Abstract

Integrated Management of Childhood Illness (IMCI) has been implemented in Banyumas Regency as a part of efforts to lower infant mortality and under five children Death. But in 2007, under-five mortality is still high at 9.6 / 1,000 live births. A preliminary study conducted in the three health centers shows that not all infants got good care IMCI well. This is due to the existence of several sub-optimal care by IMCI officer. The purpose of research is to describe the performance of health workers in implementing IMCI consisting of quality, quantity, cost effectiveness, supervision requirements and timeliness in service IMCI in Banyumas. This is an observation research consisting of 99 samples of IMCI officers. The Research instrument is a quesionar that the validity and reliability have been tested. The data analysis technique is frequency distribution that used to obtain description of the research variables.  The results show that not all officers carried SOP of IMCI (6% to 17%). Officers did not provide IMCI services to all under five children who came clinic (37.4%). Medical history records of the under five patient were not recorded separately. Supervision by the leaders did not involve the IMCI officers in finding altrenatif problem solving. Not all health centers submitted reports on time. Abstract

Pencapaian target penemuan kasus baru TB BTA positif di Kabupaten Bangka Tengah tahun 2009-2011 menunjukkan angka dibawah 70%. Peran penanggungjawab program TB sangat penting dalam pencapaian target. Tujuan penelitian ini adalah menjelaskan bagaimana kinerja penanggung jawab program TB dalam penemuan kasus baru TB BTA positif. Penelitian ini merupakan penelitian deskriptif kualitatif dengan pendekatan cross sectional. Informan penelitian adalah 4 orang penanggung jawab program TB dari Puskesmas terpilih. Data dikumpulkan dengan wawancara mendalam dan dianalisis menggunakan metode analisis isi (content analysis ). Hasil penelitian menunjukkan bahwa kinerja penanggung jawab program TB dalam penemuan kasus baru belum baik karena sebagian besar hanya melakukan penyuluhan di Posyandu, menjaring suspek dari pasien yang berkunjung di Puskesmas, persepsi kurang puas terhadap imbalan yang diterima dan persepsi terhadap beban kerja yang dirasakan cukup berat. Abstract

Angka Kematian Ibu di Puskesmas Tanjung tahun 2011 (559/100.000 KH) yang masih jauh dari target penurunan tahun 2014 (118/100.000 KH), adanya trend penurunan cakupan K1 dan K4 selama 3 tahun terakhir, serta pelayanan Antenatal Care yang belum sesuai harapan, mendorong terselenggaranya penelitian ini untuk mengetahui pengaruh persepsi kualitas pelayanan antenatal care terhadap kepuasan ibu hamil. Jenis penelitian adalah observasional dengan pendekatan cross sectional, sampel sebesar 89 orang dengan teknik pengambilan secara konsekutif sampling dan analisis data menggunakan chi-square dan regresi logistik. Hasil penelitian menunjukkan persepsi tidak baik pada kualitas pelayanan dimensi reliability 43.8%, responsiveness 60.7%, assurance 51.7%, empathy 57.3% dan tangibles 43.8% dan persepsi ibu hamil yang tidak puas sebesar 49.4%. Hasil analisis menunjukkan ada hubungan antara persepsi reliability, responsiveness, assurance, empathy dan tangibles dengan kepuasan ibu hamil. Secara bersamaan variabel yang berpengaruh adalah empathy (p = 0.0001, OR=111.507), reliability (p = 0.008, OR=22.466) dan responsiveness (p = 0.008, OR=15.074). Perbaikan kualitas empathy, reliability dan responsiveness pada pelayanan ANC diperlukan dalam peningkatan kepuasan ibu hamil. Kata Kunci : Kualitas Pelayanan Antenatal Care (ANC), Kepuasan Pelayanan

