Abstract

Recurrent aphthous stomatitis (RAS) symptoms usually painful and sometimes incapacitating. This condition is severed by recurrent characteristic and many efforts are conducted to encounter these things, but the results are still not satisfying. The etiology of RAS is still uncertain, however it is suggested that there is a correlation between the occurrence of RAS in women in their menstruation and premenompause period where there is noted decrease of estrogen. This article reports a case of RAS in a 50 year old woman who have suffered RAS for a year and become more severe in relation to the menstrual cycle especially pre menopause period. After estrogen administration the lesion is healed. This case brings attention to the clinicians, that it is important to know the estrogen level of RAS patients who are in premenopause period so they will receive an appropriate therapy.

Abstract

Diabetes Mellitus (DM) is a chronic disease complex with metabolic and vascular component and characterized by hyperglycaemia. There are over 200 million people in the world with DM and still be the center of interest because it is the third leading cause of death in USA. Approximately 3-4% of the dentists adult patient has DM. In uncontrolled DM besides the major symptoms (3Ps) there are several oral manifestations such as xerostomia, candidosis, periodontal disease, sialosis, etc. Dental patient with those findings should be screened for hyperglycaemia or referred to a clinical laboratory or physician for screening test. As member of the professional healthcare team, dentist must be able to detect new cases of DM and to render dental care to patients who are already treated for DM without endangering their lives.

Abstract

Background: In many developing countries, like Indonesia, most patients with oral mass lesions, i.e. infection, inflammation, even cancer came to the hospital at late stages of the disease. Some of them afraid of endure suffering a killer disease, either after a biopsy or even should undergo an operation. Fine needle aspiration (FNA) biopsy is a technique rarely used to diagnose oral ad oropharyngeal lesion. This technique seems promise a convenienve, painless, accurate, safe, and easy to use to help patients and surgeons, and have been demonstrated repeatedly. Objectives: This case report emphasized many advantages of FNA biopsy to examine oral mass lesions and its metastasis to lymph node. Materials and Methods: Under a local anesthesia, incisional biopsy and FNA biopsy were done on four cases of oral mass lesions. All samples were prepared for microscopic examination with Hematoxylin-Eosin (HE) staining and with Diff-Quick or Papaniculaou staining. Results: Incisional biopsy shoed features of malignancy as the same as FNA biopsy. There were bigger cells with high ratio between cells and nucleus, hyperchromatic nucleoli, and mitotic cells that refers to Squamous cell carcinoma. FNA biopsy from the nearest lymph node showed clumps atypia cells, prominent nuceloli, nuclear hyperchromatic and pleomorphism. Conclusion: FNA biopsy could well diagnose on oral mass lesions and the adjacent lymph node. FNA biopsy has many advantages particularly its convenience, painless, ease to use, cost effectiveness, and accuracy.DOI: 10.14693/jdi.v16i2.92

Abstract

Objective: to determine the daylong effect of Antiplaque® toothpaste active copound and the effect of tooth brushing or combination of tooth brushing and tongue cleaning on Volatile Sulphur Compounds (VSC) levels. Methods: Clinical experimental double blinded microbiological and clinical examination. Subjects mainly from dental faculty student with or without a chief complain of halitosis. One hundred and twenty subjects were divided into four groups that consist of thirty samples each group. Two group are treatment subjects were divided into four groups that consist thirty samples each group. Two groups are treatment groups using Antiplaque® toothpaste and two groups are control groups using placebo toothpaste. The daylong effects of VSC levels are measured by Halimeter and organoleptic. The intraoral status were measured including periodontal status and tongue index. Result: There are a reduction of VSC levels in both treatment and control group, but the reduction only significant in group using Antiplaque® toothpaste (Wilcoxon signed rank test, p<0.05). There are a reduction of VSC levels in both treatment using Antiplaque® toothpaste, but the combination of tooth brushing and tongue cleaning reduced VSC levels significantly lower than tooth brushing treatment (Mann Whitney U test, P,0.05). Conclusion: Tooth brushing and tongue cleaning using Antiplaque® toothpaste significantly reduced VSC levels.DOI: 10.14693/jdi.v16i2.95

