Abstract

The medical doctor in health centre (HC) is the most important key person to improve and maintain the  HC performance. Since 1992, the Ministry of Health of Indonesia (MOH-RI) has implemented a contract scheme for medical doctor in the HC. This study is an explorative study in searching major factors influencing the performance of contract medical doctor. Another objective of this study is to develop an approriate supervision model of the contract medical doctor in HC. Respondent consist of key policy makers at MOH-RI, provincial health officer and district health officer from Central Kalimantan, East Nusa Tenggara and South-East Sulawesi. Other respondent are 280 contract medical doctors who has been worked more than one year from 11 selected provinces. Focus group discussion, indepth interview and observation are used as the main methods for data collection at the policy makers level. Mailing questionaires are sent to the contract medical doctor as the main data collection method. The result of this study shows that age, motivation before and during employment, boring situation, job satisfaction and overtime of works are significant factors in influencing the performance of contract medical doctors. The most important finding is the uncertainty of post employment contract that made low performance of contract medical doctors. The creation of good environment, team works, good and appropriate supervision, and orientation of the HC programs in matching with the need and demand of the local problems should be carried out in improving the contract medical doctors in HC. Post employment contract should be prepared appropriately by the MOH-RI in order to increase the performance of contract medical doctor optimally.  Key Word: contract medical doctor, performance.

Abstract

The purpose of this study are to identify: 1. Each of managerial function of a nurse principal in planning, organizing, actuating, controlling, supervising and evaluating of nursing care; 2. Selection process, nursing management training, job discription, nursing standard of inpatient wards, roles of the hospital directors and hospital organization structure in nursing care. Structural and indepth interviews have been done to 88 nurse principals and 8 hospital directors in 8 province hospitals (Dili, Kupang, Mataram, Ujungpandang, Palu, Kendari, Jayapura and Ambon). Indepth interviews have also been caried out to 8 head of nursing department in those 8 hospitals. Documents and reports searching have also been done. Descriptive analysis showed that most of nurse principals have not performed their managerial functions. Selection process has not yet been done objectively and only one nurse principal has already met the nursing management training program requirement. The directors of hospitals have performed their roles in motivating the nurse principals in their management roles. However, the hospital organization structure has not yet linked with the job discription for nurse managers. Nursing standards and job discriplion m the hospitals have also not yet been operationally described Based on the results of this study, nursing task force must be created appropriately in line with the hospital organization structure and the selection procedure of nurse principals in order to improve their performances in nursing management.   Key Words : Nurse principal, nursing management, hospital.

Abstract

The urgent and complexity challenge in implementation the UU No. 22/1999 about Governmental Autonomy, is the ability of health human resources to be entrepreneur, leader and manager. Data and information showed that health human resources is not well proper with those kind of ability which is needed to handle the impact of governmental autonomy in health services. This study is to assess those ability of health human resources in Dinas Kesehatan Kabupaten/Kota, Puskesmas and Rumah Sakit Umum Kabupaten/Kota, through anquette, depth interviews and secondary data review. Result of this study showed that 51.40% of respondent have less of entrepreneurship ability, specially in the components of marketing, taking risk and job orientation. There are 62.90% respondents with the less ability of leadership, mostly in the supportive and prestige orientation components. The ability to manage conflict, 80% respondent is also in the less category. Strategic management ability that has been assessed through the strategic plan in each institutions showed that it is not as in the normative form of this study. Conclusions of this study is that health human resources has less ability to entrepreneur, lead and manage. Suggestion of this study is to continue the process in managing health services with capacity building through training specially in the marlceting component of entrepreneurship. Also an upgrading course on applied strategic management process through development of strategic planning in Dinas Kesehatan Kabupaten/Kota, Puskesmas and Rumah Sakit Umum Kabupaten/Kota.   Keywords: entrepreneurship, human resources

Abstract

A disaster is incident which can be avoided although it is naturally. But if it happenned, there are extra ordinary activities which should be managed as an emergency state. Disasters which happenned in Nangroe Aceh Darussalam, Nias and Alor is a lesson teamed process that in health mechanism of emergency response there are things that can be anticipated. Mobilization of sources included health personnels could be done by provinces, districts or municipalities which were nearest to the disaster areas. This mechanism is built with management and operational tasks which is clearly explained, what should be done in the emergency response. This is a case study which data were collected through interviews and secondary data. The respondents were purposively selected. Analysis was to answer the objectives of the study that is to develop a health mechanism prototype in disaster as lesson teamed on Nangroe Aceh Darussalam, Nias and Alor cases. The prototype of health mechanism in disaster was developed on lesson teamed from interviews and secondary data in Nangroe Aceh Darussalam, Nias and Alor. The prototype is developed with consideration on local specific disasters. The document of this prototype should be added with policies, including mapping and identification of local specific disasters. These all would be things that could be completed in preparedness, mitigation and prevention of disaster.   Keywords: Prototype, health mechanism, disaster

Abstract

This article describes the theory of Kirkpatrick on 4 level evaluation as a tool to measure a post training evaluation. The implementation on a post evaluation of research methodology training was conducted to 13 study program lectors of health polythecnics in Surabaya. Development instruments to measure each level evaluation according to the Kirkpatrick guidance was done. The validity and reliability test for development instruments showed significant at a = 0. 05. The kirkpatrick 4-level evaluation can be used as a tool to measure objectives of a training program, especially for post training evaluation on participants and also the post training on participants in their daiiy works. The development instruments to measure each of the level could not be separated from goals to be achieved of the training that has been planned in a comprehensive training program. Key words: Kirkpatrick 4 levels evaluation, post training, evaluation