Abstract

Influence of the Nurse’s Perception about Function of the Head Nurse Management toward Nursing Care Management Implementation at Inpatient Room of Dr. Amino Gondohutomo District Mentally Hospital in Semarang. The functions of management such as planning, organizing, guiding, monitoring, and controlling had not been done well by the head nurse. Nursing management is important to determine the nursing care services for client at inpatient room. Aim of this research was to analyse influence of the nurse’s perception about function of the head nurse management toward nursing care management implementation at Inpatient Room of Dr. Amino Gondohutomo District Mentally Hospital in Semarang. This was a quantitative and qualitative research with cross sectional approach. Population was nurses at inpatient room and all head nurses. Number of sample was 52 nurses taken by Proportionate stratified random sampling at 12 inpatient rooms. The questionair was used to collect data continued with cross check to the head nurses about perception of management. Result of this research showed that implementing of nursing care management is good (65,4%). Perception of a nurse about the head’s nurse management namely: (1) Good planning (53,8%), no significant association (p=0,857), and no influence (p=0,543, Exp B=0,700). (2) Good organizing (55,8%), no significant association (p=0,982), and no influence (p=0,982, Exp B=1,013). (3) Good guiding (75%), significant associated (p=0,002), and significant influence (p=0,035, Exp B=4,888). (4) Not good monitoring (51,9%), significant association (p=0,007), and significant influence (p=0,068, Exp B=3,679). (5) Not good controlling (59,6%), no significant association (p=0,873), and no influence (p=0,873, Exp B=1,100). Conclusion of the research is the nurse who perceives not good the head’s nurse guiding has a risk to be not implementing the nursing care management (p=0,035, Exp B=4,888), and the nurse who perceives not good the head’s nurse monitoring has a risk to be not implementing the nursing care management (p=0,068, Exp B=3,679).

Abstract

Abstract Background : The Department of Environmental Health is one of department in Central Java Provincial Health Office. This department is responsible to water supply program. Nowdays, the department has implemented the Surveillance Information System of Nonpipe Water Supply. But till now there are some weaknesses, the data and  information are used only by The Sub Department of Health Promotion and Environmental Health. There aren’t other subdepartment used the data and information, so there isn’t integration or collaboration in using some similar data or information.    The objective of this research is to analyze the model of Surveillance Information System of Nonpipe Water Supply which is now implemented in The Department of Environmental Health. Methods : This is an operational research that use qualitative method and system development life cycle approach. The subjects are the chief of The Sub Department of Health Promotion and Environmental Health, the chief of the section of environmental health and also the water quality surveillance officers. While the object of this research is The Surveillance Information System of Nonpipe Water Supply which is implemented. The data will be analyzed using the content analysis, descriptive analysis and the data flow diagram . Result : After analyzed the Surveillance Information System of Nonpipe  Water Supply, we found that there were some problems. The problems are rapidity, accuracy, completeness and accessibility of the data of water supply surveillance. We suggest a new system that there will be a new entity. The new entities are the provincial house of representative and the subdepartment of diseases prevention and elimination. Keywords: health information system, water quality

