Abstract

Background: According to state role on section 81 undang-undang No 13/2003 mentioned that woman workers who are suffering dismenorrhea during menstruations period could take leave on first and second day menstruation. However not all woman workers in PT.Nyonya Meneer Semarang utilize this policy. One of the reason is using hormonal contraception make their period unstable and irregular. The aim of this study was to analyze about relationships between hormonal contraception usage, knowledge and attitude about menstruation leave and the practice of menstruation leave.Method: This research was an explanatory, with a cross sectional approach. The sample recruited from woman workers of PT. Nyonya Meneer Semarang who have age less than 50 years, not menopause, and using hormonal contraception. 81 woman workers were involved in this study selected by simple random sampling. Chi-square test was employed to test significances between variables.Result: The study shows that there was a relationship between hormonal contraception usages with the practice. However, knowledge and attitude of woman workers have no relation with menstrual leave practice. This study suggest that the company should socialize about determination on menstruation leave and the effect to the woman workers in order to makethe woman workers utilize this rule.Keywords: hormonal contraception, rules of menstruation leave, women worker

Abstract

Background: The efforts to reach the national target to decreasing infant mortality, what in this time still become governmental priority programs. Nationally, mortality rate in this last decade had decreased excessively, from 50 per a thousand alive births in 2001 becoming 39 per a thousand alive births in 2005. The number revealed if seen at mortality neonatal still high enough.Method: This is an analytical survey research with case-control approach. Research subjects consists of 29 women having infant which die at neonatal age as a case group, and 29 women having alive infant passed a neonatal period, as a control group. Source of data obtained from secondary data to verbal autopsy record, history of pregnant woman and confinement note from public health center (Puskesmas). Data analyzing by univariate and bivariate analysis. Statistical test by Chi Square test that significance rate determined if p -value less than 0.05 and calculation of Odds Ratio (OR>1).Result: Result of analysis indicates that mother age (p = 0.0023 and OR = 7.69), parity (p =0.0006 and OR = 8.25), helper of confinement (p = 0.044 and OR = 6.07), born baby weight (p = 0.016 and OR=6,12) and condition of baby breathing effort (p= 0.001 and OR = 7.85), significantly risked to neonatal status. It is conclude that mother age, parity, helper ofconfinement, born baby weight and condition of baby breathing effort is risked to neonatal status. It is suggested to maturing marriage age and management of birth at healthy reproduction age, increasing of neonatal call so that can decreasing mortality rate.Keywords: neonatal mortality status, risk factors, pregnant woman

Abstract

Background: Maternal and Child Care is one of factors to reduce Maternal Mortality Rate. This service needs sufficient supports, i.e. antenatal care guideline, health personnel, and its facilities. A pregnant mother is required to have a minimum of 4 times of Antenatal Care within her pregnancy, i.e. 3rd month, 6th month, 7th and 9th month. Compared to other health centers, Tegal Barat II Health Center recorded lower rate on Antenatal Visitors Rate or about 60%. Meanwhile, the national standard requires 90% of Antenatal Visitors Rate. This research aimed to explain the association between maternal perception on maternal and child carefacilities and antenatal care (K4).Method: This research was an explanatory research, utilizing cross sectional approach. Forty five respondents were taken from mothers who have babies less than 3 months old and visited the health centers. Questionnaires were utilized to gather the data, and the analysis wasconducted based on descriptive as well as analytic.Result: Result showed that out of 45 respondents: 53.3% had perception that antenatal guideline book was not interesting, 17.8% said that health personnel had to examine fundus height metrically, and 93.3% said that all pregnant women had to obtain Fe tablets from health center. Variables that significantly associated with antenatal care (K4) were maternal perception on antenatal care guideline (at p-value: 0.012) and maternal perception on antenatal care facilities (at p-value: 0.0001). Meanwhile, maternal perception on health personnel (at p-value: 0.161) was not associated with antenatal care. It was concluded and recommendedthat the performance of Antenatal Care Guideline and Antenatal Care Facilities must be improved.Keywords: Antenatal care, service quality.

