% dan 8,5%. Dari kasus tersebut maka lapas menyediakan pelayanan kesehatan dalam  penanggulangan HIV/ AIDS. Tujuan penelitian untuk mengetahui Konteks dalam CIPP pada program skrining IMS dengan VCT bagi Narapidana di LP wanita Klas II A Kota Malang. Penelitian  ini menggunakan metode kualitatif dengan pendekatan evaluasi. Informan  kunci dalam penelitian ini adalah petugas kesehatan poliklinik LP Wanita Klas II A yang  mengarahkan  kepada tim IMS mobile Puskesmas Arjuno Kota Malang dan  ­­narapidana ­­yang mengikuti skrining. Hasil penelitian didapatkan cakupan rata-rata dalam setiap bulannya 21 WBP yang mengikuti VCT, rata-rata tersebut belum semua WBP mengikuti tes HIV. Konteks dalam pelaksanaan program skrining IMS dengan VCT di LP Wanita Klas II A Kota Malang terdiri dari Perencanaan, masalah-masalah, tujuan dan  kebutuhan. Pada perencanaan terdapat MoU atau kerjasama secara tertulis dengan pihak terkait, koordinasi, konseling kepada para karyawan  lapas dan narapidana, serta tanda tangan. Sedangkan pada kebutuhan terdiri atas sarana prasarana, tenaga kesehatan dan pemeriksaan. Pada masalah terdiri dari HIV dan IMS, faktor risiko dan keputihan. Terakhir pada tujuan terdiri dari angka kematian, derajat kesehatan, layanan kesehatan dan pengobatan HIV/ IMS. Kata Kunci: CIPP, Skrining,  IMS, VCT, Lapas

Abstract

Background: People Living with HIV/AIDS (PLWH) often face tough social problems, such as social rejection, stigma, and discrimination. Factors that affect PLWH’s quality of life may include psychosocial factors, economic status, immunological status, social support, and co-morbidity. This study aimed to determine the effect of depression, stigma, and peer support group on the quality of life of people living with HIV/AIDS in Solo Plus peer support group, Surakarta, Central Java.Subjects and Method: This was an analytic observational study with a cross-sectional design. The study was conducted at Solo plus peer support group, Surakarta, Central Java, from January to February 2018. A sample of 100 PLWH was selected for this study by simple random sampling. The dependent variable was a quality life of PLWH. The independent variables were healthy behavior, depression, family income, stigma, peer support, and social support. The data were collected by questionnaire and analyzed by path analysis.Results: Quality of life of PLWH increased with involvement in peer support group (b= 3.40; 95% CI= 0.71 to 6.10; p=0.001), social support (b= 0.70; 95% CI= 0.01 to 1.40; p=0.046), and healthy behavior (b=9.33; 95% CI= 5.30 to 13.36; p<0.001). Quality of life of PLWH decreased with stigma (b= -1.10; 95% CI=-1.84 to -0.36; p=0.003), and depression (b=-4.23; 95% CI=-6.62 to -1.83; p=0.001). Quality of life of PLWH was indirectly affected by income (b=0.06; 95% CI=10.14 to 18.61; p<0.001).Conclusion: Quality of life of PLWH is affected by healthy behavior, depression, stigma, peer support group, social support, and income.Keywords: Quality of life, PLWH, stigma, peer support group, social support Correspondence: Yuli Lutfatul Fajriyah.
