Treatment of Hypertension in Patient with Nephrotic Syndrome

Abstract

Up to this moment, hypertension is an issue that hard to be dealt with. According to WHO 1996, hypertension have taken its part as contributing factor on morbidity and mortality mainly in modern countries. The prevalence of hypertension in Indonesia is about 6 to 15%, and the estimated number of 1 billion people across the globe are having this problem, also there are 7,1 millions hypertension related death. One of the causes of hypertension is kidney disease. Chronic kidney disease can cause hypertension and also hypertension can cause chronic kidney disorder. Nephrotic Syndrome is the common chronic kidney disease. Management of hypertension in patients with nephrotic syndrome has to consider long term effect of medication, renal condition, and other comorbid condition.Keywords: Hypertension, Chronic Kidney Disease, Nephrotic Syndrome, Treatment of choices.

Epiphyseal Preservation Surgery in Distal Femur Osteosarcoma

Abstract

Conventional osteosarcoma is a primary intramedullar high grade malignant tumour in which the neoplastic cells produce osteoid Early diagnostic and accurate preoperative assessment of the extent of the tumor is crucial when a limb salvage procedure is being considered. More recent studies described the accuracy of magnetic resonance imaging for evaluation epiphyseal extension of osteosarcoma. With development of modern chemotherapy regimens, limb salvage procedure was developed. Most of surgical technique presently used to treat high grade metaphyseal osteosarcoma include resection of one or both epiphysis. If an osteoarticular allograft is used to reconstruct the joint, the epiphysis of the affected bone is lost, potentially creating anatomical mismatching or joint instability. Preservation of the epiphysis technique was potentially to maintain a nearly normal joint for several decades. The aim of this case is to report a case of a female, 12 years old, with osteosarcoma of distal femur to whom a Limb Salvage Surgery with Epiphseal Preservation technique was performed. After two years of follow up, no evidence of any metastasis was detected but the graft was fracture due to excessive load. Preservation of the epiphysis in distal femur osteosarcoma is an alternative surgery for tumor that does not extend to epiphysis. Extracorporeally irradiated autograft is a methode that can be used for reconstruction of distal femur osteosarcoma.Keywords: osteosarcoma, epiphyseal preservation, extracorporeally irradiated autograft

Abstract

UJI PRODUKSI Mo-99 HASIL FISI DENGAN BAHAN SASARAN FOIL LEU BUATAN P2TBDU-BATAN. Selama ini telah dilaksanakan beberapa kali produksi Mo-99 dengan menggunakan bahan sasaran foil LEU buatan Argonne National Laboratory (ANL). Dalam produksi Mo-99 dengan proses Cintichem yang dimodifikasibahan sasaran foil LEU sangat menentukan. Telah dilakukan uji produksi Mo-99 hasil fisi dengan bahan sasaran foil LEU (LEU, < 20% U-235) buatan P2TBDU-BATAN dengan proses Cintichem yang dimodifikasi, meliputi pelarutan foil LEU menggunakan HNO3 9,5N, penarikan gas iodium, proses pemisahan dengan pengendapan α-Benzoin Oxime dan pemurnian Mo-99 melalui kolom kromatografi. Foil LEU larut dengan sempurna selama 30 menit dan penarikan gas iodium dilakukan dengan pendinginan nitrogen cair. Setelah melalui tahap pemisahan (pengendapan) dan pemurnian kolom I dan kolom II radionuklida pengotor masih besar dan aktivitas pengotor pemancar α melebihi persyaratan standard internasional yang ditetapkan yaitu sebesar 7.716E-7 µCi/mCi Mo-99. Kata kunci: produksi Mo-99 HF, Sasaran Foil LEU P2TBDU PRODUCTION TEST OF Mo-99 FISSION PRODUCT USING LEU TARGET FOIL MADE IN P2TBDU – BATAN. Production of Mo-99 fission product using LEU target foil of Argonne National Laboratory (ANL) has been performed several times recently in BATAN. In the production of Mo-99 using modified Cinthichem method, the LEU target foil was very critical. This paper reported production test on Mo-99 fission product using LEU target foil ( LEU < 20% U-235) made by P2TBDU – BATAN using modified Cintichem method. The process included dissolving of LEU foil with 9.5 N HNO3, suctioning of iodine gas, separation process using precipitation with α–benzoin oxime, and purification by column chromatography. The LEU foil was completely dissolved after 30 minutes and gas suction was performed by cooling with liquid nitrogen. After separation and two purification steps, the radionuclide impurities were remained high and activity of αimpurities exceeded international standard requirements, i.e. 7.716E-7 µCi/mCi Mo-99. Abstract

This paper will discuss Islamic educational studies that is reviewing how to find, limit and define problems and problem-solving concepts. The central question of this paper is to describe how to solve the problem in Islamic educational research. A researcher or educator who has the knowledge, expertise, or special interest on education for example is usually having a sensitivity to issues relating to educational research. In the research dimension of religious education, there are three types of problems, namely: Problems foundation, structural problems and operational issues. In doing research in Islamic education someone should understand research problem, limiting and formulating the problem, how to solve the problem, other problem relating to the point of research, and research approach.

