Abstract

The Community Health Efforts (Usaha Kesehatan Masyarakat/UKM) and Individual Health Efforts (Usaha KesehatanPerorangan/UKP) Programs were established to enhance services towards more promotive and preventive than curative and rehabilitative services. Accordingly, the budget allocated for the UKM should be more than or balanced with the UKP budget. Due to the abscence of benchmark for ideal proportion of the UKM and the UKP budget at the District/Municipal levels, the budget allocated for both efforts were highly depending on decision of local authorities and the budget allocated by the Central level. This study aimed to determine the ideal proportion of UKM and UKP budget at the Districts/Municipalities having Index Poverty Level level equal or above 34.4. This was an observational study using a retrospective design. Data were collected at 6 (six) District/Municipal Health Offices on Health Budgets allocated during year 2003-2005 and analyzed quantitatively and qualitatively. Results showed that the proportions of budget allocated for the UKM and UKP were varied from 49.9%-57.2% for the UKM and 42.8%-50.1% for the UKP. Eventhough there was an increased of UKM budget which meant more services toward promotive and preventive activities, the services provided by the UKM were more towards curative than preventive functions. Furthermore, both efforts have continuum services from promotive to rehabilitative services. To conclude, the concept of UKM and UKP needs to be redesign in order to accomplish theDepartment of Health Grand Strategy for promotive and preventive services before stating the benchmark for the UKM and the UKP budget proportions.Key words: the UKM, the UKP, health-budget, districst, municipalities

Abstract

It was predicted that life expectancy rate is increasing in the future. There will be more people aged over 55 years old, and in consequences of the aging process various health problem may arise. This study was conducted among 150 respondents aged over 55 – 65 years, from 3 Puskesmas in South of Jakarta from 1997 to 1998. It was found that after intervention program there were changes in the gingival health status (40%) which is statistically significant (p<0,05), where as the normal teeth function > 20 teeth (50%). There were no differences found about the dental and oral health knowledge of the elderly people among the 3 Puskesmas.

Abstract

Reviewed indicators of Nationa Household Health Survey (NHHS) 1995 were done to know whether the target of dental health for all by the year 2000 which was the program goals has been achieved. The indicators reviewed were index DMF-T, prevalence of dental caries, percent people with with complete teeth. Index DMF-T is a total number of D-T (decayed teeth) score, M-F (missing teeth) score and F-T (filled teeth) score that shows level of seriousness of dental damage caused by dental caries/cavity. The findings showed that indicator “DMF-T index among children aged 12 years” was low (2.21) and achieved the target. But it is not a positive one finding that the score was dominated by D_T score which have not been took up yet, and the high prevalence of population with dental caries experience which was covered 77% of children aged 12 years. Indicator “population aged 18 years that retain all their teeth” is not a sharp one because it is only show the retaining of teeth without knowing the conditions of teeth, loss, or caries. Target year 2000 that was “ 75% population aged 35-44 years have a minimum of 20 functional teeth” can not been achieved. Indicator of “children aged 5 years with caries free” was not collected by SKRT 1995, but it is needed to monitor the program prevention. Indicator ‘population with deep pocket” is important to know the damage of teeth that caused by periodontal diseases. It is recommended to have a comprehensive services which consist of promotion, protection, early diagnosis and prompt treatment. Indicator ‘prevalence of caries” on a couple of age groups is needed, and also indicator of “children aged 5 years with caries free” and “population with deep pocket” specifically on age 35-44 years and 65+ years.

Abstract

Life expectancy rate is increasing in the future. Elderly will be more every year was predicted 2005 - 2010 will be 19 million or 8,5% (BPS, 1997). Dental oral health problems, malocclusion, communication, aesthetics, and very important, which can cause oral diseases on systemic diseases of the body due to oral focal infections. Sample : 150 respondents 55 - 65 years old, from 3 Puskesmas in South of Jakarta, 1997 - 1998, who lived in areas Puskesmas. With statistically significant (p<0,05), DMF-T before and after intervention in Puskesmas Pesanggrahan (13.8-15.0), Cilandak (11.74-14.76) and Setiabudi as Control (13.01-13.7). The results show that dental health status intervention was better than control.

Abstract

Oral dental epidemiological studies have shown the increasing of prevalence of dental caries and periodontal disease. This research was to find out base line data, among two groups of children from Urban and Rural of 20 elementary schools in sub district of Bekasi, West Java. Samples of 2205 children by the classes II, IV, V were observed 1065 in Urban and 1140 in Rural groups. The results show that prevalence of dental caries was 97,5%, DMF-T in Urban 2.225 and Rural 2.571 were significantly higher (p=0.0001) and those OHI-S in Urban 1.69^ were better than rural 2.059 (p=0.0000).
