Abstract

The validated study of extended-spectrum b-lactamase (ESBL) producing bacteria inIndonesia is scarce. Multi-centre study on susceptibility of ESBLs producers is our point of view. A survey was carried out in three teaching hospitals in Surabaya (Dr. Soetomo), Malang (Dr. Saiful Anwar) and Semarang (Dr. Kariadi). Clinical ESBL-producers were collected in over four months period (January to April 2010) up to 300 strains. The susceptibility against 6antibiotics below were used as a point of view in analysis. As many as 300 isolates were collected, 140 (Surabaya), 85 (Semarang) and 75 (Malang) respectively. The three most prevalent ESBL producers were: E. coli (42.7%), Klebsiella pneumoniae (47.3%) and Enterobacter spp (7%). The other 9 strains were: Citrobacter spp, Klebsiella oxytoca, Proteus mirabilis and Serratia spp. The susceptibility analysis was then performed on the three most prevalent isolates. The sensitivity rate of E. coli, Klebsiella pneumoniae and Enterobacter spp against tested antibiotics were 3%, 4% and 5% for cefotaxim; 91%, 87% and 90% for Amikacin; 27%, 54% and 43% for Ciprofloxacin; 98%, 93% and 100% for Cefoperason-Sulbactam; 100%, 96% and 100% for Meropenem; 95%, 94% and 86% for Fosfomycin. As a conclusion, we found that amikacin, cefoperason-sulbactam, meropenem and fosfomycin, are prospective for emperic therapy in clinical setting of health services where ESBL producing bacteria were prevalent as causative. Abstract

Latar belakang. Kasus infeksi saluran kemih oleh bakteri penghasil extended spectrum beta-lactamase (ESBL) dilaporkan sudah banyak terjadi di dunia. Informasi terkait ESBL di Indonesia sangat sedikit, terutama pada pasien anak.Tujuan. Mengkaji gambaran terapi serta luaran pasien infeksi ESBL di RSUD Dr. Soetomo Surabaya periode Januari-Juni 2014.Metode. Penelitian observasional dengan studi deskriptif, digunakan data rekam medis pasien anak RSUD Dr. Soetomo Surabaya periode Januari-Juni 2014. Variabel penelitian meliputi bakteri, uji sensitivitas, terapi serta status luaran pasien.Hasil. Pasien anak berjumlah 56, terbanyak penyebab ISK Klebsiella pneumoniae (60,7%). Uji sensitivitas tertinggi meropenem (90,5%). Pilihan terapi definitif meropenem (25%), status luaran membaik 78,57%.Kesimpulan. Meropenem merupakan pilihan terapi infeksi ESBL pada pasien dengan keadaan klinis membaik. Kontrol penggunaan antibiotik secara rasional dibutuhkan untuk pencegahan infeksi ESBL

Abstract

Pseudomonas aeruginosa is the main cause of nosocomial infection which is responsible for 10% of hospital-acquired infection. Pseudomonas aeruginosa tends to mutate and displays potential for development of antibiotic resistance. Approximately, 10% of global bacterial isolates are found as Multidrug-resistant Pseudomonas aeruginosa. Pseudomonas aeruginosa have a quite tremendous severity index, especially on pneumonia and urinary tract infections, even sepsis, which 50% mortality rate. Pineapple (Ananas comosus L. Merr) has antimicrobial properties. The active antimicrobial compounds in Ananas comosus L. Merr include saponin and bromelain. This research aims to find the potency of antimicrobial effect of pineapple (Ananas comosus L. Merr) extract towards Multidrug-resistant Pseudomonas aeruginosa. Multidrug-resistant Pseudomonas aeruginosa specimen is obtained from patient’s pus in orthopaedic department, Dr Soetomo Public Hospital, Surabaya. Multidrug-resistant Pseudomonas aeruginosa specimen is resistant to all antibiotic agents except cefoperazone-sulbactam. This research is conducted by measuring the Minimum Inhibitory Concentration (MIC) through dilution test with Mueller-Hinton broth medium. Pineapple extract (Ananas comosus L. Merr.) is dissolved in aquadest, then poured into test tube at varying concentrations (6 g/ml; 3 g/ml; 1.5 g/ml; 0.75 g/ml, 0.375 g/ml; and 0.1875 g/ml). After 24 hours’ incubation, samples are plated onto nutrient agar plate, to determine the Minimum Bactericidal Concentration (MBC). The extract of pineapple (Ananas comosus L. Merr) has antimicrobial activities against Multidrug-resistant Pseudomonas aeruginosa. Minimum Inhibitory Concentration (MIC) could not be determined, because turbidity changes were not seen. The Minimum Bactericidal Concentration (MBC) of pineapple extract (Ananas comosus L. Merr) to Multidrug-resistant Pseudomonas aeruginosa is 0.75 g/ml. Further study of in vivo is needed.

Abstract

Polymorphisms in TNF-α gene promoter region are known of its role in the production of TNF-α which may influences the pathogenesis of liver disease. SNPs in positions 238 and 308 of TNF-α gene promoters may affect the production of these cytokines. This study was aimed to detect Single Nucleotide Polymorphism (SNP) on -238 and -308 positions in the TNF-α gene promoter among liver cirrhosis patients with HBV infection in Surabaya, Indonesia. This was descriptive exploratory research with cross sectional study design using serum liver cirrhosis patients with HBV infection in Endoscopy Outpatient Clinic Dr. Soetomo General Hospital, Surabaya from April-May 2017. SNPs at -238 and -308 on TNF-α gene promoter (rs361525 and rs1800629 respectively) were detected using Polymerase Chain Reaction-Restriction Fragment Length Polymorphism (PCR-RFLP) with primers specific for the TNF-α promoter region and restriction enzymes NcoI and MspI. The genotypes of TNF-α gene promoter were assessed according to the length of the fragments produced in RFLP. Serum TNF-α levels was measured by commercial ELISA. In this study, as much as 149 positive HBsAg patients was found in Endoscopy Outpatient Clinic, Dr. Soetomo General Hospital, Surabaya. From those amount, as much as 30 liver cirrhosis patients with positive HBsAg were obtained. From 2/30 (6.7%) patients showed the GA heterozygote SNP either position -238 or -308. No patient had the AA genotype. Median blood TNF-α level in women (38 ng / L) was higher than in men (33 ng / L). TNF-α levels in patients with GA heterozygote genotype at -238 and -308 in this research was not different than wild-type (GG genotype). Among patients with liver cirrhosis due to chronic HBV infection in Surabaya, Indonesia, Surabaya, we found GA polymorphisms the TNF-α promoter gene at positions -238 and -308 in 6.7% patients, and did not find homozygous AA polymorphisms. Further studies including larger numbers of patients from various ethnic backgrounds in Indonesia are needed to provide robust data on TNF-α gene promoter polymorphisms and their role in the pathogenesis of liver cirrhosis with HBV infection in this country.
