The Influence of Organizational Culture on Performance through Employee Work Motivation

Abstract

Public health centre is a the technical implementation unit of the office regency are responsible for organizing health development in a working area. In the work environment, Puskesmas is increasingly aware of the importance of organizational culture in working for employees, in order to increase employee work motivation, which will encourage employees to improve their performance. But sometimes the implementation of good performance can not run optimally with existing theory as happened at some health centers in Lumajang district. This research aims was to analyzed the influence of organizational culture on performance through employee worked  motivation in the Worked Areas of  Labruk Kidul Public Health centre, Gesang Public Health centre, and Sukodono Lumajang Public Health Center. This type of research was a observational analytic, and used cross sectional approach method. Sample in this study amounted to 167 employees used total sampling technique, 55 employees in Labruk Kidul Health Center, 53 employees at Gesang Public Health Center, and 59 at Sukodono Lumajang Public Health Center. Data analysis technique was done by Partial Least Square (PLS) method. Based on the test results there was an influence of organizational culture on performance through employee worked motivation, with a t value of 8,716 statistics (t statistics> t table 1.96) means that the better organizational culture applied will increased employee worked motivation, so employee performance will also increased. To realize the excellent service oriented on improving the performance of employees, must be supported from all aspects of human resources, infrastructure, methods, environment both internal and external, and good management, so that existing facilities are expected to facilitate all aspects in the field of health in relation to public services which may h ave an impact on improving the performance of technical officers.

Abstract

AbstractRecurrent aphthous stomatitis (RAS) is an oral mucosal lesion in the form of asingle or multiple recurrent ulcer. The etiology is still not known, usually occurs in thesecond decade. RAS is more likely to strike in early adulthood with a tendency moreoften in women than men (3:1). The RAS precipitating factors are infection, trauma,hormones, emotional effect, fatigue, immunological disorders, nutritional disorders,and hereditary. Clinically RAS is in the form of ulcers that are round, single ormultiple, varying in diameter, white-centered, redness in the edge, and painful. The RASClassification: aphthous ulcers major, minor aphthous ulcers and herpetic form.Ulceration is a mucosal damage to the stratum basale, painful and interfere whilechewing food. RAS can heal itself between 7-14 days and even more but theexcruciating pain will torture the patient. Wound healing is a complex process thatinvolves many elements which work in sync to achieve wound healing. Pomegranate isone of the widely studied medicinal plants for health. Pomegranate has benefits as antiinflammatory,anti-oxidant, anti-microbial, immunostimulatory, cheap, easy to obtain,have no side effects. Pomegranate has very complex potential that can eliminate variouspredisposing factors in a single therapy and also can increase endurance. Based on thisfact the purpose of this research is to theoretically study pomegranate as an alternativetherapy for RAS biomolecularly. The conclusion is the active ingredient of pomegranatetheoretically has the functions of anti-inflammatory, anti-oxidant, anti-microbial,immune-stimulatory and can regulate the fibrosis process, so that pomegranate cancure RAS.Keywords: RAS, pomegranate, healing process.

Efek ekstrak buah delima (Punica Granatum L) terhadap ekspresi wild p53 pada sel ganas rongga mulut mencit strain swiss webster

Abstract

Background: Squamous cell carcinoma is the most common cancer in the oral cavity. DNA tests showed that almost 90% of cases revealed wild p53 gene mutations. Wild p53 gene mutations cause p53 inactivation so the cell cycle does not stop in G1 phase but continues to S phase and G2 and M, it makes the mutated DNA remains multiplied and apoptosis does not occur. One candidate of the cancer treatment alternatives is pomegranate extract (Punica granatum L – PGL). Purpose: The purpose of study was to examine the effect of PGL on wild p53 expression in oral cavity malignant cell of swiss webster strain mice. Methods: Thirty- two swiss webster strain mice (Balb/c) 5 months old were randomly divided into four groups. Two control groups (K0: no benzopirene exposed and untreated; K1: benzopirene exposed and untreated); and 2 treatment groups (P1: benzopirene exposed and given EA; P2: benzopirene exposed and given PGL extract). The expression of wild p53 was determined by immunohistochemical techniques. Results: The results showed that administration of PGL could increase the expression of wild p53 in malignant epithelial cells in the oral mucosa of mice, and the expression was higher than EA. Conclusion: This study suggested that the PGL extract could express wild p53 in the oral cavity malignant cells of swiss Webster strains mice.Latar belakang: Karsinoma sel skuamosa merupakan kanker yang sering terjadi pada rongga mulut. Pemeriksaan DNA menunjukkan hampir 90% kasus dijumpai adanya mutasi gen wild p53. Mutasi gen wild p53 menyebabkan inaktivasi wild p53 sehingga siklus sel tidak berhenti pada fase G1 tetapi berlanjut ke fase S dan G2 dan M, sehingga DNA yang mengalami mutasi tetap dilipatgandakan dan apoptosis tidak terjadi. Salah satu kandidat obat kanker adalah ekstrak buah delima (Punica Granatum L - PGL). Tujuan: Penelitian ini bertujuan untuk meneliti efek ekstrak PGL terhadap ekspresi wild p53 pada sel ganas rongga mulut mencit strain swiss webster. Metode: Tiga puluh dua ekor mencit (Balb/c) strain swiss webster jantan berumur 5 bulan dibagi secara random menjadi 4 kelompok, yaitu 2 kelompok kontrol (K0: tidak dipapar benzopirene dan tidak diberi perlakuan; K1: dipapar benzopirene dan tidak diberi perlakuan); serta 2 kelompok perlakuan (P1: dipapar benzopirene dan diberi EA; P2: dipapar benzopirene dan diberi ekstrak PGL). Pemeriksaan ekspresi wild p53 dilakukan dengan teknik  imunohistokimia. Hasil: Hasil penelitian menunjukkan bahwa pemberian ekstrak PGL dapat meningkatkan ekspresi wild p53 pada sel epitel ganas pada mukosa rongga mulut mencit, dan lebih tinggi dibanding dengan pemberian EA. Simpulan: Penelitian ini menunjukkan bahwa ekstrak PGL dapat meningkatkan ekspresi wild p53 pada sel ganas rongga mulut mencit strain swiss webster

