Abstract

Tuberculosis is an infectious disease and is an important public health problem. Based on data from East Java Province Health Department reported that number of tuberculosis patient in Surabaya is the highest in East Java on year 2014. Early identification and good treatment based on the result of sputum identification are the strategy use to control tuberculosis widespread. So that why, microscopic observation to identify acid fast bacilli (AFB) is the fundamental stage to determine recovery treatment. Initial sputum smear positivity is necessary to determine infectious graded. The objectives of the study were to identify of initial sputum smear positivity on treatment failure of category 1 therapy for pulmonary tuberculosis in RS Paru Surabaya year 2011-2014. This study used case control method with quantitative approach. Forty two samples were taken from secondary data. Case group is 21 samples who have treatment failure and control group is 21 successful treatment. Samples were selected by simple random sampling. The chi square correlation showed that highly positive initial smear (p = 0,045; OR = 5,4) have correlated and risk factor to treatment failure on category 1 therapy for pulmonary tuberculosis. The conclusion is patient’s high positive sputum smear initially correlated to treatment failure on category 1 therapy for pulmonary tuberculosis in RS Paru Surabaya year 2011–2014.Keywords: initial sputum smear positivity, treatment failure,, category 1 therapy for pulmonary tuberculosis

Abstract

ABSTRACTDiabetes Mellitus is a chronic disease whose prevalence continues to increase. WHO predicts that diabetes will be 7 major diseases that will cause the death in 2030. Many people with diabetes are not aware of  it so that they get complications. The increasing number of people with diabetes mellitus who develop complications because of the bad controlling of blood sugar levels  and the lack of support from family. The Complications were observed in this study is microvascular complications. This study aims to analyze between family support and control blood sugar levels with the incidence of microvascular complications in the patients with symptoms of diabetes mellitus in the Community Health Center Jagir Surabaya.This research uses analytic observational study sample of patients with diabetes mellitus who had diabetes mellitus ≥ 5 years as many as 34 samples. The Sampling was done by the simple random sampling, while the data analysis was done by the Chi Square test.The results showed that there was no relationship between family support with symptoms of microvascular complications (p=0.069), but there is a relationship between the control of blood sugar levels with symptoms of microvascular complications (p=0.002).The Microvascular symptoms that are often experienced by people with diabetes mellitus in the Community Health Center Jagir is symptomatic neuropathy complications . The proportion of patients with diabetes mellitus who received family support at 55.9%, while  the patients with diabetes mellitus who control the blood sugar levels well totaling 17 respondents. It is expected that people with diabetes mellitus to control eating diet and regular exercise. Keywords : Family support, blood sugar levels control, symptoms of microvascular complications.

Abstract

ABSTRACTThe increasing of life expectancy led to a growing number of elderly. There are many thing that occurs caused by aging process, one of them is decreases of body function. Decreasing body function can cause variety of health problems. The problem that may occurs are malnutrition, chronic disease, and lack of independence in performing daily activities. All of them are risk factor of mental emotional disorder. This study was conducted to determined the relationship between mental emotional disorders in the Elderly Care Unit Jombang in Kediri. This research was analytical study and used cross sectional. Dependent variable was mental emotional disorders. Independent variables were BMI, physical independence and characteristic. Interview was conducted to obtain information about variables in this study. Data analyzed by chi square and pearson correlation. The prevalence of mental disorders found in this study was 23,4%. Respondents who didn’t have physical independence were 4,3%. Respondents who had body mass index that wasn’t normal is 26,2%. The variables showed correlation only variable physical independence and mental emotional disorders (p = 0,008). While the body mass index and characteristic were not showed correlation with mental emotional disorders. The conclusion of this study was the elderly at risk for emotional suffering mental disorders. Physical independence associated with mental disorders in elderly emotional. So the need for more attention especially from the psychological to the elderly who do not have physical independence.Keywords: risk factors, BMI, physical independence, mental emotional                           disorder, elderly

