Abstract

Appropriate diet and physical activity, maintaining a healthy body weight and propercontrol of diabetes using oral anti diabetic (OAD) in people with diabetes type 2 will helpreduce its complications. However, some advance diabetic cases need insulin-based regimen toaggressively lower blood glucose level. Local data was insufficient to determine whether type 2diabetes patients on insulin were indeed attaining glycemia goals. The aim of this study was toassess the clinical response of type 2 diabetic patients who were switched to another insulinregimen to achieve glycemic control. The study was performed as a non-interventional prospective disease registry that mirror real life management of these subjects after a 3 month follow up. To assess the clinical response, we performed measurement of fasting blood glucose level, HbA1c, waist circumference and body weight. One hundred and fifteen type 2 diabetes patients who were already on insulin therapy with unsatisfactory control of glycaemia parameters were admitted in this study of an actual clinical setting. Only 93 cases have been examined in a 3 month follow up. Premix, Short-acting and Intermediate-acting insulin were the most popular insulin-based regimens that previously been prescribed, while basal analogue insulin is the most common regimen for switching therapy. The newly prescribed insulin-based regimen showed significant reduction of important glycaemia parameters such as fasting blood glucose level and HbA1c. Serum blood glucose was decreased 61.76 mg/dL (p<0.001) and the level of HbA1c was also decreased 1.39 unit (p<0.001). In addition, waist circumference was significantly reduced for 0.70 cm (p=0.041). However, there was a slight increase of body weight althoughnot statistically significant (p=0.389). We concluded that switching insulin-based regimen for unsatisfactory although glycemic control patient gave the probability to improve clinical response. Abstract

Earning management has become the practice in business. Yet, not all of the companies dothis earning management. The study aims to investigate earnings management behaviormeasured by real and accruals transactions i.e. abnormal cash flow of operation, abnormalproduction cost, abnormal discretionary, short-term discretionary accruals, and long-termdiscretionary accruals. The study applies a perspective of prospect theory to predict earningsmanagement behavior of management. In prospect theory, it is predicted that managers tendto manage earnings to avoid negative earnings. Positive earnings around zero is an indicationof earnings managed to avoid negative earnings. It shows that most of Indonesian publiccompanies tend to manage earnings based on real transactions than accruals transactions. All proxies of real earnings management support hypotheses that positive earnings aroundzero are managed through the real transactions. On contrary, only long-term discretionaryaccruals which support hypothesis revealing that positive earnings around zero is managedthrough accruals accounts. This result indicates that the Indonesian public companies engagein earnings management in accordance with the predictions of prospect theory, especiallythose based on real transactions.

Abstract

Background: Chronic Helicobacter pylori (H. pylori) infection affects the mechanisms of gastric mucosal protection. In patients with diabetes mellitus, data on the prevalence of H. pylori infection are scanty and contradictory. We have examined, using histological fixation technique, the thickness of the adherent mucus gel layer in the gastric mucosal and H. pylori infection in dyspeptic patients with or without diabetes. Method: A cross-sectional study was conducted in 86 dyspeptic patients consisted of 43 diabetics and 43 non-diabetics patients. In all cases, upper gastrointestinal endoscopy were performed, measurement of the gastric corpus and antral mucus thickness was carried out at the corpus and antral biopsy specimens were snap frozen and cryostat sections were stained using a hematoxyline eosin. One biopsy within 2 cm of the pylorus was examined for detection of H. pylori status by using polymerase chain reaction (PCR). Results: In all sections the mucus layer was continuous. At the gastric corpus and antrum, the mucus thickness of diabetic patients was thinner than non diabetic patients: 35.2 ± 2.2 µ m and 43.9 ± 3.8 µ vs 45.2 ± 2.5 µ m and 51.2 ± 2.2 µ m. The results were significantly different (p = 0.001). The difference of H. pylori prevalence between diabetics (52.5%) and nondiabetics patients (47.5%) was not significant (p = 0.67). Conclusion: This study shows a significant thinning of the adherent mucus gel layer both in diabetic patients and H. pylori-positive individuals. No difference has been found between patients with H. pylori infection and diabetes mellitus.   Keywords: mucus thickness, Helicobacter pylori infection, diabetic patient

Abstract

Kata Kunci : BUMN, Manajemen Laba (Discretionary Accrual), Kinerja Saham (Return Saham), Financial Performance (Return On Equity).

Abstract

The research was conducted in Selecta Recreation Park since the increase of competition in the tourism industry in Kota Batu. As the oldest tourist which features nature, Selecta Recreation Park has to have a strategy in order to be able to compete with other tourism entrepreneurs. To realize this strategy, the company should have a good performance measurement system. The purpose of this study is to analyze the performance of Selecta Recreation Park by using the Balanced Scorecard method. The use of the Balanced Scorecard is to measure the performance of companies through the four perspectives which are: financial perspective, customer perspective, internal business processes perspective, and learning and growth perspectives. From the results of measurements that have been achieved, it can be seen that overall the performance of the Selecta Recreation Park has been good. It can be seen from scorecard tables that exist in every perspective. In the financial perspective suggests that performance on this perspective is good. In the customer perspective, the scorecard table shows that the performance is already good. In the internal business process perspective, it can be concluded that this perspective is considered very good. In the learning and growth perspective, researcher concludes that the performance is also good. The conclusion that can be drawn from the results of the analysis that was done, is that the Balanced Scorecard is a performance measurement tool that is suitable to be applied in Selecta Recreation Park. This is because the Balanced Scorecard is able to measure non-financial performance such as customer perspective, internal business processes perspective and learning and growth perspective.
