Abstract

Abstract: Background: Electrical burns are caused by direct contact electric with the body, and often wound more serious than what is visible on the surface. Electric current can cause injury in three ways: Cardiac arrest (cardiac arrest) due to electrical effects on the heart; damage to muscles, nerves, and tissues by electric current through the body; thermal burns due to contact with the power source. The purpose of this study was to determine the profile of patients with electrical burns on BLU RSU Prof. Dr. R. D. Manado Kandou for period August 2009-August 2012. Methods: This study used a retrospective descriptive study method by the medical record data on BLU RSU Prof. Dr. R. D. Kandou Manado August 2009-August 2012. Results: The total cases of electrical burns are 40 cases, with 15 cases on period of August 2009 to August 2010, 9 cases on period of August 2010 to August 2011, and on period August 2011 to August 2012 found are 16 cases. Most incidents occurred in the age of 25-44 years with more incident occurred to men. Seen from the work, most incidence is occupied by the employees construction workers. Based on the strength of electric current, the highest incidence in medium voltage (domestic installations), which indicates that the housekeeping installation is not good, using of low quality cable, no electrical installation checks on a regular basis, using of cable rollers with excessive load, and using incorrect electrical tools. All of those items increasing the incidence rate. Conclusion: The accident caused electricity easily happen to anyone. Most of those who work in place are susceptible to electrical currents. On illegal electricity usage may also increase the risk of electricity. In children which less supervision from their parents can also occur. These facts required tights attention from all parties to realize unsafe power consumption and attention to safety themselves. Keywords: Profiles, electrical burns, handling   Abstrak: Latar belakang: Luka bakar listrik disebabkan oleh kontak langsung aliran listrik dengan badan, dan sering lukanya lebih serius dari apa yang terlihat di permukaan. Arus listrik dapat menyebabkan cedera dalam tiga cara: Henti jantung (cardiac arrest) akibat  efek listrik pada jantung; kerusakan otot, saraf, dan jaringan oleh arus listrik yang melewati tubuh; luka bakar thermal akibat kontak dengan sumber listrik. Tujuan penelitian ini adalah untuk mengetahui profil penderita luka bakar akibat listrik di BLU RSU Prof. Dr. R. D. Kandou Manado periode Agustus 2009–Agustus 2012. Metode: Penelitian ini menggunakan metode retrospektif deskriptif melalui penelitian data rekam medik di BLU RSU Prof. Dr. R. D. Kandou Manado Agustus 2009–Agustus 2012. Hasil: Keseluruhan luka bakar listrik sebanyak 40 kasus, yaitu 15 kasus pada periode Agustus 2009 ‑ Agustus 2010, 9 kasus pada periode Agustus 2010 ‑ Agustus 2011, dan pada periode Agustus 2011 ‑ Agustus 2012 didapatkan sebanyak 16 kasus. Insiden terbanyak terjadi pada umur 25-44 tahun dengan insiden terjadi lebih banyak pada pria. Dilihat dari pekerjaan terbanyak, insiden terbanyak ditempati oleh para pekerja buruh bangunan. Berdasarkan kuat arus listrik, insiden terbanyak adalah voltase menengah (instalasi rumah tangga), yang menunjukkan bahwa instalasi rumah tangga yang tidak baik, penggunaan kabel kualitas rendah,  tidak adanya pengecekan instalasi listrik secara teratur, penggunaan kabel rol dengan beban berlebihan, serta penggunaan alat-alat listrik yang tidak benar, menyebabkan insiden meningkat. Kesimpulan: Kecelakaan akibat listrik mudah terjadi pada siapa saja. Kebanyakan bagi mereka yang bekerja ditempat yang rentan dengan arus listrik. Kemudian pada pemakaian listrik illegal juga dapat meningkatkan resiko terjadinya listrik.Pada anak-anak yang kurang pengawasan dari orang tuanya pun dapat terjadi. Meningkatnya angka kejadian ini mendapat perhatian dari berbagai pihak untuk menyadari pemakaian listrik yang tidak aman dan memperhatikan keselamatan diri sendiri. Abstract

