Abstract

Background Prevalence of cardiac complications in children 'With human immunodeficiency virus (HIV) infection have increased, in association 'With the availability of antiretroviral (ARV) therapy and the decrease of opportunistic infections. However, studies on cardiac complications in HIV patients in the various HIV clinical and immunologic categories have been limited. Furthermore, cardiac complications in Indonesian HIVinfected children have never been reported.Objectives To detennine the prevalence of cardiac complications in HIVinfected children and to compare ventricular function and dimensions based on HIV clinical and immunologic categories. Methods A cross????sectional study was done in the Department of Child Health, Cipto Mangunkusumo Hospital, Jakarta between October and December 2009 on 7 4 HIV????infected children aged below 15 years. Children with severe chronic or acute systemic diseases were excluded. Investigations included physical examinations, 12????lead electrocardiography and echocardiographyto assess left ventricular dimension and ventricular function. Cardiac findings among children in different clinical and immunological categories were compared.Results Five children showed left ventricular fractional shortening below 25% and 16 had right ventricular dysfunction. Mean cardiac function and dimension were in the normal range. No differences in cardiac function and dimension, among the clinical HIV category groups (p>0.05) or immunologic suppression status groups (p>0.05) were observed.Conclusion In children 'With HIV infection, the prevalence of left ventricular dysfunction and right ventricular dysfunction was 7% and 22%, respectively. No differences in cardiac function and dimension were found among the different HIV clinical and immunological categories.

Abstract

The definite diagnosis of cardiac disease in infants and children usuallycannot be made on the clinical evidence alone; in most instances supportingexaminations are required. lt is understandable, therefore, that non-cardiologists might suggest that normal subjects are thought to have cardiac problems; the reverse is also true: infants and children with cardiac disease may be ignored. This study aimed to examine the clinical and laboratory findings of normal infants and children who were initially suspected to have cardiac disease. Of 3601 patients referred to our OPD of the Division of Cardiology, Department of Child Health, Medical School, University of Indonesia, from January 1983 to December 1992; in 1782 patient (49.5%) no cardiovascular problems were detected. Most of them (66.2%) were of the age of less than 1 month. Most of the referring physicians (66.3%) were general practitioners. The referring diagnoses were congenital heart disease (286), cardiomegaly (197), rheumatic fever or rheumatic heart disease (110), and syndromes with cardiac involvement (104). The diagnoses were based on dyspnea, cyanosis on crying, chest pain, joint pains, and easy fatiguability. Murmurs found on examination were systolic in 355 patients (19. 9%), and continuous in 6 patients (0,33%). No diastolic murmurs were noted. The final diagnoses were normal (including innocent murmurs and sinus arrhythmias) in 85.8%, mild cardiomegaly in 10.4%., breath holding spells in 2.0%, sinus tachycardia in 0.9%, polyarthritis in 0.2% and other in 0.7% of all cases. More practice in cardiac physical examination is needed for medical students to reduce the unnecessary referrals.

Abstract

Background Pulmonary arterial hypertension (PAH) is a common complication seen in those with a left-to-right shunt congenital heart defect (CHD). Corrective procedures by surgery or catheterization are the therapies of choice for reversible PAH. Since morbidity and mortality due to PAH after correction is high, sildenafil has been used as a selective vasodilator of the pulmonary artery, in order to decrease pulmonary arterial pressure. Objectives To evaluate the effect of sildenafil on pulmonary arterial pressure and clinical outcomes after left-to-right shunt CHD corrective procedures. Methods Left-to-right shunt patients aged < 18 years scheduled for corrective treatment were randomized in a double-blind fashion, to receive either oral sildenafil or placebo, given on days 3 to 30 after the corrective procedure. Clinical and pulmonary arterial pressures were evaluated by echocardiography before, 3 days after, and 30 days after the corrective procedure. Results From July 2013 to June 2014, 36 patients were included in the study: 17 in the placebo and 19 in the sildenafil groups. There were no differences in pulmonary arterial pressure or in clinical outcomes after corrective procedure between the two groups. There were no adverse events during the treatment. Conclusion Sildenafil has little effect on decreasing pulmonary arterial pressure, as most of our subjects seem to have hyperkinetic PAH. As such, pulmonary arterial pressure returns to normal soon after corrective procedures.