Abstract

Background : The goal of maternal and antenatal care is to strengthen to improve access and to omprove maternal and child health status effectively and efficiency. One of indicators to evaluate the program at health center level is the coverage of pregnant mothers who seek theservice (K4). The antenatal care service is provided free of charge for underprevilege families at community health centers, village midwives and village health posts under social security community health service. However, the coverage of pregnant mothers who seek the service(K4) for underprevilege families at Sulang is still under target as recorded on 2005. The coverage of this community health center was still reach 39,6 %. This condition could be assumed that the utility of antenatal care servese is not at optimum level. The preference of antenatal careservice is a form of behavior in health. This research aims to know the association between demographic factors, maternal knowledge and antenatal care service preference amongst underprevilege families at Sulang Community Health Center, Rembang Regency.Method : This study utilized explanatory survey. The respondents are 35 mothers from underprevilege famil ies who prefer to use the designated antenatal care service base on recommendation by local government and 40 mothers from underprevilege families who prefer to use the others on 2005. Proportionate random sampling was utilized based on randomnumber table. Data was analyzed using Chi-square test.Result : Results showed there was a significant association between age and antenatal care service preference ( p-value= 0,001), a significant association between educational attainment and antenatal care service preference ( p-value= 0,007), a significant association between parity and antenatal care service preference ( p-value= 0,003). However range of birth is not association with antenatal care service preference ( p-value= 0,348). It is recommended to give communication, information and education amongst pregnant mothers at the age of <20 years old and >35 years old. Those include the risk of pregnancy and giving birth withinthe high risk age, antenatal care service procedure, place and requirement to obtain free facility for antenatal care service amongst underprevilege family. The programme can be delivered through integrated health service posts (posyandu) and on household meetings.Keywords : Antenatal care, Underprevileged family, Health assurance

Abstract

Latar Belakang: Keberhasilan program KB dapat ditempuh dengan strategi perencanaan pola kontrasepsi yang rasional diperlukan kompetensi petugas terhadap pelaksanaan konseling yang merupakan inti dari program KB. Sebagian besar dalam pelaksanaaan konseling kontrasepsi tidak menjelaskan konseling KB sesuai Standar Operating Prosedur (SOP) yaitu dengan langkah SATUTUJU. Tujuan penelitian ini adalah menganalisis hubungan karakteristik program dengan pelaksanaan konseling kontrasepsi oleh bidan di wilayah Dinas Kesehatan Kota Surakarta. Metode penelitian survey analitik dengan pendekatan cross sectional. Sampel penelitian ini adalah bidan yang memberikan pelayanan konseling kontrasepsi sebanyak 117 diwilayah Dinas Kesehatan Kota Surakarta. Pengumpulan data dilakukan menggunakan angket. Analisa dilakukan secara univariat, bivariat dengan uji Pearson Correlation dan multivariat dengan Regresi Linier. Hasil penelitian dapat disimpulkan secara univariat masih ditemukan bidan yang belum memahami klien, memperhatiakan kebutuhan klien , persiapan media bila diperlukan saja sekitar (15,4%) menjelaskan panjang lebar dengan istilah medis, tidak memperhatikan sikap klien, penggunaan ABPK sebesar (17,9) , mengatakan tidak ada waktu untuk istirahat, tidak ada waktu pendokumentasian sebesar (11,1%), pekerjaan yang banyak kadang membuat mereka menghindar dari pekerjaan (16,2%) dan tidak mengidentifikasi pedoman KB, evaluasi pelaksanaan dan evaluasi SOP sebesar (17,1%).Simpulan Secara bivariat variabel yang berhubungan dengan pelaksanaan kompetensi bidan (nilai p=0,044,r= -0,187), motivasi kerja bidan (nilai p=0,0001, r=0,347), supervisi oleh pimpinan (nilai p=0,006, r=0,250), dan secara multivariat yang memiliki pengaruh paling besar adalah motivasi dengan (nilai p = 0,0001). Kata Kunci: Konseling Kontrasepsi, Program KB, Karakteristik Program, Pelayanan Kontrasepsi.

