Abstract

Pelaksanaan reformasi perencanaan dan penganggaran yang berbasis kinerja merupakan bagian dari reformasi birokrasi untuk pelaksanaan good governance di dalam upaya pelayanan masyarakat yang lebih baik, dengan penggunaan anggaran publik yang lebih transparan, akuntabel, efektif dan efisien.Hasil penelaahan dokumen LAKIP Dinas Kesehatan Provinsi Jawa Tengah tahun 2012 serta dokumen pelaksanaan perubahan anggaran APBD, dapat dijelaskan pada kegiatan percepatan penurunan AKI dari target output 104 per 100.000 KH, namun realisasi capaiannya sebesar 116,34 per 100.000 KH dengan alokasi anggaran APBD. Tujuan penelitian ini untuk mengetahui evaluasi pelaksanaan perencanaan dan penganggaran kegiatan percepatan penurunan AKI berbasis kinerja Dinas Kesehatan Provinsi Jawa Tengah tahun 2012.Penelitan ini menggunakan rancangan penelitian kualitatif dengan pendekatan retrospektif. Populasi penelitian adalah seluruh satker pada Dinas Kesehatan Provinsi Jawa Tengah dengan menggunakan pendekatan Purposive Sampling, diambil sampel dalam penelitian ini ada 10 orang, terdiri atas 6 orang informan utama dan 4 orang informan triangulasi. Analisis data menggunakan pendekatan deskriptif analitik kontekstual yang disajikan dalam pembahasan. Hasil penelitian menunjukkan perencanaan kinerja di Dinas Kesehatan Provinsi Jawa Tengah tahun 2012 belum efektif di dalam mencapai target dari penurunan AKI dinas (104/100.000 KH), berdasarkan hasil wawancara mendalam (indepth interview) dan telaah dokumen, diketahui penyusunan indikator kinerja dari percepatan penurunan AKI, masih ada ketidaksesuaian di target dengan yang ada di Renstra namun ada kesesuaian dengan RAD serta realisasi anggaran mencapai 96 %.Dari penelitian dapat disimpulkan, implementasi perencanaan dan penganggaran kegiatan percepatan penurunan AKI berbasis kinerja Dinas Kesehatan Provinsi Jawa Tengah tahun 2012, dilihat dari penggunaan indikator kinerja keluaran, belum efektif di dalam mencapai target dari penurunan AKI dinas. Di dalam pelaksanaannya, masih ditemukan adanya kendala pada sosialisasi informasi yang dilakukan, sumber daya yang dialokasikan, kurangnya komitmen dari pelaksana, pembagian tugas dan koordinasi yang belum efektif dan kurang komprehensifnya supervisi dan evaluasi yang dilakukan.    Reform planning and budgeting implementation being based on performance represent the part of bureaucracy reform for the implementation of good governance in better society service effort, with using of public budget which more transparent, accountable, efficient and effective. Result of observation the LAKIP document from Health Office Central of Java of year 2012 and also implementation change of APBD budget document, can be explained at activity of accelerate degradation Mother Mortality Rate (MMR) from output goals 104 per 100.000 live born, but realize its performance equal to 116,34 per 100.000 live born, with the APBD budget allocation. Target from this research to know the evaluation of planning and budgeting implementation activity of accelerate degradation Mother Mortality Rate based on performance in the Health Department Central of Java Province of year 2012. This research used qualitative project research with approach of retrospective. Research population is all of employee at Health Department Central of Java Province by using approach of Purposive Sampling, taken by sampling in this research there are 10 people, consist of 6 primary informant people and 4 triangulation informant people. Data analysis use contextual analytic descriptive approach presented in the explanation. Result of research show performance planning in Health Office Central of Java Province of year 2012 not yet effective reaching goals from degradation of MMR on department (104/100.000 live born), pursuant result of in-depth interview and document review, known compilation of performance indicator from accelerate of degradation MMR, there is still inappropriate in goals with strategic planning, but an appropriate with the District Action Plan (RAD) and budgeting realization to reach 96 %.From research can be concluded, planning and budgeting implementation activity of accelerate degradation Mother Mortality Rate based on performance in the Health Office Central of Java Province of year 2012, seen from usage of output performance indicator, not yet effective in reaching goals from degradation MMR on department. In its implementation, still found the obstacle at information socialization have done, allocation of resources, lack of commitment from implementer, division of task and coordination which not yet effective, and less comprehensive it supervise and evaluation have done