Abstract

ABSTRACTChildren are the ones who are at risk of contracting tuberculosis (TB), but so far it has not been a priority. The proportion of tuberculosis in children case detection remains low among all tuberculosis cases. Bojonegoro is one among regencies in East Java which was appointed as the operational trial tuberculin test. The case detection begins with screening of tuberculosis suspects. Screening of tuberculosis suspected children by health centers officer will determine how much the cases of tuberculosis in children are found in health centers. Health officers at the health centers became the most dominant factor in screening of tuberculosis suspected children. Training about tuberculosis in children was done in 16 health centers among 36 health centers in Bojonegoro. This study aimed to analyze the effect of training to the screening of tuberculosis suspected children by health center officers in Bojonegoro. This study is observational analytic with cross-sectional design. The sampling technique is simple random sampling with respondents consisting of 29 doctors and 29 nurses who are responsible for tuberculosis programs in 29 health centers in Bojonegoro. The variables studied were the training and screening of tuberculosis suspected children. The results showed that screening of tuberculosis suspected children was inappropriate from the target (75.9%) and officers who have not attended the training of tuberculosis in children were 58.6%. Logistic regression test showed that there was effect of training to the screening of tuberculosis suspected children (p = 0.019) and prevalence ratio showed that health officers who was attended the training of tuberculosis in children were able to do the screening of tuberculosis suspected children well 8,50 times greater than those who didn’t. The training for the officers who have not attended the training of tuberculosis in children needs to be done, so that screening of tuberculosis suspected children in Bojonegoro can be optimally implemented.Keywords: screening, suspected, training, tuberculosis, tuberculosis in children

Abstract

ABSTRACTDiphtheria is caused by Corynebacterium diphteriae. Children aged 2-10 years are a vulnerable group contracting. Diphteri crawl probable case is difficult, so there are still children who died from diphteri every year. Cadres holds a very important role in the field of public health services so that its presence should be maintained. Level of activity the role of cadres is very dependent on the LKMD role as a manager, and also the public community. This study aims to analyze the factors which associated with a cadres active role in detecting diphteri probable case in Sidoarjo health center. Type of this study is an observational analytic with cross sectional design. Population of this research was all cadres in Sidoarjo health center as much as 516 people. Samples are taken by simple random sampling of 103 respondents. The data was collected using a questionnaire research instruments (questionnaire). The variables of this study is the role of an active cadre to crawl probable case of diphtheria, internal factors (knowledge, attitudes of cadre), external factors (community support, support for community health centers, training of cadres). Techniques of data analysis was done by logistic regression.The results showed 75% of cadres in Sidoarjo health center is active in detecting diphteri probable case. Statistical analysis showed that the internal factors that affect the active role of cadres is the attitude of p = 0.024, external factors are affecting the support of community leaders with p = 0.022, p = 0.042 support community health centers, training of cadres p = 0.041. The advice that can be given is expected that the cooperation between the cadres, community leaders, health center personnel, in support of efforts to increase the capacity and activity of cadres in helping in the field of public health services.Keywords: diphtheria, crawl probable case, cadre, activeness, PHC in Sidoarjo