Abstract

A Province Health Office has an important role to conduct post hospital accreditation survey monitoring. In Indonesia the hospital acreditation survey was implemented since 1994, but most of the accredited hospitals had not yet received post survey monitoring from Province Health Offices. Actually this monitoring was a part of the accreditation cycle that has purpose for continueing quality improvement at the hospitals. The reason for not yet optimal performences was differences of perception about the Hospital Accreditation concept among the Province Health Office personnels who have the capability to do the monitoring. This is why Hospital Acreditation socialization still seriously needed to develop an understanding of the concept among the Province Health Office personnels so that monitoring the activities of the post accreditation surveys can be done continously. Key words: Province Health Offices, monitoring, post hospital accreditation survey

Abstract

The married women with unwanted children to delay the next children but did not using family planning method is defined as not fulfilled or unmet need for family planning. The unmet need could be due to unadequacy acces to information on family planning services. Good knowledge is achieved by adequate information so consideration on decide to select rational contraception for users could be fulfilled. This research aimed to determine health providers in providing qualified family planning services in health centers and district hospital. It was a descriptive study, located in Malang and Trenggalek Disrict in East Java Province In each District, 1 (one) urban health center, and 1 (one) rural health center and also 1 (one) district hospital were selected purposively. The population of the study were family planning providers selected health centers and district hospitals for MCH/FP services programmers and providers for FP services at HC, sub HC , village delivery Health and also programmer and provider of FP services at the District Hospitals. Data were collected by interview with qustionair guide with variables adopted from self assessment instrument they were analysed descriptevely. Results should that the understanding of FP service provider at HCs for information on FP method was at the average good, meanwhile at Malang and Trenggalek District Hospital was low. Furthermore, the understanding of FP provider at HC for the information on informed choice, informed consent and client for medical (feasibility) at the average was good but at the district hospital was low. It suggests to enhance the understanding of FP service providers at HCs, and District Hospitals on contraceptive methods, informed choice, informed consent to provide qualified FP services.Key words: understanding, provider, information family planning

Abstract

An assessment was done by Budijanto, etc (2002) about the leadership, entrepreneurship and managerial ability of district health human resources.  Result of this assessment was used to developed a draft module to increase the health human resources ability in entrepreneurship mainly. The module Health Development Through Entrepreneurship, a Guide for Marketing Strategy in Health Services in Hospital and Health Center". Implementation of the module was done in the District of Kutai Kartanegara, East Kalimantan Province, District of East Lombok, Nusa Tenggara Barat Province and District of Magetan, East Java Province. Respondent in this study were health human resources from General Hospital, Health Center and District Health Office who participated in the implementation process. Implementation was done in three days through learning the contents of the module, filling workpapers as a reflection of understanding the contents of the module and discussion. Evaluation of the implementation process was done using level I of Kirkpatricks evaluation to get the reaction of the respondents about the conformity of the modules contents and the learning process. The indicators of advantages of implementation process is > 60% respondents will react that the modules contents is relevan with their everydays work and teaming process was also accomodate. Using  scores, indicates that < 2 means less, 2-3 means sufficient and > 3 means good in reacting that the modules contents is relevan with their everyday & work and learning process. The result showed that more than 60% respondents in the District of Kutai Karlanegara (n = 22). District of East Lombok (n = 25) and District of Magetan (n = 22) valueing that the contents of module is relevan with the institutional programs. More than 60% responden in those three district valued that the learning process was sufficient and good. Implementation the module through learning process can be done with using level I Kirkpatricks evaluation to valued the process. Learning process should be continued with coaching in implementation the contents of module in everydays work in Health Center or in the Hospital wich are also a method to value the advantage of the implementation process.

Abstract

Health centres (HCs) were in the period of transition into business institutions and therefore it needs marketing strategy such as Customer Relationship Marketing by managing data of patients as customers. Understanding the customer relationship data management was conducted through implementation of module Entrepreneurship in Health Services with Customer Relationship Marketing. The objective of this study was to identify the understanding of human health resources in HCs about customer relationship data management through module implementation. The locations of the study were in Loa Ipuh and Sebulu 1 HCs in Kutai Kartanegara District, East Kalimantan Province. Respondents were human health resources of HCs who participated in the implementation process. Data were collected by conformity of the module contents with its standards of six variables on customer relationship data management in the learning and coachmg processes. The results showed that in learning process the respondents in Loa Ipuh and Sebulu 1 HCs were on the category of less understood. But in the coaching process, the category of respondents in both HCs was of understood. One of the reasons was that coaching process helps to understand the data management in the implementation process through discusston which followed by problem solving. The implementation of the customer relationship data managing needs a change in the human resources mindset not only in the HCs but also in the supra sytem as the District Health Office and District Governments. Therefore customer relationship data management should be represented among them to achieve the same perseption in striving HCs into busniess institutions.   Keywords: customer relationship data management

Abstract

This article describes the concept, relevance, principles and milestones about policy. Based on William Dunns framework, it also tries to describe the purposes of policy analysis and its implementation that are needed on health decentralization. In Indonesia the health decentralization has been implemented for some years but policy analysis is understood as regulation analysis which has been directed by the Department of Health. At present the implementation process of the health decentralization, the Central government still has double functions as policy maker and performer In fact based on regulations, the responsibilities have been delegated to District governments. Understanding the policy analysis on the implementation of health decentralization would provide challenges to interpretate in accordance with the local specific conditions.   Key words: policy, analysis, decentralization of health
