Abstract

History has emerged as an increasingly prominent field of study in Indonesia at least since 1980s, after being neglected for several decades. New theoretical, philosophical, and empirical analyses of Indonesian history were tought widely at university and appear in professional journals and books. At the same time, a new generation of students and young scholars in a variety of fields was being made aware of the interrelationship between their fields with history. Above all however, there are growing doubts about the truth of Indonesian history at any level of society following current political changes after the resignation of Soeharto in May 1998. Indonesian history is considered primarily as a product of social and political engineering of the New Order rather than an appropriate scholarly apparatus. Consequently, Indonesian historiography is no longer appreciated and people are simply asking for a new history, a deconstructed history. The most vocal and trenchant criticism of existing historiography, however, has come not from within historian community. This is an irony. The need to deconstruct contemporary Indonesian history is not an important issue in academic or professional historian community. The polemic took place mostly in daily newspapers, popular weekly magazines, or tabloids rather than in highly recognised historian forum. Most prominent historians at the university obviously had chosen a different path from the one which was being led by recent popular viewpoint. Instead, politicians, journalists, and other social-humanities scholars took over the place of historians in discussing the truth of the past.

Abstract

So far, the development of southern Sumatra has been explained by its integration into the world market. This view ignores the local ecological conditions, that offer opportunities of the world market. This article therefore gives an overview of the local ecological situation and the changing ways the people exploited their environment.

Abstract

Ketika sastra dan sejarah dibicarakan secara bersama-sama, segera muncul pertanyaan, apakah ada fiksi di dalam sejarah dan apakah ada fakta di dalam sastra? Bagi sebagian orang, pertanyaan itu mungkin kedengaran agak aneh dan seolah-olah hanya dibuat-buat karena secara umum sastra selalu dikaitkan dengan fiksi, sedangkan sejarah tidak dapat dipisahkan dari fakta masa lalu. Hal itu berarti bahwa pernyataan di atas tidak memerlukan jawaban. Akan tetapi, persoalan menjadi lain ketika perbincangan tentang sastra dan sejarah memasuki dunia wacana dekonstruktif di samping rekonstruktif. Sebagai sebuah realitas, sejarah dan sastra sering dianggap berada dalam tataran yang sama. Fiksi dan sejarah tidak dapat begitu saja secara kaku diasosiasikan hanya dengan salah satu di antara keduanya, yaitu hanya berkaitan dengan sastra atau hanya dengan sejarah.

Abstract

AbstrakRinitis alergi merupakan gangguan fungsi hidung, terjadi setelah pajanan alergen melalui inflamasi mukosa hidung yang diperantarai IgE. Walaupun rinitis alergi bukan penyakit yang berat, penyakit ini dapat menurunkan kualitas hidup penderita. Allergic Rhinitis and Its Impact on Asthma (ARIA-WHO) menganjurkan tes kulit tusuk (TKT) sebagai pemeriksaan penunjang diagnostik, tetapi tidak semua sarana kesehatan memiliki fasilitas tersebut, oleh karena itu perlu dipilih pemeriksaan alternatif yang sesuai atau setara yaitu eosinofil kerokan mukosa hidung. Suatu uji diagnostik dilakukan pada 50 subjek tersangka rinitis alergi periode Februari-April 2005 di poliklinik alergiBagian THT-KL FKUP–RSHS. Sebanyak 78% penderita menunjukkan TKT positif, 76% terdapat eosinofil pada mukosa hidung, 64% positif terhadap kedua pemeriksaan. Uji statistik menunjukkan hubungan bermakna antara TKT positif dan eosinofil positif (X2= 3,559; p = 0,03), sensitivitas eosinofil kerokan mukosa hidung 82,1% (95%CI: 82,02-82,18), spesifisitas 64,3% (95% CI: 63,52-65,08), dan akurasi 74% (95% CI: 73,94–74,06). Penelitian ini menunjukkan pemeriksaan eosinofil kerokan mukosa hidung dapat digunakan sebagai pengganti tes kulit tusuk pada sarana kesehatan yang tidak mempunyai fasilitas tersebut. [MKB. 2010;42(1):6-11].Nasal Scrapping Eosinophil As a Diagnostic Test for Allergic RhinitisAllergic rhinitis is clinically defined as a symptomatic disorder of the nose induced by IgE mediated inflammation. Allergic Rhinitis and Its Impact on Asthma (ARIA-WHO) recommended the use of skin prick test (SPT) as a diagnostic test, but the facility to acquire this method is not always available at some of the health centres, therefore alternative diagnostic test should be performed such as nasal smear eosinophil count. The aim of this study was to assess the usefulness of nasal smear eosinophilia compared with SPT for the diagnosis of allergic rhinitis. A diagnostic study using the cross sectional design, conducted in Department of ORL-HNS at FKUP-RSHS (February-April 2005). Fifty patients with a clinical history suggestive of nasal allergy were studied. A positive SPTwas demonstrated in 78% of the study population, 76% of the patients demonstrated positive eosinophil count. There was 64% correlation between positive SPT and nasal eosinophil and it was statistically significant (X2= 3.559;p=0.03). The sensitivity of nasal smear eosinophilia count was 82.1% (95% CI 82.02-82.18), specificity 64.3% (95%CI 63.52-65.08), and accuracy 74% (95% CI 73.94-74.06). The eosinophil count of nasal scrapping can be used as the replacement of the SPT at the health centre which do not have the SPT facility.