Abstract

ABSTRAK Terapi intravena merupakan salah satu prosedur invasif yang bertujuan untuk mensuplai cairan, obat, vitamin, komponen darah, dan monitoring status hemodinamik. Pasien yang mendapatkan terapi intravena dalam jangka panjang berisiko tinggi terinfeksi, plebitis dan ekstravasasi vena. Penelitian ini bertujuan untuk menganalisis pengaruh karakteristik pasien terpasang kateter intravena terhadap kejadian plebitis. Desain penelitan ini adalah case control dengan besar sampel 45 pasien pada kelompok kasus dan kontrol. Sampel kasus pada penelitian ini adalah pasien yang terdiagnosa plebitis sedangkan sampel kontrol adalah pasien yang tidak terdiagnosa plebitis di RSU Haji Surabaya pada bulan Januari sampai dengan April 2017. Variabel independen adalah umur, jenis kelamin, status gizi, hipertensi dan DM, sedangkan variabel dependen adalah kejadian plebitis. Pengelolahan data menggunakan analisis regresi logistik berganda. Hasil penelitian ini menunjukan bahwa dari 5 faktor terkait dengan karakteristik pasien hanya 3 faktor yang berpengaruh terhadap kejadian plebitis adalah  jenis kelamin dengan p value = 0,043 dan odds ratio = 3,45, umur dengan p value = 0,016 dan odds ratio 4,10 dan  DM dengan p value = 0,000 dan odds ratio = 9,78. Sedangkan 2 faktor yang tidak berpengaruh terhadap kejadian plebitis adalah status gizi dengan p value = 0,74 dan odds ratio = 0,79 dan hipertensi dengan p value = 0,178 dan odds ratio = 2,35. Berdasarkan hasil analisis data, dapat disimpulkan bahwa faktor dominan dan berpengaruh terhadap kejadian plebitis adalah status DM. Hasil penelitian ini diharapkan dapat menjadi acuan bagi RSU Haji Surabaya, terutama dalam prosedur pemasangan dan perawatan terapi intravena yang perlu mempertimbangkan kondisi pasienKata Kunci : Karakteristik, intravaskular, pengaruh, plebitis, terapi intravena.ABSTRACTIntravenous therapy is one of the most common invasive procedures used for injecting  fluids, drugs, blood products, nutritional and monitoring of hemodynamic status. The insertion and daily use of these devices isassociated with risk plebitis and complications that can have impact on the clinical status and outcome of the patient.. The aims of this research was  to analyze the effect of patient characteristics on intravenous catheter to the occurrence of plebitis. This study used case control with sample size 45  for each group. Case sample was patients who diagnosed with plebitis while control samples was diagnosed patients with no plebitis at RSU Haji Surabaya in January until April 2017. Independent variables were age, sex, nutritional status, hypertension and DM, whereas the depenendent variable was plebitis occurrence. Those variables was analyze with  logistic regression. The results of this study showed that the 5 factors related to the characteristics of patients only 3 factors that affect the incidence of plebitis include the gender with p value = 0.043 and odds ratio = 3.45, age with p value = 0.016 and odds ratio 4.10 and DM with p value = 0.000 and odds ratio = 9.78. While the 2 factors that have no effect on the occurrence of plebitis is the nutritional status with p value = 0.74 and odds ratio = 0.79 and hypertension with p value = 0.178 and odds ratio = 2.35. Based on the results of data analysis, it can be concluded that the most dominant factor and the effect on the occurrence of plebitis is DM status. The results of this study are expected to be a reference for RSU Haji Surabaya, especially in the installation procedure and treatment of intravenous therapy that needs to consider the condition of the patientKeywords: Characteristics, influences, intravascular, intravenous therapy, plebitis.

Abstract

Diabetes mellitus (DM) is a diseases caused by less effective insulin use. East Java is the ten highest province for Diabetes prevalence all over Indonesia. DM patient risk higher for TB. This research aims to find out relationship between social-economy and nutrition status with TB occurrence to DM-type 2 sufferer in Kecamatan Tambaksari. This research is retrospective quantitative with case control design with age and sex corresponding. Sample of the research was counted by matching formula obtained out of 38 DM-TB sufferers for cases and 38 DM for control. Sample case and control were taken by simple random sampling. Characteristic result of DM and TB sufferers in Kecamatan Tambaksari 50% aged between 56–65 year. Amount of the sufferers 60,5% male. Education level is irrelated to the risk of TB occurrences in DM-type 2 sufferer, (p = 0.608 (> α= 0.05). Working status is related to the risk of TB occurrence to type 2 DM sufferer, (p = 0.022,OR = 3.2; 95%CI = 1.288-8.440). Waged level is related to the risk of TB occurrence to DM-type 2 sufferer, (p.= 0.009,OR = 13.214; 95%CI = 1,597-109.370). Nutrition status is related to TB occurrence to DM-type 2 sufferer, (p = 0.000, OR = 17.889; 95%CI = 4.653-68.772). The conclusion of this research is social economy factor, that is unemployed and wage < UMK, and nutrition status IMT < 18.5 related to TB occurrence to DM-type 2 sufferer. It needs to perform socialization for eating pattern regulation to DM-type 2 sufferer, TB screening to sufferer to DM-type 2 sufferer with nutrition status IMT < 18.5, and assistance for DM-type 2 with TB to have medical treatment routinely.Keywords: Tuberculosis, Diabetes Mellitus, socioeconomic, nutritional status