Abstract

IUD is one of the long term contraceptions which its usage is still below the target of Kebumen goverment on 11%. The local government has done a workshop of Contraception Technology Update (CTU) for the midwives in the Kebumen regency so the numbers of the IUD acceptors had increased to 6.57% in June 2012 but however, this is still below the government's target. Some efforts have been done by the local governement but it still can not show a good of IUD contraveption good services. Objective of the research is to give clear descrpition of the analysis of factors  relate to intra Uterine Device (IUD) contraception services by midwives in  Kebumen Regency.This study   this is an observational research with analytic survey method and cross sectional approach. The data were collected using interview consited of strutured questioners to 80 midwives in Kebumen area. The data were analyzed using univariat, bivariat and multivariat analyses techniques.The results show that the variables of knowledge, motivation, acces, human resource availability and policy are in good category but there are still 40% of midwives who have low motivation so they still prefer to give the other more practical contraceptions instead of IUD, and 31% of the places for the IUD services still have not been provided with flipcharts. The variable of IUD contraception has correlation with the variables of knowledge with (p=0.037), motivation (p=0.045), and the availability of the human resources (p=0.021). Meanwhile the variables of service acces (p=0.691) and policy (p=1.000) have no correlation with the behaviour of IUD contraception service. Togetherly the variables of motivation and human resource availability have influenced the behaviour of IUD contraception service p≤ 0.05 with Exp.(B) > 2.It is recomended to midwives to motivate them selves in giving more rasionally Family Planning service and it is necessary to hold an Up to date Contraception Technology Workshops for midwives to increase the quality of IUD contraception services. Abstract

Banjarkerta, a village in district Karanganyar, Purbalingga, is a central village of mattress craftsman with textile waste materials as a substitute for cotton. One of negative impacts in consequence of work for the craftsmen is lung function disorder, which is caused by dust of textile waste. The other risk factors are age, work period, Body Mass Index, use of masks, smoking habits, and history of pulmonary disease.The purpose of this study was to find out the risk factors for lung function disorder in craftsmen of mattress in Banjarkerta, Purbalingga, year 2011.This research was conducted with cross sectional approach, observing 85 respondents, using random sampling techniques. The data collected was obtained by measurements (dust variables, BMI, lung capacity) and interviews (age variables, gender, work period, smoking behavior, and history of pulmonary disease). The collected data is then presented in tabular and narrative. The data analysis is done by analyzing tables and cross-table analysis, using the calculation of Ratio Prevalence  (RP).The results of this study is that there is lung function disorder in mattress craftsman, with the normal criteria (23.5%), the only restriction or obstruction (15.3%), the combined restriction and obstruction (61.2%). Factors associated with lung function disorder are age (RP = 1,220); gender (RP = 3.137); working hour (RP = 1,333); the custom of wearing masks (RP = 1.319) and a history of pulmonary disease (RP = 1.502). Factors not associated with risk of lung function disorder are work period (RP = 0.611);, BMI (RP = 0.807 and smoking habits; (RP = 0.583). Indoor levels of airborne dust in the home industry (26,656 and 80,000 ìg/Nm3 ìg/Nm3), already far exceeds the Threshold Limit Value (TLV) which allowed for the air space of industry, that is 10,000 ìg/Nm3.  Researchers gave suggestions for the craftsmen to wear a mask that is good and right, and to get occupational health monitoring by the Health Center / Department of Health.

Abstract

Perinatal service is one of eminent services in Kraton Hospital. Limitation of trained staff andless completeness of equipment as well as less leadership supports for “infant caringhospital” has been problems in the perinatal service in Kraton Hospital.ResearchObjective was to analyze influence of leadership practice perception to service qualityperception in perinatal room Kraton Hospital. Type of research was observationalquantitative analytic, with cross sectional approach. Data collection used questioner andobservation. Sample was total population. Those were all medical staff and paramedical staffin perinatal room Kraton Hospital, totally 36 respondents. Data analysis used Pearsonproduct moment correlation and Rank Spearman followed by logistic regression. Resultsshowed that perception of challenging process of leadership practice had category good(58.3%); inspiring of shared vision had category moderate (58.3%); empowering others hadcategory good (63.9%); become a model had category moderate (52.8%); driving spirit hadcategory good (50%); and perception of quality of services had category good (55.6%).There was a correlation between challenging process of leadership practices (ρ=0.832;p=0.0001); inspiring shared vision ( =0.78, p=0.0001); driving spirits ( =0.615,p=0.0001) and become a model ( =0.445, p=0.007) and perception of quality of perinatalservices. There was no correlation between perception of empowering others of leadershipand (ρ=0.252, p=0.139). Logistic Regression analysis showed that perception of inspiringshared vision of leadership had the greatest influence to perception of perinatal qualityservices (exp β = 84.5), followed by driving spirit of leadership (exp β = 22,8) and become amodel (exp β = 6,4).