Abstract

Pilihan utama pengobatan karsinoma nasofaring adalah radioterapi. Pemberian radioterapi dosis tinggi menimbulkan kerusakan struktur jaringan, salah satunya adalah gangguan pendengaran. Gangguan di koklea timbul akibat kerusakan struktur sel rambut akibat degenerasi stria vaskularis, atrofi spiral ligamen, dan membran basilaris.Tujuan penelitian adalah untuk mengetahui fungsi sel rambut luar koklea akibat radioterapi penderita karsinoma nasofaring. Tipe penelitian adalah studi analitik observasional dan dilakukan di Bagian IK.THT-KL/RS. Hasan Sadikin Bandung mulai Maret sampai September 2007. Dilakukan pemeriksaan audiometri, timpanometri, danemisi otoakustik (otoacoustic emission)/OAE sebelum radioterapi, serta pemeriksaan timpanometri serta OAE saat radioterapi dan satu bulan pascaradioterapi. Untuk menguji pengaruh radioterapi eksternal digunakan uji McNemar dan uji Z. Diperoleh 42 telinga dari 27 subjek penelitian yang memenuhi kriteria inklusi. Didapat 18 lakilakidan 9 perempuan. Hasil penelitian menunjukkan insidens gangguan fungsi sel rambut luar koklea sebesar 9,6% pada dosis 2.000 cGy, 61,5% pada 4.000 cGy, 81,1% pada 6.600 cGy, dan 82,8% satu bulan pascaradiasi. Hubungan gangguan fungsi sel rambut luar koklea memberikan nilai sangat bermakna pada dosis 4.000 cGy sampai 6.600 cGy (p<0,001). Kesimpulan penelitian radioterapi eksternal penderita karsinoma nasofaring menyebabkan gangguan fungsi sel rambut luar koklea.

Abstract

Otitis media merupakan peradangan lapisan mukoperiosteum di telinga tengah tanpa melihat penyebab atau patogenesisnya. Angka kejadiannya bervariasi, di Bandung dan sekitarnya mencapai 6,9%. Penyebab otitis media karena terganggunya fungsi tuba eustakius dapat ditimbulkan oleh pemakaian pipa nasogastrik (PNG). Dilakukan penelitian analisis observasional untuk melihat pengaruh PNG pada kejadian otitis media efusi pada penderita rawat inap di Departemen Ilmu Kesehatan Telinga, Hidung, Tenggorok-Bedah Kepala dan Leher Rumah Sakit Hasan Sadikin. Sebanyak 34 orang subjek didapatkan dalam periode Januari 2007 yang memenuhi kriteria inklusi diikutsertakan dalam penelitian. Pada 34 subjek dilakukan pemeriksaan fisik THT dan timpanometri sebelum dan selama pemasangan PNG. Pemeriksaan timpanometri diulang tiap 24 jam sampai PNG dilepas. Timpanogram dibaca dan data yang diperoleh dianalisis menggunakan uji Friedman untuk membandingkan nilai tengah tekanan dan compliance antara sebelum dan sesudah pemasangan PNG. Uji Spearman correlation untuk mengetahui hubungan tekanan dengan compliance di telinga tengah. Besarnya kejadian ganguan otitis media efusi diuji dengan uji binomial. Terjadi penurunan puncak tekanan dan compliance di telinga tengah sampai hari ke-3 (p=0,197), sedangkan pada hari ke-4–ke-7 terjadi peningkatan kembali. Simpulan, penggunaan PNG tidak berpengaruh pada tingkat otitis media efusi, tetapi pada penurunan puncak tekanan (daPa) dan compliance (mmho) telinga tengah. [MKB. 2011;43(1):42–8].Kata kunci: Compliance, otitis media, pipa nasogastrik, tekanan, telinga tengah, tuba eustakiusThe Effect of Using Nasogastric Tube on Incidence of Otitis Media withEffusion Otitis media is an inflammation of the middle ear mucoperiosteal without reference to its cause or pathogenesis. The incidence rate in Bandung area was 6.9%. Otitis media caused by Eustachian tube dysfunction might be induced by the use of nasogastric tube (NGT). An observational analytic was conducted to know the effect of NGT on incidence rate of otitis media with effusion in hospitalized patients at Department of Otolaryngology-Head and Neck Surgery, Dr. Hasan Sadikin Hospital. There were 34 subjects in January 2007 were included in this study. Before NGT insertion all subjects had physical examination and tympanometry. Every 24 hours tympanometry was performed till exertion the NGT. Tympanogram was collected and analysed statistically by Friedman test to compared median value of pressure before and after NGT insertion, and that of compliance. Spearman correlation test to identify correlation between peak pressure and compliance in the middle ear, and binomial to test hypotesis. There was decreasing pressure and compliance in middle ear until day 3 (p=0.197) and increased on day -4 and -7. In conclusion, the incidence rate of otitis media with effusion is not affected by using of NGT. The using of NGT is associated with reduced peak middle ear pressure (daPa) and peak compliance (mmho). Abstract