Abstract

Treatment of Recurrent Aphthous Stomatitis (RAS) often becomes a problem until now. It is because of the RAS characteristic that has a high recurrence, unclear etiology and has a lot of predisposing factors. Antiplaque™ tooth paste with active ingredients of Sodium monophosphate, Arnica tincture, Cloxiphenol and Oleum cariofill, has been used in this study to accelerate healing and to reduce the recurrence of RAS. Ninety one students and employees of Faculty of Dentistry, University of Indonesia who have minor RAS as subjects were divided into study group and control group. Regarding of healing the results of 58.7% from study group compared to 35.9% from control group, implies that the lesions healed faster. RAS recurrence of 83.3% from study group compared to 42.9% from control group states that recurrence had decreased.

Abstract

Oral lichen planus (OLP) is a chronic inflammatory condition that effects oral mucous membranes with a variety of clinical presentations including reticular, atrophic, plaque and ulcerative lesions. Corticosteroid is one of the effective therapy for OLP in reducing the sign and symptoms of this disease, but this therapy have a serious side effect, therefore to administering one must consider the patient’s systemic condition. We reported a case of OLP in 52 years old female patient who has no history of diabetes mellitus and unknown glucose level. Management of this patient included application of corticosteroid swish, topical 0.05% clobetasol propionate and 0,1% triamcinolone acetonide, antimycotics, improvement the oral hygiene status and referral to internal medicine specialist. The next treatments plan challenging because unstable blood glucose level which in turn effects the drug choice and teeth extraction plan. We concluded that the treatment of OLP requires a complete assessment of medical status and lab studies specially on the first visit so the drug selection with corticosteroid therapy and the treatment planning of predisposing factor are effective in reducing the sign and symptoms of OLP with minimum systemic side effect.DOI: 10.14693/jdi.v16i1.19

Abstract

Pemphigus vulgaris is an autoimmune–mediated disease of skin and mucous membran leading to progressive blistering and chronic erosions. It often begins with blister formations which easily rupture. The characteristic feature is positive nikolsky sign which may or not be presented simultaneously. Establishment of early definite diagnosis is critical and requires correlation of clinical and histopathological findings. Because of this conditon is a potentially life-threatening, the risk of complications and mortality rate increases if initial management is non comprehensive and inadequate. Treatment is directed at supression of autoimmune process, typically administration of corticosteroids. This article report a case in a 51 years old woman who had painful chronic oral ulcer and poor general health condition. Prior to the visit to Oral Medicine clinic, patient was treated by her general practitioners for several months, without either established diagnosis nor comprehensive and adequate management, so that she had no clinical improvement. Clinical examination at the first visit in Oral Medicine clinic Cipto Mangunkusumo Hospital, revealed bula in the skin, conjunctivitis, easily bleed oral mucosae, widespread erosions and ulcerations of the lips, gingiva, tongue, and buccal mucosae. Biopsy of one of skin erosions demonstarting suprabasal intraepithelial acantholysis. Multi divisions in Hospital, such as Oral Medicine, Dermatology, Internal Medicine, Ophtalmology, ENT, were involved in treating this case. Patient received high dose methylprednisolone sistemically and prednisone topically for the lips. General remission achieved in several days. Thus, establishment of early definite diagnosis and adequate management are important in management of Pemphigus vulgaris.DOI: 10.14693/jdi.v16i1.20

Abstract

Behçet’s Disease (BD) is a multisystem inflammatory disorder characterized by recurrent oral-genital ulcers, ocular inflammation and frequently involves the joints, skin, central nervous system and gastrointestinal tract. We reported a case of severe BD in 30 years old male patient with oral-genital ulcerations and ocular involvement. He had suffered from the disease since twelve years ago and ocular involvement has lead to blindness of the right eye. Eye drop treatment for eight years, was suspected to cause lichenoid lesion on the buccal mucosa. Management of this patient included the application of topical steroid, 1 % povidone iodine and oral hygiene promotion. We concluded that disease condition, poor oral hygiene, disability and less compliance of the patient could slowed the acceleration of lesions healing. Elimination local exacerbating factors and a good coordination between dentist-patient and among medical team work are required to achieve the optimal recovery and to minimize complication of its treatment in the oral cavity.DOI: 10.14693/jdi.v15i2.67