Abstract

Poor collaboration can affect the quality of service to patients, thus affecting the recovery of patients. For the profession, collaboration can affect the relationship and the job satisfaction of both doctors and nurses. Poor collaboration between doctors and nurses in hospital management can can influence a decrease number of Bed Occupation Rate  and an increase in length of stay so that it can have an impact on patient satisfaction.This study used a quantitative design and descriptive analytic with qualitative approach through observation, review of documents, in-depth interviews  with informants who already selected and determined using cross sectional approached. The sample in this study were all general practitioners and all nurses who work in Permata Medika Hospital until December 2015, ie 15 doctors and 15 nurses.The results show that the control of power  is still difficult due to a balanced relationship between the two professions and poor communication. The scope of practice between doctors and nurses has established good collaboration. The shared interests of the two professions do not pay attention and see the procedure of service. At the collaborative goals of doctors and nurses there is overlapping responsibility for reasons of rushed work without regard to the Standard Operating Procedure (SOP) and job description.The Recommendations for management are need to be carried out definite career path for nurses, increase the knowledge and enhance the collaboration both nurses and doctors, increase the collaboration between nurses and doctors, follow up the results of service audits need to be coordinated and controlled

Abstract

Antenatal care to pregnant women with malaria was a part of an integrated antenatal care; thisantenatal care was focused on managing pregnant women with malaria through early detection,treatment and prevention of malaria and its complication. Objective of the study was to analyzethe implementation of an integrated antenatal care and malaria service to pregnant women, andthe analysis included communication, disposition, availability of resources and bureaucracystructure aspects in Tobelo primary healthcare center (puskesmas), Tobelo district, NorthHalmahera district, North Maluku province. This was an observational study with qualitativeapproach. Selection of informants was conducted using purposive sampling. Data were collectedthrough in-depth interview. Content analysis was applied in the data analysis. Results of thestudy showed that integrated antenatal care and malaria service to pregnant women were not performed properly by midwives. They did not perform the service according to the guideline onthe management and prevention of malaria on pregnant women. This was caused by insufficientnumber of midwives, no clear standard operating procedure (SOP), and a number of midwiveswho had not attended in the training on prevention and management of pregnant women withmalaria. Communication among people who provided information on the integrated antenatalcare to pregnant women with malaria was conducted throughthe leader. The number ofmidwives who implemented integrated antenatal care and malaria service was still insufficient.Disposition of midwives’ attitude was shown by willingness and eagerness in socializingintegrated antenatal care and malaria service. However, midwives were still reluctant in givingOAM due to past experiences about the dead of pregnant women with malaria and OAMtreatment. Standard operating procedure was not formulated. Training had been performed;however, not all midwives had been trained.

Abstract

Social Insurance Organizing Body is to protect community and Indonesian workers from healthproblems. Until 2010, many companies in Mataram City had not been a member of social insurancefor workers because there was a lack of socialization in terms of the importance of health insurancefor companies’ sustainability. This research aimed to identify factors influencing readiness ofcompanies in implementing Social Insurance Organizing Body.This was analytic survey with cross-sectional approach. Samples were 113 company managers.Data were collected using a questionnaire and analyzed using the methods of univariate, bivariate,and multivariate (Linear Regression test).The result of this research revealed that percentage of respondents graduated from Diploma III was27.4%, type of business based on the highest income level of a micro business (64.6%), and type ofsmall business based on number of employees (90.3%). In addition, mostly respondents had lowknowledge (52.2%), bad characteristics of innovation (50.4%), good communication channel(52.2%), bad social system (54%), bad promotion of reforming agent (50.4%), and unready company(54%). Furthermore, variables of knowledge (p=0.0001, r=0.898), characteristics of innovation(p=0.0001, r=0.888), communication channel (p=0.0001, r=0.886), social system (p=0.0001,r=0.894), and promotion of reforming agent (p=0.0001, r=0.704) had significant correlation withreadiness of companies in implementing Social Insurance Organizing Body. Multivariate analysisshowed that variables of knowledge, characteristics of innovation, communication channel andsocial system jointly influenced readiness of companies (F test= 223.404).As a suggestion, dissemination of information needs to be conducted among companies. Additionally,providing information in companies needs to be done by using a method of interpersonalcommunication. Provided information must be clear and complete.