Abstract: Management of wound has improved together with development of medical sciences, especially biomolecular science and traumatology. Wounds, especially burn wound is prone to edema and infection, associated with a lot of free radicals. Due to tissue edema, hypoxia and hypoperfusion occur. Exposure of high pressure oxygen increase gamma interferron (IFN-γ) which further induces nitric oxide synthase (i-NOS) and VEGF. Besides that, IFN-γ increases the number of T helper 1 cells (TH-1) which influence B cells to produce IgG. Due to the increased IgG, the phagocytosis effect of leucocytes increase, therefore, it is assumed that hyperbaric oxygen therapy (HBO2) can reduce the degree of infected wound. This study aimed to analyze the effect of HBO2 on the numbers of bacteria in deep burn wound (2nd degree) in rabbits. This was a pure experimental study with a post test group design. Samples were 34 rabbits with deep burn wound (2nd degree) on their backs sized 2x1 cm. On day-5, bacteria cultures were obtained from all the wounds, and then the rabbits were divided randomly into 2 groups: treated with HBO2 and without; each group consisted of 17 rabbits. On day-10, the second bacteria cultures were done. The numbers of bacteria of the two groups were compared before and after HBO2. The results showed that distribution of bacteria in the two groups were as follows: Citrobacter freundii (34%), Citrobacter difersus (32%), Proteus vulgaris (13%), Citrobacter mirabilis (10.5%), and Staphylococcus aureus (10.5%). The Mann-Whitney U test showed a significant difference in the number of bacteria between the 2 groups before and after treatment (P < 0.001). The treated group showed a decrease of bacteria number. Conclusion: Hyperbaric oxygen therapy could reduce the number of bacteria in burn wounds.Keywords: burn wound, hyoperbaric oxyhen therapy, bacteriaAbstrak: Paradigma penatalaksanaan luka berubah seiring dengan perkembangan ilmu kedokteran, khususnya bidang ilmu biomolekuler dan traumatologi. Dalam bidang luka, terutama luka bakar, bagian tubuh mengalami edema dan infeksi. Pada bagian ini ditemukan radikal bebas dalam jumlah besar. Akibat edema jaringan terjadi hipoksia karena hipoperfusi. Paparan oksigen tekanan tinggi menyebabkan peningkatan interferon gamma (IFN-γ) yang menginduksi nitric oxide synthase (i-NOS) dan VEGF. IFN-γ meningkatkan sel T helper 1 (TH-1) yang memengaruhi sel B untuk menginduksi Ig-G. Dengan meningkatnya Ig-G, efek fagositosis dari leukosit juga akan meningkat, sehingga dapat diasumsikan bahwa hiperbarik mengurangi derajat infeksi pada luka. Penelitian ini bertujuan untuk menganalisis pengaruh terapi oksigen hiperbarik (TOH) terhadap jumlah kuman pada luka bakar derajat dua dalam pada hewan coba kelinci. Jenis penelitian ini eksperimental murni dengan desain post test kelompok. Penelitian dilakukan pada 34 kelinci yang diberikan luka bakar derajat dua dalam di daerah punggung dengan ukuran 2x1 cm. Pemeriksaan kultur kuman pada luka bakar dilakukan dua kali. Pada hari ke-5 diambil kultur dengan cara swab pada semua luka bakar di bagian punggung 34 kelinci, setelah itu kelinci dibagi secara acak menjadi dua kelompok yaitu yang menerima perlakuan TOH total 17 kelinci dan kelompok kontrol yang tidak menjalani TOH sebanyak 17 kelinci. Pada hari ke-10 setelah menyelesaikan TOH dilakukan kultur kuman pada luka bakar dengan cara swab pada kedua kelompok. Jumlah bakteri dibandingkan pada kedua kelompok sebelum dan setelah pengobatan. Hasil penelitian memperlihatkan berdasarkan distribusi jenis kuman pada dua kelompok dalam penelitian ini ditemukan Citrobacter freundii (34%), Citrobacter difersus (32%), Proteus vulgaris (13%), Citrobacter mirabilis (10,5%), dan Staphylococcus aureus (10,5%). Uji Mann-Whitney U menunjukkan perbedaan bermakna jumlah kuman antara kedua kelompok sebelum dan sesudah diberikan TOH dengan nilai P <0,001 di mana kelompok TOH menunjukkan penurunan jumlah kuman. Simpulan: TOH dapat mengurangi jumlah kuman pada luka bakar kulit.Kata kunci: luka bakar, TOH, kuman

