Abstract

The Indonesian Code of Medical Ethics or Kode Etik Kedokteran Indonesia (KODEKI) is a reference of moral values for the Indonesian physicians applied in their day-to-day practices. The aims of the present study were to obtain reflection response rate by the physicians on values included in the KODEKI along with its affecting factors. This is a cross sectional study among 400 clinical doctors at Jakarta Special Province. To measure reflection levels of KODEKI, we used KODEKI Reflection Questionnaire (KRK). Personal data sheet was used to obtain characteristic respondent and influencing factors reflection level of KODEKI. The prevalence of KODEKI reflection response rate in most subjects was moderate, i.e. 338 (84.5%). There was a significant correlation between the duration of physician mission time (p=0.01) and the main location of medical practice (p=0.02) against the KODEKI reflection response rate based on statistic tests. There was no significant correlation of factors against the KODEKI reflection response rate by the duration of time spent in consultation (p=0.10), doctor’s competence level (p=0.11), and the experience of having training/knowledge about medical ethics (p=0.71). The duration of physician mission time and the main location of medical practice were affecting factors to reflection level. Both factors had association with frequency of experience and exercise facing ethical dilemma.Keyword: reflection, The Indonesian Code of Medical Ethics, KODEKI Reflection Questionnaire

Abstract

Informed consent is a process of sharing information with patients essential to their ability to make rational choices among multiple options in their perceived best interest. This cross-sectional study was designed to investigate the association between consultation frequency and satisfaction between doctor-patients with information quality of informed consent in preoperative elective surgery patients from their perception, along with other factors. Ninetyeight subjects were included from Cipto Mangunkusumo General Hospital in Jakarta, Indonesia. Data obtained from interview included quality of information and Consultation Satisfaction Scale (CSS) and from medical record included medical background and consultation frequency. This study showed Frequency of consultation and CSS were significantly associated with the quality of information (p=0.048 and p<0.001, respectively). Education, socio-economic status, type of disease, and prognosis of disease were not associated with the quality of information (p>0.05). In conclusion, this study suggests consultation frequency and satisfaction in communication between doctor-patients are key factors in informed consent process.Keywords: consultation frequency, consultation satisfaction, informed consent, quality of information

Abstract

Basic principle of bioethics is one of medical ethics teaching material with cognitive approach in medical faculty. It is a relevant method to develop student’s reasoning analysis of medical ethics. The purpose of this study was to evaluate item analysis, reliability and validity of Basic Principles of Bioethics/Kaidah Dasar Bioetika Test (KDB Test) designed to measure level of cognitive of principle based of bioethics among medical student. This an evaluative descriptive study. The participant comprised of 99 medical students of Faculty of Medicine University of Riau academic year 2007. KDB Test was developed based on literature review and consultation with ethical expert. Item analysis of instrument was determined discrimination index and difficulty index. The validity of instrument was determined by measuring the face validity and content validity. Internal consistency reliability was examined using Kuder Richardson-20 (KR 20). The final version KDB Test showed r (coeffisien correlation to total) ranged between from 0.338 to 0.807 with level significant p<0.05 and internal consistency reliability (KR 20 = 0.701). The conclusion, KDB Test was reliable and valid instrument to measure level of cognitive among medical student.Keywords: Basic principle of bioethics, KDB Test, level of cognitive

Abstract

Adolescent is clearly a high risk group for drug abuse, and could threaten to the comunity. This cross sectional study was designed to investigate degree of problem to drug abuse and risk factors among senior high school students. Two hundred and ten students were included from 7 senior high school at Pekanbaru city. Multiple logistic regression was used to analyze risk factors. The degree of drug abuse was measured using Drug Abuse Screening Test (DAST-10) and self-reported questionnaires were administered to identify risk factors. This study showed that 67% of respondent did not have a problem related to drug abuse. There were significant association of sex, living with parents, smoking habit, friend smokes, and attending extracurricular activity (p<0.005) with drug abuse while parent’s marital status, academic performance and member of peer group were not (p=0.920, p=0.236, p=0.937, respectively). Regression analysis showed risk factors related degree of drug abuse were living of respondents (OR=3.969, p=0.002), active in extracurricular activity (OR=0.316, p=0.001) and smoking habit (OR=0.227, p=0.001).Keywords: adolescent, drug abuse, DAST-10, risk factors, senior high school students

Abstract

Autopsy is the scientific examination of bodies after death, where whole surface of the body as well as all the body cavities are explored to record the finding. While doing so, we have to collect all the possible findings which will help in establishing the circumstances leading to the death and also may help the law enforcing agencies. Growing amount of evidence supports the of virtual autopsy as complimentary to conventional autopsy and as study of choice situation where conventional autopsy is not performed. Abstract

Understanding of Principle based of bioethics, its can increase reaching higher degree of ability moral judgments. The purpose of this study was to investigate the association between Cognitive of Principles Based of Bioethics With Ability of Moral Judgments among Medical Student at Faculty of Medicine University of Riau. This cross-sectional study was conducted into two groups of medical student at Faculty of Medicine University of Riau that have had or have not had the knowledge of principle based ethics. Total respondents were 40 for each group, respectively. Score of cognitive of principles based of bioethics as a parameter was measured using Kaidah Dasar Bioetika (KDB) test and ability level was measure using Moral Judgment Test (MJT). This study showed that cognitive of principles based of bioethics were significantly associated with ability level of Moral Judgments (p=0.001). There were significant association of had got principles based of bioethics material in formal education to cognitive of principles based of bioethics and ability level of Moral Judgments (p<0.001 and p<0.001, respectively). There was no correlation between academic achievement level with knowledge level of principles based bioethics (p=1.170) and ability level of moral judgments (p=0.611). Abstract

(VeR) of injury for living victims is the most frequent form of assistance requested by investigating officers compared to other kinds of VeR. A VeR basically is a compilation and interpretation of a forensic medical examination as well as a physical examination on a routine medical examination. A VeR should not only fulfill the writing standard, but must also fulfill several terms and conditions for a court system. The objective of a forensic examination on injury cases is to know the type of wound, type of injuriy and degree of injury. This is meant to fulfill the formulation offence in Indonesian’s Penal Code (KUHP). Clinical judgment in defining a wound degree following a crime could be various, which could negatively affect the court sentence. A good Visum et repertum (VeR) of injury could be reached by understanding the medico legal aspect and determining the degree of injury severity.Keywords : Visum et repertum, injury, medico legal aspect, degree of injury

Abstract

Liquid silicone is an inert material utilized for tissue augmentation by physician as well as illegally by nonmedical personnel. We present a case report, 52 years old man who died after illegal silicone injection. From autopsy examination, we found needle mark at corona of the glans penis, asymetric enlargement of penis, oil substance in artery coronaria’s vessel and depth’s organs were oily. Histopathological examination showed silicone embolism in almost all of the victim’s organs. Abstract

Mechanical asphyxia is the most popular killing methods in infanticide. Infanticide by blunt forced injury is rare and those by sharp injury is less common. We present a case report, a newborn baby corpse, male, nine months of gestational age, life birth, no perinatal care signs and viable. From autopsy examination, we found blunt forced injury on the head, smothering, manual strangulation and also signs of asphyxiation in most internal organs. Stab wounds were found on the head and showed intra-vital sign, while the other stab wounds were found on the chest, which was inflicted after death.