Abstract

The local regulation Number 4 Year 2012 about health of maternal, infant, baby, and childrenunder 5 years old in District of Hulu Sungai Selatan had not been optimally implemented becauseit was not supported by other regulation below its regulation since it was approved two years ago.This research aimed to arrange a draft outline of Regent regulation as an operational regulation ofthe local regulation Number 4 Year 2012 about Health of Maternal, Infant, Baby, and Childrenunder 5 years old in District of Hulu Sungai Selatan in Province of South Borneo.This was qualitative research presented using descriptive-explorative methods. Number of maininformants were 4 persons selected using non probability sampling with purposive sampling. Theyconsisted of head of District Health Office, head of family health department, and heads of maternaland child health section and nutritional section. Informants for triangulation purpose were dividedinto two groups namely 12 health workers and 12 people of target of the local regulation. Datawere collected using in-depth interview, FGD, Brainstorming, and workshop. Furthermore, datawere analyzed using content analysis.The result of this research showed that: 1) the indicators to evaluate the local regulationimplementation were as follows: a) access to reach service facilities and obtaining the variousservices depended on geographical location; b) the services of Health of Maternal, Infant, Baby,and Children under 5 years old had not been used by all target people; c) frequency of target peoplewho obtained the services varied depending on types of providers; d) there was any irrelevance inimplementing the local regulation; e) there was any unstandardized service; f) a form ofaccountability was only as a routine activity; g) a released policy was not appropriate with necessityof target people. 2) There had been done a process of problem identification and input to arrangethe draft outline of the regent regulation from target people and health workers. 3) Some importantpoints that would be written on the draft outline were: authority, local government responsibility,delivery service, types of workers, types of health service means and qualification needed for theservices, reporting procedure, complaint, forming a complaint unit and a procedure to verify andlevel of readiness.

Abstract

Government  wants to integrate traditional health care in the healthcare system, then some of the regulatory framework published. Including Permenkes no. 1109 tahun 2007, Kepmenkes no. 121 tahun 2008 and PP no 103 tahun 2014. The Government is also targeting the implementation of the integration of traditional health care in the Ministry of Health Strategic Plan 2015 - 2019, with indicators 15% Health Center organizes traditional medicine at 2015. In Semarang until mid 2015 only 3%(1) Public health center implement the integration of traditional health services which is Halmahera Health center.This study aimed to analyze the implementation of the integration of traditional health services in Puskesmas Halmahera Semarang. This research is a descriptive study using a qualitative approach. Collecting data with in-depth interviews. The informants are health workers implementing the integration of traditional health services in Halmahera Health center, Section Head of Pharmaceuticals, Food Beverage and Health Supplies, Head of Health Services Semarang City Health Office, Pharmaceutical and Food Supervisory Staff BPOM.Research shows the Halmahera health centers and DKK do not yet know the policy indicators, Nurses do not have SBRTPKA (Proof of Registration Complementary Alternative Healers)   and STTPKA (Letter of Complementary Alternative Healers Task Force) because of they does not know how to get SBRTPKA and STTPKA. There is no separate room for acupressure, there is no allocation funds for the program, they do not have Standard Operating Procedures. There are to high workload for nurse, doctors rarely suggest acupressure. Semarang do not have regulatory about alternative complementary medicine. The society has the will to utilize traditional health care, but they have objections about the cost of traditional health care utilization

Abstract

The Government of the Republic of Indonesia considering to implement the provisions of Pasal 59 ayat (3) about traditional health care in Undang-Undang Nomor 36 Tahun 2009 about health, need to establish government regulation of Traditional health care, then demoted to Peraturan Pemerintah Nomor 103 Tahun 2014 about traditional health ministry to revise Keputusan Menteri Nomor 1076 tahun 2003 about  traditional medicine and Peraturan Menteri Kesehatan Nomor 1109 tahun 2007 about implementation of complementary medicine Alternative , which has been issued before.After the set appeared the response from the Association of Traditional Therapist in Semarang region. This research aims to identify the readiness of the Association of Traditional Thearapist in Semarang about the implementation plan of PP Nomor 103 Tahun 2014. This research is descriptive qualitative research with in-depth interviews. The subjects of these studies amounted to 6 people as the main informant and 5 people as informant triangulation. The main informant was chosen based on the type of traditional medicine, they are herb and skills, while the informant triangulation is the Parties assessed involved in the world of traditional medicine and health care.Unreadiness of the informant described which represent the two Traditional Therapist Associations, namely in terms of understanding the different policies, the attitude tends to be refused, the characteristics of the organization that are not ready, the relationships between the organizations is quite good, as well as resources and support of social, political, economic, which is quite conducive. Both the Association of  Traditional Therapist need to improve knowledge of the policy, to more open minded, improve quality, enhance the commitment of the Organization, and expanding communication with the others.