The first report of myeloma with IgD κ and free κ in Indonesia

Abstract

IgD κ myeloma is a rare plasma cell neoplasm case and has never been reported before in Indonesia. In normal condition, IgD level in blood is very low, therefore increase of IgD level in myeloma could be missed by serum protein electrophoresis. A case of a 59 years old female with severe bone pain is reported. In radiology evaluation, there were thoracal compression fracture and thoracal foramen narrowing. For this patient, the myeloma diagnosis was based on WHO criteria, the stage IIIb was based on Durie and Salmon criteria, and bad prognosis with prognostic index stage III diagnosis was based on International Prognostic Index from International Myeloma Working Group, respectively. In serum protein electrophoresis we found a very small monoclonal spike and in immunofi xation there were monoclonal IgD κ and free light chain κ. Ewing sarcoma of the left big toe with trans-articular skip lesion

Abstract

We report the case of the patient who had Ewing Sarcoma in whom radiological and hystopathological appearances revealed a tumor mass in the left big toe along with trans-artikular skip lesion on the left diaphysis of tibia. In Cipto Mangunkusomo Hospital since 1995 until 2004 we have found 20 Ewing sarcoma cases, but only one skip lesion Ewing sarcoma was found. The diagnosis of transarticular skip lesion in association of Ewing sarcoma was confirmed in clinicopathological conferrence. The initial evaluation of all patients included the recording of the medical history, physical examination, and hematological studies. Radiographs of the chest and the site of the primary tumor were made routinely. Systemic staging was performed with use of total-body bone scan. Ray amputation of left big toe and open biopsy from mass of mid-shaft of tibia had been done to confirm the diagnosis. The patient underwent induction chemotherapy and above knee amputation. Ten months after diagnosis, he died because of advanced-distant metastasis. (Med J Indones 2008; 18: 139-44)Key words: Ewing sarcoma, trans-articular skip lesion

Periosteal osteosarcoma: A case report

Abstract

Periosteal osteosarcoma is a rare type of malignant bone neoplasm, with predominantly cartilaginous component and arising on the bone surface. Reports of the case in the literature were rare. Last case was reported by Mayo Clinic in 1999. We report a case of periosteal osteosarcoma in a 17-year-old male, who was treated surgically with a limb salvage procedure, neoadjuvant and adjuvant chemotherapy were also given to the patient. Abstract

This paper deals with the reaction of al-Shinqīṭī towards this latter way of mystical vision, particularly in the case of Aḥmad al-Tijānī. For al-Shinqīṭī, sufis’ claim of having a fully consciousness physical contact with the Prophet after his death is impossible because nothing, whether religious or rational proofs, can sustain it. The extreme case of such claim is expressed by Aḥmad al-Tijānī who, insists that a sufi (ṣūfī) can really see the Prophet with his physical eyes. In al-Tijānī’s opinion, the ability of the physical eyes to see the Prophet when awake was a common trait of those who attained the status of pole (quṭb).DOI: 10.15408/ref.v13i3.902

Prevalence and Distribution of Anemia Risk Factor in Patient with Chronic Hepatitis C who Has Combination therapy of Interferon Alpha and Ribavirin

Abstract

Background: Interferon alfa and ribavirin combination therapy is one of effective standard therapy for chronic hepatitis C (CHC.) However, anemia is a common side effect of this therapy that patients have to reduce or discontinue ribavirin therapy. But ribavirin dose reduction or discontinuation can reduce the effectivity of the therapy. Hence, it is important to know the prevalence of anemia and to determine the factors associated with anemia. Objective: To know the prevalence of anemia and some risk factors associated with anemia caused by the combination therapy in chronic hepatitis C. Method: Sixty one of CHC patients who received combination therapy were included in this study. The study used cross sectional design and data were obtained by measured complete blood count on 8th week of therapy. Result: Subjects 47 (77%) were males, 14 (23%) were females with mean age 38.9 years. Subjects had genotype 1 and 4 were 23 (71.9%) and 44 (72.1%) subjects received 1,000 mg ribavirin. Prevalence of anemia was found to be 52.5%. On multivariate analysis, only pretreatment hemoglobin concentration < 14 g/dl was found to be the risk factor of anemia. Conclusion: Prevalence of anemia was 52.5%. Pretreatment hemoglobin concentration < 14 g/dl was only found to be the risk factors of anemia. Although age > 50 years or female were not found to be the risk factor of anemia but patient with these risk factors should be carefully monitored. Intervention to prevent anemia should be considered to these patients. Eight subjects from 32 anemia patients had ribavirin dose reduction, and no patient had discontinuation treatment on 8th week of therapy.   Keywords: chronic hepatitis C, IFN-alfa- RIB combination therapy, risk factors of anemia