The purpose of this writing is to express that friendship is more important than hostility. The reason of hostility is annoyance, emotion and lack of patience in facing problems. The effect of it is the destruction of friendship and the increase in annoyance among them. Feminist is a method that is used in this writing. The application of this method is to express that man and woman have the right to make their own decisions. Both of them will respect and understand. They will have responsibility for everything they have done. This method is also expressing that human have freedom in their life. In this novel, there are four women. They are Carmen, Bridget, Lena and Tibby. The four of them spend their summer holiday into different places. In their summer holiday, they have experienced incidents which can destroy friendship. Carmen hates her father, Lena hates Kostos, Bridget hates Eric and Tibby hates Bailey. Finally, they realize that annoyance between them can destroy their friendship. The writer used Feminist approach because the main characters wanted to destroy the patriarchy of man. They insist that man can respect woman’s opinion.   Key words: Friendship, Conflict, Respect

Abstract

Background:Low density lipoprotein (LDL) cholesterol is a prognostic factor for hemodialysis mortality through atherosclerosis mechanism. Vitamin C and E have been well known as anti-oxidants that reduced LDL cholesterol. However, there were no studies have been done to address the effectiveness of vitamin C, E, and its combination on lowering LDL in patients with hemodialysis. This study aimed to determine the effect of vitamin C, E, and C and E combination on LDL cholesterol. Methods:A randomized controlled trial was conducted in 64 patients receiving hemodialysis at RSUD Dr.Moewardi(RSDM) Surakarta. The sample was taken at random and then divided into 4 groups: placebo, vitamin C (500 mg), vitamin E (400 mg), and vitamin C + E combination. The treatment lasted for 30 days. LDL cholesterol before and after treatment were analyzed at the Clinical Pathology division, RSDM. The reduction in LDL cholesterol before and after treatment was analyzed using One Way Anovaand (pair-wise) Post-Hoc Test. Results:One Way Anovashowed statistically significant reduction in LDL cholesterol between groups (p&lt;0.001). Post hoc testshowed that vitamin C, vitamin E, or its combination reduced LDL cholesterol more than the placebo (p&lt;0.001). Vitamin C and E administered in combination led to statistically significant lower LDL cholesterol than administered alone (p=0.002). There was no statistically difference in LDL cholesterol level between those provided with vitamin C and vitamin E. Conclusion: Vitamin C and E, individually or in combination, reduces LDL cholesterol. The highest reduction was achieved by vitamin C and E combination.Keywords: vitamin C, vitamin E, LDL cholesterol

Abstract

Nationalism is a given cultural reality which keeps changing its meaning through out our history. Such changing is partly due to a manipulative exercise by the rulers, partly through socialization of the so called national history. Societal conflict and tention emerged in many part of Indonesia recently has been due to unfinished transformations, yet pursuing the manipulative path of nationalization of nationalism lead us to absurdity of our collective identity. Reconstructing Indonesian historiography is an imperative, whithin which local history should have a fair treatment.