Abstract

Pada tahun 2006 sosialisasi program MTBS dan pelatihan kepada petugas puskesmas telahdilakukan, dimana masing-masing Puskesmas diwakili oleh 1 orang tenaga medis (dokter)dan 2 orang tenaga paramedis (bidan, perawat). Akan tetapi kematian balita di kabupatenPasuruan mengalami kenaikan, yaitu tahun 2007 sebesar 5,2/1000 kelahiran hidup, tahun2008 sebesar 5,4/1000 kelahiran hidup dan tahun 2009 sebesar 6,1/1000 kelahiran hidup. Darikematian tersebut diketahui penyebabnya antara lain karena gizi buruk, pneumonia, DBD,diare serta infeksi. Oleh karena itu perlu dilakukan analisis terhadap faktor faktor dalamimplementasi program MTBS di Puskesmas Kabupaten Pasuruan. Jenis penelitian adalahdeskriptif kualitatif, metode sampling yang digunakan adalah purposive sampling. Sebagaiinforman utama adalah petugas MTBS (dokter, bidan, perawat) di Puskesmas wilayahperkotaan dan pinggiran kota yang melakukan MTBS, berjumlah 12 orang. Sedangkansebagai informan triangulasi adalah 4 kepala Puskesmas, satu Kasie Kesga Dinas KesehatanKabupaten Pasuruan. Variabel dalam penelitian ini adalah faktor komunikasi, faktor sumberdaya, faktor disposisi, serta faktor struktur birokrasi. Penelitian memberikan hasil sosialisasidan pelatihan program MTBS sudah dilakukan. Petugas yang melayani balita sakit belummenunjang keberhasilan pencapaian tujuan MTBS oleh karena belum semua petugasmendapatkan pelatihan MTBS, jumlah petugas tidak sebanding dengan jumlah balita sakityang berkunjung. Seluruh petugas MTBS mempunyai sikap positif untuk mendukungprogram MTBS. Meskipun sudah tersedia SOP namun tidak semua petugas menggunakannyadalam melayani MTBS.Pembinaan dari DKK belum dilakukan rutin, supervisi masihbersifat umum, serta tidak ada tindak lanjut yang diberikan. Agar pelayanan MTBS terlaksanadengan baik maka perlu ditingkatkan sosialisasi SOP yang disertai pelatihan yang meratauntuk semua petugas serta supervisi yang spesifik pada MTBS.

Abstract

Facilitative supervision of primary healthcare center (puskesmas) coordinator midwife (bikor) toself-employed practice midwives (BPM) constituted an ongoing and on-target clinical andmanagerial supervision. Current implementation of facilitative supervision was not optimum;therefore, further study was required. Objective of this study was to analyze the implementationof facilitative supervision of primary healthcare coordinator midwives to BPM concerningpregnancy and delivery services in Semarang city. A qualitative approach was applied in thisstudy. Data were analyzed using content analysis method. Study variables were supervisedaspects (clinical, non-clinical, logistic, and managerial aspects), assessment method andimplementation stages. Results of the study showed that supervision had included all supervisedaspects. Not all bikor used delivery care list. Supervision on clinical aspect did not include directobservation. Pre supervision implementation steps: facilitative supervision training for bikor ofthe primary healthcare centers had not been done, reference book for facilitative supervisionhad not been distributed. Steps of the implementation of supervision were not performedaccording to the guidelines. This was caused by on the spot implementation of supervision.Suggestion for District Health Office is to conduct facilitative supervision for all primaryhealthcare centers coordinator midwives. Abstract

Based on bed occupancy ratio (BOR) in 2010, it was known that the number of inpatient visit atPermata Bunda hospital was relatively high. As a consequence, the need of linen increased. Basedon preliminary study, it was known that planning, implementation, and controlling of linen werenot adequate. The objective of this study was to know how linen in Permata Bunda hospital Purwodadi was managed.This was a descriptive explorative study with cross sectional approach. Study population was workersof Permata Bunda hospital Purwodadi who were involved in linen management. Main informantswere chief of nurse in the general ward, chief and assistance chief of linen logistic unit, chief andassistant chief of laundry unit. Triangulation informants were general manager, logistic unit chiefand patients.Results of the study showed that planning for linen requirement estimation was not good because it wasdone based on lost or damaged linen assumptions. Ordering process was still insufficient due to nospecial staff who ordered the linen. Supply linen was inadequate due to no standard for linen supply.Maintenance of linen was good; it was performed according to the right phases and procedures.Distribution of linen was not good due to separation of the rooms by the street. Linen service to patientwas inadequate; not all patients’ linen was changed once a day. Linen management organization wasnot good; it was still done together. Linen stock control was not good; calculation for linen requirementwas still lacking, and it had not used the standard of 3 times of the number of bed. Implementation oflinen inventory was still inadequate; no specific recording was done according to the dirtiness level ofthe linen. Linen coding or identification was still inadequate; coding was not in detail.Based on the results of the study, it is suggested that calculation of linen requirement should bedone based on the number of bed with availability of 3 par stock linen in each bed. Written standardoperating procedure in linen management should be produced; it will be used as guideline orstandard by linen staffs or management. Identification or coding of linen should be improved; itincludes hospital name, room name, type of linen and type of material.

