Abstract

The Indonesian Code of Medical Ethics or Kode Etik Kedokteran Indonesia (KODEKI) is a reference of moral values for the Indonesian physicians applied in their day-to-day practices. The aims of the present study were to obtain reflection response rate by the physicians on values included in the KODEKI along with its affecting factors. This is a cross sectional study among 400 clinical doctors at Jakarta Special Province. To measure reflection levels of KODEKI, we used KODEKI Reflection Questionnaire (KRK). Personal data sheet was used to obtain characteristic respondent and influencing factors reflection level of KODEKI. The prevalence of KODEKI reflection response rate in most subjects was moderate, i.e. 338 (84.5%). There was a significant correlation between the duration of physician mission time (p=0.01) and the main location of medical practice (p=0.02) against the KODEKI reflection response rate based on statistic tests. There was no significant correlation of factors against the KODEKI reflection response rate by the duration of time spent in consultation (p=0.10), doctor’s competence level (p=0.11), and the experience of having training/knowledge about medical ethics (p=0.71). The duration of physician mission time and the main location of medical practice were affecting factors to reflection level. Both factors had association with frequency of experience and exercise facing ethical dilemma.Keyword: reflection, The Indonesian Code of Medical Ethics, KODEKI Reflection Questionnaire

Abstract

Basic principle of bioethics is one of medical ethics teaching material with cognitive approach in medical faculty. It is a relevant method to develop student’s reasoning analysis of medical ethics. The purpose of this study was to evaluate item analysis, reliability and validity of Basic Principles of Bioethics/Kaidah Dasar Bioetika Test (KDB Test) designed to measure level of cognitive of principle based of bioethics among medical student. This an evaluative descriptive study. The participant comprised of 99 medical students of Faculty of Medicine University of Riau academic year 2007. KDB Test was developed based on literature review and consultation with ethical expert. Item analysis of instrument was determined discrimination index and difficulty index. The validity of instrument was determined by measuring the face validity and content validity. Internal consistency reliability was examined using Kuder Richardson-20 (KR 20). The final version KDB Test showed r (coeffisien correlation to total) ranged between from 0.338 to 0.807 with level significant p<0.05 and internal consistency reliability (KR 20 = 0.701). The conclusion, KDB Test was reliable and valid instrument to measure level of cognitive among medical student.Keywords: Basic principle of bioethics, KDB Test, level of cognitive

Abstract

Aim The place of ethics in undergraduate medical curricula is essential but the methods of teaching medical ethics did not show substantial changes. “Basic principles of bioethics” is the best knowledge to develop student’s reasoning analysis in medical ethics In this study, we investigate the effects of an additional small group discussion in basic principles of bioethics conventional lecture methods to cognitive achievement and retention. This study was a randomized controlled trial with parallel design. Cognitive scores of the basic principles of bioethics as a parameter was measured using basic principles of bioethics (Kaidah Dasar Bioetika, KDB) test. Both groups were attending conventional lectures, then the intervention group got an additional small group discussion.Result Conventional lectures with or without small group discussion significantly increased cognitive achievement of basic principles of bioethics (P= 0.001 and P= 0.000, respectively), and there were significant differences in cognitive achievement and retention between the 2 groups (P= 0.000 and P= 0.000, respectively).Conclusion Additional small group discussion method improved cognitive achievement and retention of basic principles of bioethics. Abstract

BACKGROUND: Conventional autopsy in Indonesia is not well accepted as it is contrary to religion and culture. New radiological imaging method such as multislice computed tomography (MSCT) scan has potential to be a diagnostic tool in forensic pathology. The purpose of this study is to determine the ability of MSCT scan in finding abnormalities in close blunt head injury compared with autopsy.METHODS: This study used descriptive qualitative method. Postmortem cases in Department of Forensic Medicine and Radiology of Dr. Cipto Mangunkusumo Hospital were selected based on inclusion criteria. Then MSCT scan and autopsy were conducted. MSCT scan and autopsy results were compared and analyzed.RESULTS: There were 491 postmortem cases of blunt head injury. However, only 10 cases fulfilled inclusion criteria. Subarachnoid haemorrhages were identified 100% with MSCT scan and 80% with autopsy. Cerebral oedemas were identified 100% either with MSCT scan and autopsy. Subdural haemorrhages were identified 100% with MSCT scan, while 50% with autopsy. Multiple fractures were identified 80% with MSCT scan, while 40% with auto.CONCLUSION: MSCT scan showed a sensitive detection in finding abnormalities in close blunt head injury. Therefore it could be as an alternative choice of examination in close blunt head injury cases.KEYWORDS: multislice computed tomography scan, postmortem, blunt head injury, autopsy

Abstract

Imunisasi secara medis merupakan tindakan yang aman, namun sesekali terancam olehefek samping atau efek buruk yang disebut KIPI. Dalam bentuk program, imunisasi massalakan memunculkan kekerapan KIPI yang dapat merugikan jasmani dan bahkan nyawapasien yang semula sebagai klien petugas kesehatan. Hukum, khususnya hukum administrasinegara yang dilandasi oleh etika sosial dan manajemen yang lege artis mengharuskan pejabatkesehatan hingga ke tenaga pelaksana kesehatan melaksanakan program tersebut sehinggatujuan imunisasi tercapai, tanpa menimbulkan gugatan hukum yang tidak perlu dari pasienyang dirugikan akibat KIPI. Efektivitas dan efisiensi program akan seimbang denganyuridisitas dan legalitasnya. Bila gugatan hukum muncul, kerangka hukum penyelesaiandan perlindungannya terdapat dalam lingkup hukum administrasi negara, yang berbedadengan hukum kesehatan perorangan. Petugas kesehatan dilindungi oleh standar proseduroperasional, pemberian informed-consent kolektif, pembuatan surat-tugas dan bahkantindakan diskresioner sesuai dengan kondisi dan situasi lapangan demi kepentingan kliendan pasien. Persyaratan diskresi tersebut secara hukum diuraikan, termasuk rantaitanggungjawabnya hingga ke pejabat tertinggi dalam bidang kesehatan. Pokja KIPI yangberfungsi sebagai lembaga independen dan penasehat pemerintah, dapat berfungsi sebagailembaga yang memverifikasi fakta hukum, penyelesaian kasus sengketa medik pada KIPI,serta usulan pemberian santuan kepada korban bila diperlukan, sebelum kasus tersebutmasuk ke lembaga peradilan resmi yang seringkali justru sulit menciptakan keadilan.