Abstract

Background: The high prevalence, complexity and negative impacts of angular cheilitis are affecting the patient quality of life indicates the importance of the disease caring and treating. Collecting data is required to identifying the disease frequency in a population as an early step attempted to planing, preventing, caring, and healing the disease. Objective: to determine the incidence rate of angular cheilitis at Dental Hospital of Jember University on October-December 2015, and the distribution based on age, sex, nutritional status, and residece. Method: This observasional descriptive study used cross sectional survey design. The study was conducted by identifying sample with angular cheilitis patient were found on October-December 2015 at Dental Hospital of Jember University. Result and conclusions: The incidence rate is 6,7%. The distribution based on age, sex, nutritional status and residence are most common in children 5-11 (89,2%), male (62,5%), malnutrition status (52%) and rural (59%), so that the rate is estimate the risk factor of angular cheilitis increase in children, male, malnutrition and villagers.  Keywords: Angular Cheilitis, Distribution, Incidence

Abstract

RAS is more likely to strike in early adulthood with a tendency more often in women than men (3:1). Abstract

The problem of mental disorders in Indonesia is a significant health problem in this society because the disease of mental disorder in Indonesia is still quite high, based on the result of basic health research 2007 the prevalence of emotional mental disorder equal to 11,6% and varies between province and regency. The purpose of this study was to analyze the role of health workers covering preventive, promotive, education and evaluation in handling persons with mental disorders (ODGJ) physical restraint in Public Health Centers of Jember Regency. This type of research was a case study. Determination of informants using sampling technique purposive sampling. Number of informants were 9 people. The results of the research was that all informants have made promotive, preventive, education and evaluation efforts through counseling, screening, home visits and monitoring persons with ODGJ access in taking medication.

The inhibition of malignant epithelial cells in mucosal injury in the oral cavity of strains by pomegranate fruit extract (Punica granatum linn) through Bcl-2 expression