Abstract

Growth and development detection and early intervention stimulation (SDIDTK) was activities tostimulate basic skill of children 0-6 years old, hence they grew and developed optimally. Less stimulation could cause deviation in the growth and development of the children, and disorder could be permanent.Coverage of SDIDTK in Malang city in 2010 reached 56.13%, in 2011 was 69.41%, these coveragewere still below the target of 90%. Objective of this study was to analyze determinant factors of theimplementation of SDIDTK by cadres in primary healthcare centers in Malang cityThis was an observational analytical study with cross sectional approach. Independent variableswere cadre characteristics (age, education, occupation, length of period as a cadre), knowledge,attitude, facilities, community supports, midwives supports, and training. Dependent variable wasimplementation of SDIDTK. Study subjects were 81 cadres who conducted SDIDTK, and they wereselected purposively. Data were collected through interview guided by structured questionnaireand observation list. Chi square test and Fisher exact test were applied in the bivariate analysis;logistic regression method was applied in the multivariate analysis.Results of the study showed mean of respondents’ age was 45.4 years, average length of period asa cadre was 10.6 years, 49.4% of respondents’ level of education were high school, 88.9% ofrespondents’ occupation were housewives, 50.6% of SDIDTK were good, respondents with goodknowledge were 95.1%, respondents with good attitude were 61.7%, 23.5% of facilities wereadequate, 74.1% of respondents stated that community supports were good, 88.9% of respondentsstated that midwives supports were good. No association was found between age of cadres (p=0.311), level of education (p= 0.146), occupation (p= 0.647), knowledge (p= 0.551), attitude (p=0.218), facilities (p= 0.233), community supports (p= 0.749), midwives supports (p= 0.516) andthe implementation of SDIDTK. Length of period as a cadre and training together had an associationwith the implementation of SDIDTK by cadres.Determinant factors of the implementation of SDIDTK by cadres were training and length of periodas a cadre. Improvement of the implementation of SDIDTK is required; it can be done by conductingSDIDTK training to all posyandu (health service post) cadres.

Abstract

In 2009-2011, coverage of delivery by health workers in Luwu district was about 76-77%, and thenational target was 90%. This could be caused by post-delivery maternal who were not satisfytowards delivery service quality by midwives. Objective of this study was to identify associationbetween normal delivery service quality by village midwives and post-delivery maternal satisfactoryin Luwu district, South Sulawesi.This was a quantitative study using observational-analytical design with cross sectional approach. Study subjects were 70 village midwives and 70 post-delivery maternal in Luwu district that wereselected consecutively. Data were collected using structured questionnaire. Chi-square and Pearsoncorrelation were applied in the bivariate data analysis.Results of the study showed that midwives average age was 29 ± 4.4 years, and average workperiod was 6.5 ± 4.07 years. The majority of midwives level of education was D3 in midwifery(92.9%). Maternal average age was 27 ± 4.3 years. Majority of mothers were unemployed (84.3%)and in the intermediate school or higher level of education (57.1%). The majority of midwivesprovided good quality of delivery service. No significant associations were found between qualityof delivery service and age (p= 0.147), work period (p= 0.214), education (p= 0.343). No significantassociations were found between satisfaction towards village midwives delivery service and age(p= 0.622), occupation (p= 0.072) of patients. Significant association was found between maternaleducation and satisfaction towards village midwives delivery service (p= 0.005). Sixty percent ofpatients were satisfied. Significant association between village midwives delivery service qualityand maternal satisfaction (r= 0.164, p= 0.043) was found.In conclusion, quality of midwives delivery service had positive association with maternalsatisfaction. Maternal level of education had negative association with satisfaction towards villagemidwives service quality.