Abstract

Infant mortality is infants death who aged 0 to 11 months. Infant Mortality Rate Purworejo from year to year has increased (in 2013 amounted to 11.54/1000 kh and 2014 amounted to 12.55/1000 kh), and still higher than IMR Central Java (10.41/1,000 kh). Region of Ngombol’s PHC is one areas that experienced high infant mortality and increased for 2 years. This study aims to determine direct and indirect causes of infant mortality in area of Ngombol’s PHC. Research method using qualitative descriptive which retrospective. Research sample are main informant consisted of 11 women whose babies died in 2014, and informant triangulation head of PHC, midwife coordinator, and 3 village midwife. Research variables consisted of maternal factors, infant, environmental, and health services. Direct causes of infant mortality in Ngombol region on 2014 was LBW, asphyxia and congenital abnormalities. Age of mother at risk (> 30 years) was found in maternal LBW infants and asphyxia. Almost all women had experience pregnancy complications such as hypertension, HB less, KEK. All mothers get exposure chemical smoke from her husband. Almost all informants have cage attached to house. Quality of antenatal, childbirth and neonate care still less than optimal, both in terms service providers and community. Most mothers do not know pregnant women class program because less socialization. GSIB program is not running as a cross-sectoral support is lacking. Conclusion this study is indirect causal factor of infant mortality in Ngombol region dominated by factors maternal age, pregnancy complications, environment exposure, and health services quality.

Abstract

Implementation of a regional action plan is a document that refers to regional development for the increasing achievement of health development in the Region,the Regional Action Plan removes the MDGs. Semarang city includes as the category of the highest maternal mortality rate in fifth place in Central Java, so it is necessary to seek to reduce AKI, the government issued a regulation mayor No. 14 In 2013, a testament to the commitment of RAD MDGs to support the acceleration of the achievement of the MDGs in the improvement of maternal health in point 5. This study used a qualitative research design approach Retrospective study of population the local area that Health Departement, Bapermas and BAPPEDA.This study used purposive sampling thatwas taken from five respondents consisted of 2 main Informants and 3 Triangulation Informants. Analysis of the data presented in the discussion of the descriptive approach. Based on the results of in-depth interviews (depth interview) is that the implementation of the Regional Action Plan point 5 Maternal Health in document indicator RAD MDGs are used not in line with the indicators used by district Departement particular to maternal health, the lack of information with the involvement of stakeholders, barriers and challenges are not optimal integration between central and local government, which has not been structured system, and lack of funds. Then, from the aspect of good communication is not optimal communication both sectors and programs. Disposition aspects of the lack of support and commitment of the entire structural level are in accordance with official rules. Resource aspects of resource constraints, both infrastructure and funds as well as aspects of Bureaucratic Structure constrained schedule so that the intensity less intense coordination among stakeholders

Abstract

The scope of exclusive breastfeeding survey by Department of Health in dicated that in 2012, the mothers who breasfeed their children in District of Semarang were only as many as 34.6% lower than National Target (80%). PT. APAC employees consist of 4,081 worker women, with a total of 5% of the bore and only 4 % were breastfeeding. Policies has been made such as Statutes, but there are still many companies has not implement those policies.The objectives of this research was to find out the implementation of exclusive breastfeeding policy for workers during working hours in PT. APAC, the District of Semarang. The factors that affecting implementation policy are communications, resources, dispositions and bureaucratic structures.This research used qualitative method, and in-depth interview and observation for data collecting method. Respondents, or the subject of the research are Section Head of Health, Safety, Environment in PT APAC as prior informant, Section Head of Health Promotion in Department of Health the District of Semarang, Section Head of Manpower and Transmigration Department and breastfeeding working mother as triangulation respondent.The result of the research showed that the implementation of exclusive breastfeeding in PT. APAC in District of Semarang did not work well. This is due to the lack of internal regulations made by the company, as well as a chance to breastfeeding various factors affecting including: 1) Communication, lack of socialization from the company to all employees; 2) Availability of fund for implementing exclusive breastfeeding policy, because the lack of specific official individuals for breastfeeding such as breastfeeding counselor in the company; 3) Attitude and commitment of executor towards breastfeeding policy in PT APAC in District of Semarang, and coordination within Unit Supervisors; 4) Unavailable Standard Operating Procedure (SOP) and Operating Coordination with Unit Supervisors related to exclusive breastfeeding policy in the company.