Abstract

Background: Squamous cell carcinoma of oral cavity is a malignant neoplasms derived from epithelia. The malignant neoplasms are cells that have changed their structure and function, and their number becomes increasing abnormally, invasive, and metastatic. Carcinoma can be caused by the resistance of malignant cell apoptosis. Bcl-2 is a proto-oncogene of Bcl family that inhibits the process of cell apoptosis and suppresses Bax protein (pro-apoptotic). The management efforts of cancer diseases, however, still have many obstacles. Thus, the researcher was triggered to explore more herbal plants, namely pomegranate. Pomegranate as a medicinal plant is accessible and cheap. Ellagic acid (EA) is a single active compound derived from whole pomegranate fruit extract (PGL), which has anti-cancer activity as in vitro, but EA is low concentration in plasma, low water solubility, and insoluble in intestinal. These facts prompted the researcher to compare between pomegranate extract, which consists of several active compounds, and that, which only consists of ellagic acid. Thus, this research is expected to know how some active compounds can work synergistically in the PGL, so the effect can be more potent. Purpose: The purpose of this research, therefore, was to compare EA with PGL in reducing the expression of Bcl-2. Methods: This laboratory experimental research was used 32 male mice (Balb/c) in the age of 5 months. They were randomly divided into 4 groups: 2 control groups (K0: which was not exposed with benzopirene and also untreated and K1: which was exposed with benzopirene and also untreated), 2 treatment groups (P1: which was exposed with benzopirene and also treated with EA and P2: which was exposed with benzopirene and also treated with the PGL). Next, an examination was conducted by using immunohistochemical techniques. results: The results then showed that the provision of the PGL could decrease the expression of Bcl-2 significantly higher than that of EA in the malignant epithelial cells of the oral mucosa of those mice. Conclusion: It may be concluded that the provision of the PGL can kill malignant cells in the oral cavity of mice by increasing apoptosis through decreasing Bcl-2 expression that was higher than the provision of EA.latar belakang: Karsinoma sel skuamosa rongga mulut adalah istilah yang digunakan menyebut neoplasma ganas berasal dari epitel. Neoplasma ganas adalah sel yang telah berubah struktur dan fungsi, sehingga mengalami peningkatan jumlah secara abnormal,invasif dan metastasis. Terjadinya karsinoma salah satu disebabkan oleh karena hambatan apoptosis terhadap sel ganas. Bcl-2 adalah protoonkogen keluarga Bcl yang berperan menghambat proses apoptosis sel dan bekerja menekan protein Bax (pro apoptosis). Berbagai upaya penatalaksanaan penyakit kanker masih banyak menemui kendala,sehingga peneliti menggali tanaman obat yaitu buah delima. Buah delima sebagai tanaman obat, mudah didapat dan harganya murah. Ellagic acid (EA) senyawa tunggal bahan aktif dari ekstrak buah delima yang memiliki aktivitas sebagai anti kanker secara in-vitro tetapi EA aktivitas dan konsentrasinya dalam plasma rendah, kelarutan dalam air rendah, metabolisme (EA) tidak larut dalam intestinal. Fakta ini mendorong peneliti untuk membandingkan dengan whole ekstrak delima (PGL) yang terdiri dari beberapa senyawa bahan aktif, tidak hanya ellagic acid, memungkinkan beberapa senyawa bahan aktif pada PGL bisa bekerja sinergis, sehingga efeknya lebih poten. tujuan: Tujuan penelitian ini membandingkan antara EA, dengan whole ekstrak buah delima (PGL) dalam menurunkan ekspresi Bcl-2. Metode: Metode penelitian yang digunakan adalah eksperimental laboratories, 32 ekor mencit (Balb/c), jantan, umur 5 bulan dibagi secara random menjadi 4 kelompok, 2 kelompok kontrol (K0: tidak dipapar benzopirene dan tidak diberi perlakuan, K1: dipapar benzopirene dan tidak diberi perlakuan), 2 kelompok perlakuan (P1: dipapar benzopirene dan diberi EA, P2: dipapar benzopirene dan diberi PGL). Pemeriksaan dengan menggunakan teknik imunohistokimia. hasil: Hasil penelitian menunjukkan bahwa pemberian PGL dapat menurunkan ekspresi Bcl-2 lebih tinggi dibandingkan EA pada sel epitel ganas mukosa rongga mulut mencit. Kesimpulan: Kesimpulan penelitian ini adalah pemberian PGL dapat membunuh sel ganas pada rongga mulut mencit dengan jalan meningkatkan apoptosis melalui penurunan ekspresi Bcl-2 lebih tinggi dibandingkan pemberian EA.

Abstract

RSD Balung is a BLUD hospital, but the tariff determination had not been based on unit cost including the operation in the Installation of Hospital Operation Room Service Operations continue to show an increased, so operational expenses were increased. In JKN, there were several surgical actions that have a tariff difference that harms the hospital. The tariff claimed by hospital to BPJS was not as expected because the nominal tariff was less. The purpose of this research was to arrange tariff based on unit cost of operation cost action of room installation room of RSD Balung with Activity Based Costing (ABC) method. This research was a descriptive quantitative. The unit of analysis of this research was the Operation Room in RSD Balung by looking at the cost and service component of the service or the action to be done the operational action tariff analysis. Respondent was taken by pusposive sampling. The number of respondents in this study were 13 people. At the service of BPH, SC, and cataract in Operation Room RSD Balung there were difference of tariff either from BLUD tariff, tariff based on unit cost, and INA CBGs tariff. This difference was particularly evident from BPH services. This service was harming RSD Balung. While in SC and Cataract service, unit cost tariff was lower than BLUD tariff.

Effects of Service Quality on Word of Mouth through Satisfaction in Outpatient at Sakinah Islamic Hospital in Mojokerto Regency

Abstract

Hospital is health service institution organizing individual health service. In an increasingly competitive environment, hospitals should be increasingly aware of the importance of providing the best quality service for the patients. In Sakinah Islamic Hospital showed the decrease of the number of outpatient patient visits, on 2014 until 2016, decreasing by 697 patients. This research aimed to analyze the effects of service quality on word of mouth through patient satisfaction. This was quantitative research with observational analytic approach. The samples of this research were 90 outpatients using multistage sampling technique. The data analysis was conducted by using Partial Least Square. Based on the testing result, it was found the effects of service quality on word of mouth through satisfaction, with t-statistic value by 2.039 (t-statistic > t table of 1.96). It means that the better service quality provided, the higher the level of patient satisfaction and increase the word of mouth as well. Sakinah Islamic Hospital needs to improve the quality and quantity of the human resources, equip facilities infrastructure and conduct maintenance and repair to support the service continuity for the patients, therefore, they will be satisfied and encouraged to do word of mouth and increase the number of patient visits.