Abstract

WHO estimates that approximately 40 % of maternal death in developing countries areassociated with anemia in pregnancy. Anemia in pregnancy is often called a "potential danger tomother and child", there for requires serious attention of all relevant parties, especially in healthcare. Detection of cases of anemia in pregnancy has not been conducted in accordance with thestandard management of anemia in pregnancy. There for it is necessary to analyze the factorsthat affect the performance of midwives in the management of anemia in pregnancy inSingkawang. The general objective of the study to determine the factors that affect theperformance of midwives in the management of anemia in pregnancy in Singkawang. Typeanalytic approach Retrospective survey research. The study population is the entire area of thehealth center midwife Singkawang totaling 110 people. The sample as many as 52 people from 5 regions Singkawang Health Center. The independent variables of knowledge, infrastructure,leadership , motivation and communication. The dependent variable performance of midwives inthe management of anemia in pregnancy. Data were collected by interviews using a structuredquestionnaire and observation. Data analyisis using the Spearman rank and logistic regression.The results of the study the majority of midwives aged between 20-35 years (63.5 %), educationDIII/DIV/S1 Midwifery (90.4 %), service life ≥ 10 years (55.8 %). Performance categorizedeither midwives (59.6 %), good knowledge (51.9 %), the complete infrastructure (53.8 %), goodleadership (63.5 %), good motivation (55.8 %), good communication (53.8 %). Allindependentvariables (knowledge, infrastructure, leadership, motivation and communication) has a value ofρ< 0,05, which means there is a connection with the performance of midwives in themanagement of anemia in pregnancy. Results of multivariate analysis , the variables that affectthe performance of midwives in the management of anemia in pregnancy is knowledge (ρ=0.002)and leadership (ρ=0.006).

Abstract

Antenatal care provided health service for pregnant women and their fetuses according to theminimum standard of 7T, and minimum of four examinations during pregnancy. These standardswere intended to assure protection to pregnant women, such as risk factors early detection,complication prevention and management. Based on the coverage of maternal and children localarea monitoring (PWS-KIA), maternal and infant mortality rates, and preliminary survey tovillage midwives, it was concluded that work performance of village midwives was stillinadequate. Objective of this study was to identify factors affecting work performance of villagemidwives in conducting early detection of high risk pregnancy in the antenatal care. This was anexplanatory research with cross sectional approach. Data were collected using structuredquestionnaire and observation. Study population was village midwives. Samples consisted of 93respondents who were selected using simple random sampling technique. Results of the studyshowed that 73.1% of village midwives’ age were in the range of 25-35 years old, and 37.6% ofvillage midwifes duration of work were in the range of 36-60 months. Bivariate analysis withcross tabulation and Pearson Product Moment test indicated that variables related to workperformance were knowledge (p= 0.000), motivation (p= 0.004), perception on supervision ofcoordinator midwives (p= 0.016), and perception on workload (p= 0.047). Multivariate analysisusing multivariate logistic regression test indicated common effect of knowledge and facilitiesvariables with significance value of 0.000 and r 2 = 0.394; it meant that 39.4% of workperformance variable could be explained by variation of those two variables. District healthoffice is expected to provide training, to complete facilities for village midwives before startingto work in the village; to form team or committee as a consultation place. Puskesmas is expectedto complete documentation facilities; head of puskesmas and coordinator midwives are advisedto actively doing supervision.