Abstract

Abstract: Besides decompression sickness, hyperbaric oxygen therapy can be used as a therapeutic modality of several diseases inter alia wound healing in gangrene, compromised grafts and flaps, crush injuries, thermal burns, and intracranial abscess. This study was aimed to obtain the usage of hyperbaric oxygen therapy in healing process as indicated. This was a descriptive retrospective study using data of patients who had hyperbaric oxygen therapy performed on them at Prof. Dr. R. D. Kandou Hospital from 2013 to 2016. The results showed that there were 128 patients that had hyperbaric oxygen therapy performed on them,consisted of decompression sickness in 60 patients (46.87%), thermal burn in 29 patients (22.65%), diabetic ulcer in 19 patients (14.84%), followed by crush injury, skin graft, and pre-post amputation in 6 patients each (4.68%), and gangrene in 2 patients (1.56%). Conclusion: The usage of hyperbaric oxygen therapy for decompression was 46.87% and for wound healing was 50.13%.Keywords: hyperbaric oxygen therapy, wound healing Abstrak: Terapi oksigen hiperbarik (TOHB) merupakan salah satu modalitas terapi tambahan yang bisa digunakan sebagai modalitas terapi dari beberapa penyakit selain decompression sickness yaitu penyembuhan luka seperti pada gangrene, compromised grafts and flaps, crush injuries, thermal burns, dan intracranial abscess. Penelitian ini bertujuan untuk mengetahui seberapa banyak penggunaan terapi oksigen hiperbarik sebagai salah satu modalitas terapi pada proses penyembuhan penyakit yang termasuk dalam indikasi. Jenis penelitian ialah deskriptif retrospektif menggunakan data pasien yang melakukan TOHB di RSUP.Prof. Dr. R. D. Kandou periode tahun 2013-2016. Hasil penelitian mendapatkan 128 pasien yang menggunakan TOHB sebagai berikut: decompression sickness sebanyak 60 pasien (46,87 %), luka bakar sebanyak 29 pasien (22,65%), ulkus diabetik sebanyak 19 pasien (14,84%), diikuti crush injury, skin graft dan pre-post amputation masing-masing sebanyak 6 pasien (4,68%), dan gangren 2 pasien (1,56%). Simpulan: Penggunaan TOHB untuk dekompresi sebanyak 46,87% dan untuk penyembuhan luka sebanyak 50,13%. Kata kunci: TOHB, decompression sickness, penyembuhan luka

Abstract

Background: Frontoethmoidal encephalomeningocele (FEEM) is a congenital defect of the skull which poses many problems to the patient as it results in many craniofacial and neural morbidities. While recently surgical correction of this disease is done in a single-stage procedure, many in Indonesia still perform twostage surgery which bears more risks and is technically difficult to achieve good aesthetic results. This case series intend to assess the feasibility and convenience of teamwork approach between plastic surgeon and neurosurgeon in correcting FEEM in a single-stage operation. Methods:We reviewed 8 patients with FEEM treated in Plastic and Reconstructive Surgery Division, Cipto Mangunkusumo Hospital Jakarta from November 2005 until March 2010. Four of the cases were secondary cases from Neurosurgery Department, and the other 4 cases were treated in single-stage operation, in teamwork with Neurosurgery Department, using the Chula technique. Results of each surgery was assessed using objective parameters, which are Intercanthal Distance (ICD) and Interorbital Distance (IOD); and also subjective parameters which is aesthetic improvement.Result: All of the patients showed significant improvements in ICD and IOD measurements. No complication was found intra and post-operatively. All patients, especially the ones treated with singlestage surgery show good aesthetic results. Conclusion: To achieve goals of defect correction and aesthetically pleasant appearance, single-stage surgery in teamwork with the neurosurgery department seems to be most suitable and convenient.
